The results of BK virus DNA detection in urine and blood were observed after operation. They were divided into three groups according to different immunosuppressive induction regimens; 35 patients in group A, 42 patients in group B, and 32 patients in group C (basiliximab). To explore the effect of different immune induction regimens on BK virus infection in renal transplant recipients.Results: The positive rate of urine BK virus in all patients in 1 month after operation was 10.09% (11/109), which was significantly higher than that of blood BK virus 0% (0/109), and the difference had a statistical significance (p < .05). The positive rate of urine BK virus in all patients in 6 months after operation was 31.19% (34/109), which was significantly higher than that of blood BK virus 3.67% (4/109), and the difference had a statistical significance (p < .05). The positive rate of urine BK virus in all patients in 12 months after operation was 35.79% (39/109), which was significantly higher than that of blood BK virus 5.50% (6/109), and the difference had a statistical significance (p < .05). The urinary BK virus infection rate was increased significantly from 1 month to 6 months after operation, but was not increased significantly from 6 months to 12 months after operation. There was a statistically significant difference between the two groups (p < .05). The BK virus infection rate in renal transplant recipients induced by basiliximab within the first month was significantly lower than that in patients using polyclonal antibodies, but the urinary BK virus infection rate after one year was not significantly different from that in patients using polyclonal antibodies.Conclusions: There are slight differences in BK virus infection after early renal transplantation with different immune induction therapies, but there is no significant difference in the long-term. It is recommended to strengthen the early monitoring of BK virus after renal transplantation, timely adjust  immunosuppressive regimens to achieve the early detection and early treatment.","PeriodicalId":63789,"journal":{"name":"临床病例讨论杂志(英文版)","volume":"53 1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-06-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"75994999","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Right ventricular hypoplasia is usually associated with congenital anomalies of the pulmonary or the tricuspid valve, whereas biventricular isolated apical hypoplasia has never been described. We report the case of a 48-year-old man with no history of known cardiac disease who was found to have a complex cardiac abnormality characterized by: 1) Deficiency of the myocardium within the biventricular apex with adipose tissue infiltration; 2) Truncated right ventricle because of an absent trabecular portion of the inflow tract; 3) Truncated and spherical left ventricular apex; 4) Origin of the mitral papillary muscle in the flattened anterior left ventricular apex. Multimodality imaging was performed to delineate the morphological and functional characteristics of this cardiomyopathy fully. To the best of our knowledge, this is the first description of a new cardiac abnormality characterized by the hypoplasia of the apical region of both ventricles in the absence of valvular or coronary artery disease.","PeriodicalId":63789,"journal":{"name":"临床病例讨论杂志(英文版)","volume":"89 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-06-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"83860268","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is prevalent only in the Southeast Asian region. A 5-year-old boy was presented with diagnosed Acute Lymphoblastic Leukemia undergoing chemotherapy and suffering from high-grade fever for 15 days positively responded to antibiotics on the initial day, but later he became unresponsive. He was found negative for blood cultures of bacterial growth. Ultrasound of the abdomen showed a borderline enlarged liver with multiple small hypoechoic areas of variable sizes seen scattered in both lobes of the liver. Morphological and microscopic analyses of liver tissues showed multiple septate hyphae of P. marneffei. Amphotericin B was found to be effective in treating the child. Appropriate identification is essential to treat the pathogen-causing disease, especially in immuno-compromised conditions like a leukemic patient, and determine underlying mechanisms behind the pathogenesis.","PeriodicalId":63789,"journal":{"name":"临床病例讨论杂志(英文版)","volume":"83 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-04-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"75535068","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

TB can involve any organ system in humans; however, the disease affecting the cardiovascular system is rare. On the other hand, myocardial tuberculosis is a serious entity and can, unfortunately, be fatal if the diagnosis is missed or delayed. Therefore, the possibility of TB involving the myocardium should always be considered in areas with a high prevalence of TB, like the Indian subcontinent. Anti-TB therapy (with corticosteroids in some cases), along with early detection and suspicion, is proven to be quite effective in management and cure in most cases. We describe a case of sputum AFB (acid-fast bacilli) positive pulmonary TB with dilated cardiomyopathy, and impaired/decreased left ventricular systolic function, which gradually improved after anti-tubercular decongestive therapy.","PeriodicalId":63789,"journal":{"name":"临床病例讨论杂志(英文版)","volume":"19 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-03-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"74365553","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Therefore, the surgery’s meticulous planning is essential to avoid such complications. This case report describes a 63-year-old male patient diagnosed with an impacted lower third molar (tooth #38) with concurrent basal cell carcinoma of the right ear and squamous cell carcinoma of the right vocal cord treated with radiotherapy. Taking into account the patient’s health status, a minimally invasive osteotomy and a corono-radicular separation procedure were performed. This procedure allowed us to reduce the risk of mandibular ORN. Three years later, the healing was complete, and the situation was stable.","PeriodicalId":63789,"journal":{"name":"临床病例讨论杂志(英文版)","volume":"3 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-10-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"75152757","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
