A real psychopathological condition is prevalent over social danger in urgent psychiatric hospitalization and in compulsory hospitalization as well. A decrease in the number of compulsory hospitalizations in the last few decades is due, among other reasons, to the connection between the in-patient service and the alternative methods of treatment and the development of the outpatient net of psychiatric care. The study included 390 urgent psychiatric hospitalizations; 5.9% of the patients were brought after the intervention of the Security Police and in 4.6% cases the patients were hospitalized against their will. In a broader sense, the reasons for the compulsory hospitalization of males were aggressive behaviour, especially of those under the influence of alcohol, and acute psychotic states. The analysis of the sex distribution has revealed that a greater number of male children was affected by the disease (p = 0.64). The average age ranged from 2 months to 3.5 years. Usually the first visible symptom is a white pupillary reflex, while the first clinical findings show that there is a tumorous formation in the fundus. {"title":"The left ventricular ejection time/filling period ratio as derived and simplified new index for noninvasive approximation of primary lusitropy disturbances in chronic hypertensive heart disease: a comparative Doppler-cardiographic and digital-echocardiographic study.","authors":"D Huml, M Beus-Huml, I Fazlagić","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A theoretical new Doppler-cardiographic index was developed for a noninvasive approximation of primary lusitropy disturbances from the peak filling/peak ejection rate ratio. This simplified index, the left ventricular ejection time/filling period ratio was directly extrapolated from the outflow and inflow sample volumes. An index was significantly lower in a group of hypertensive patients with global relaxation disturbance than in the group with \"partial\" lusitropy disturbance. Between this index (x) and the isovolumetric relaxation period (y) there exists a very tight linear correlation: y = -124 x + 148; r = -0.89. All patients had a pressure overload left ventricular hypertrophy and therefore a prolonged protodiastolic period from the aortic valve closure to the mitral valve opening without a significant difference between the groups of patients. There were significantly higher values of the isovolumetric relaxation period and the standard and normalised first time derivatives of protodiastolic left ventricular dimension changes in the group of patients with global relaxation disturbances. There were determined confidence limits for the appearance of primary lusitropy disturbances on the basis of defined parameter changes, the left ventricular ejection time/filling period ratio, the isovolumetric relaxation period and the standard and normalised first time derivatives of protodiastolic dimension changes. {"title":"[Use of extracorporeal circulation in the treatment of 2,4-dichlorophenoxyacetic acid poisoning].","authors":"Z Duraković, V Gasparović","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The paper presents the course of illness in four patients poisoned by the 2,4-dichlorophenoxyacetic acid. The first patient, 50 years old, attempted suicide by taking 400 ml of 40% solution of the 2,4-dichlorophenoxyacetic acid (2,4-D). He was hospitalized in a comatous state 6 hours after intoxication. He was treated by the extracorporal hemodialysis and the course of the illness was successfull. The second patient, 80 years old, drank up 100 ml of 40% solution of 2,4-D accidentally. He was admitted in a comatous state a few hours after intoxication. He was treated by hemodialysis and resin hemoperfusion (XAD-4) successfully. Before treatment, the serum 2,4-D concentration reached 177 mg/100 ml, with the 2,4-D clearance of 56.3 ml/min. The third patient, 24 years old, attempted suicide by taking 200 ml 40% solution of 2,4-D, but it remained questionable whether it was 2,4-D or paraquat intoxication. He was admitted to hospital 10 hours after intoxication. He was treated by hemodialysis and hemoperfusion methods (XAD-4) successfully. The serum 2,4-D concentration reached the amount of 122.5 mg/100 ml, and the clearance of 2,4-D was 85.4 ml/min. The fourth patient, 50 years old, drank 100-200 ml of 40% solution of 2,4-D accidentally. He was hospitalized in a comatous state 3 hours after intoxication. He was treated by the hemodialysis method successfully. The present study was undertaken to test the hypothesis that there is a significant correlation between the position of the wrist and metacarpophalangeal angulation in common rheumatoid hand deformities. Measurements were carried out on standard radiographs of the hands from two comparative groups: 50 adult patients (100 hands) with classical or definite rheumatoid arthritis and the control group of 40 adults (80 hands) suffering from no rheumatoid arthritis but coming for routine check up relating to some degenerative diseases. Streptokinase was given intravenously to 54 of them. Out of 54 patients, in 52% the localisation of the myocardial infarction was in the anterior and in 48% in the posterior wall. The patients were divided in the three categories according to the onset of symptoms: up to 2 hours, 2-4 hours and 4-6 hours. The dose of streptokinase was 1,500,000 i.u. during 1.5 hours. The CPK and MBCPK level reached the maximum values up to 12 hours and was to lowest in the first group. In 74% of the patients the serum enzyme level showed successful reperfusion of a coronary artery. Electrocardiographic findings of a successful myocardial reperfusion amounted to 36% in the first, 40% in the second, and 17% in the third group. Malignant ventricular arrhythmias appeared in the first group in 3.6%, in the second in 12.5% and in the third group in 14.2%. The criteria for successful myocardial reperfusion by echocardiography (analysed regional wall motion) were present in 52% in the first, in 50% in the second, and in 17% in the third group. Coronarography and ventriculography were performed in 17 patients after 21 days of treatment, with successful results in 88% (15 out of 17 patients). In a group of 176 patients unable to be treated with streptokinase because of the onset of symptoms of myocardial infarction (more than 6 hours) or because of contraindications for that treatment, the lethality rate reached 17.6%. The lethality rate in the group of 54 patients treated with streptokinase was 5.5%. Control animals were pair-fed on adequate diet. The feeding was continued for 180 days, and the effects on cerebral cortex and white matter lipid contents were studied. A generalized increase in the lipid phosphorus and lipid-N-acetylneuraminic acid (NeuAc) level was found. The increase was statistically significant for phosphatidylethanolamine and phosphatidylserine, ganglioside GM1, GD1a and GDIb in the cerebral cortex, as well as for phosphatidylinositol and phosphatidic acid in the cerebral white matter. {"title":"[The value of systematic monitoring of oxygen consumption in the diagnosis and therapy of septic shock].","authors":"V Pilas, M Cubrilo, V Bakula, S Vranjkovic, B Bakula, A Bilic","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In 31 patients with sepsis and multiple organic dysfunction, changes in the systemic oxygen consumption (VO2) during reanimation were observed in order to discover more objective indicators of the course and prognosis of the disease. In a prospective randomized study, 21 live (Group 1) and 11 dead patients (Group 2) were included. The investigation was based upon the application of the invasive tracing of oxygen hemodynamics and transport. The findings of the initially hypovolemic status were compared with those of the stabile normovolemic status obtained by the application of infusions and the blood volume substitute. In the early phase of the disease there were no significant differences in the clinical finding of the circulatory shock and the volume deficit of the circulated blood between these two groups of patients. Group 1 patients had lower values of the cardiac index (CI) and the systemic oxygen transport (DO2). In them there was a greater frequency of acute organic insufficiency, especially pulmonal, renal and hepatal. In the initial status VO2 decreased. In the normovolemic status of Group 1, a significant VO2 was found, while in Group 2 in spite of a DO2 increase and hemodynamics improvement, a more significant VO2 increase was not obtained. The impressions of the palates were made with optosil and these models were photographed. The photographs were analysed. The results indicate a significant genetic determination of the shape of the palate and the papilla incisiva and of the direction of the plicae palati. The following regime of treatment was applied: 3 days on levamisole (150 mg per day) followed by a 10 day interval without treatment, for a period of three months. The sperm count increased in 5 out of 6 patients and motility increased in 2. At the same time a reduction in seminal plasma IgG levels was observed in all treated patients.
