Methods　 All 316 chronic HBV infection subjects including 119 with chronic hepatitis B(CHB),99 with liver cirrhosis(LC)and 98 with acuteonchronic liver failure(ACLF)were enrolled in this study.The differences of serum HBsAg and HBV DNA levels in patients with CHB，LC and ACLF，and in CHB patients with different degree of liver inflammation and fibrosis were compared.HBsAg was assayed with Roche electrochemical luminescence method.HBV DNA was quantified using a realtime polymerase chain reaction(PCR)assay.Liver histological inflammation(G)was graded on a scale from 0 to 4 and histological fibrosis(S) was staged on a scale from 0 to 4.Results　There in CHB，LC and ACLF，were no significant differences between HBsAg and HBV DNA level.Serum HBsAg level in G14 were(288.8±182.1)IU/ml，(264.8±175.6)IU/ml，(279.8±163.6)IU/ml，(317.8±150.0)IU/ml；HBV DNA were(5.0±2.1)log10 IU/ml，(6.1±1.7)log10 IU/ml，(5.6±1.8)log10 IU/ml，(5.8±1.7)log10 IU/ml，respectively.HBsAg level in S01，S2，S3，S4 patients were(234.7±182.5)IU/ml，(261.2±175.6) IU/ml，(305.8±149.2)IU/ml，(320.8±157.4)IU/ml；HBV DNA were(5.5±2.1)log10 IU/ml，(6.2±1.9)log10 IU/ml，(6.0±1.5 log10 IU/ml，(5.3±1.8)log10 IU/ml，respectively.No statistical differences were found among groups.The level of serum albumin in G3，G4 and S3，S4 decreased significantly comparing with other patients.Conclusions　 The degree of hepatic pathological changes did not parallel with serum HBsAg and HBV DNA level in chronic HBV infection patients.","PeriodicalId":63159,"journal":{"name":"中华临床医师杂志(电子版)","volume":"5 1","pages":"3799-3802"},"PeriodicalIF":0.0,"publicationDate":"2011-09-17","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70278908","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Dong, Zhong Li-yong","doi":"10.3877/536","DOIUrl":"https://doi.org/10.3877/536","url":null,"abstract":"Objective　To analyze and explore the clinical manifestations and possible pathogenesis in patients concomitant with autoimmune thyroid disease (AITD) and Moyamoya syndrome (MMS).Methods　Seven patients with MMS diagnosed were summarized by DSA and MRA from 2007 to 2011 in Tiantan Hospital.All these patients were diagnosed with AITD at the same time and also ruled out the common reasons for angiostenosis.Results　A total of seven AITD patients (two male and five female) concomitant with MMS were screened during four years.Five patients were associated with Graves′ disease while two patients were associated with Hashitoxicosis.Six patients were Hyperthyroid while one patient was hypothyroid.High level of TPOAb was found in six patients.Four patients were TRAb positive.High level of TGAb was found in five patients.All seven patients had typical stenosis or occlusion on terminal internal carotid arteries (TICA)/middle cerebral arteries (MCAs)/anterior cerebral arteries (ACAs)，five of these patients with smokelike vascular network in basal ganglia，and the other two patients accompanied with stenosis on bilateral proximal cerebral arteries (PCAs).Six patients with hyperthyroidism were treated with tapazole and bypass surgery to superficial temporal arteries and middle cerebral arteries，and one remaining patient with hypothyroidism was treated with Euthyrox and bypass surgery to superficial temporal arteries and MCAs.No patient developed complications during or after surgery and all patients recovered and were discharged.Conclusions　AITD concomitant with MMS may correlate with abnormal thyroid function and autoimmune response.It is necessary to perform thyroid function test and antibody screening to MMS patients.It is essential to treat thyroid disease in MMS patients who are treated by surgery.","PeriodicalId":63159,"journal":{"name":"中华临床医师杂志(电子版)","volume":"5 1","pages":"3523-3527"},"PeriodicalIF":0.0,"publicationDate":"2011-09-17","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70278820","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Jiahong, Huang Zhi-qiang","doi":"10.3877/439","DOIUrl":"https://doi.org/10.3877/439","url":null,"abstract":"Objective　To evaluate the social and economic benefits of day surgery laparoscopic cholecystectomy(LC/DS).Methods　Clinical datum of 1240 patients undergoing LC/DS and 476 cases undergoing routine LC at General Hospital of Chinese People′s Liberation Army from November 2009 to December 2010 were retrospectively studied.Rate of conversion to open procedure，complications，length of stay in hospital，medical costs and patient satisfaction rate were compared between patients received LC/DS and routine LC.Data was analyzed using SPSS 16.0，differences between two groups were assessed using t tests for continuous variables and chi-square tests for categorical variables.Results　The average age in patients of LC/DS group was 52.3 years.1236 cases were preformed by LC and conversion to laparotomy occurred in 4 patients，the conversion rate was 0.16% as two cases of gallbladder carcinoma were excluded.Six (0.48%) complications developed.The severe complications of bile duct injury and stomach or bowel injury did not happen in LC/DS group.The average age in patients of routine LC group was 51.9 years.Eight (1.68%) patients required conversion to the open technique.Eleven (2.31%) complications including one bile duct injury and one colon injury developed in routine LC group.None of the patients died as a result of LC in both LC/DS and routine LC groups.Rate of conversion to open operation was 0.16%and 1.68%(χ2＝11.2086，P＝0.0008) in LC/DS and routine LC groups respectively，complication was 6 and 11 cases (χ2＝11.706，P＝0.001)，length of stay in hospital was (1.6±0.6)and(4.8±0.9)days (t＝85.244，P＝0.0000)，medical costs was (8028±338)and(9016±266)Yuan (RMB)(t＝57.32，P＝0.0000)，patient satisfaction rate was 95.4% and 95.7% respectively (χ2＝0.125，P＝0.723).Conclusions　Implementation of the day surgery laparoscopic cholecystectomy resulted in a significant reduction in medical resource use，including a decreased length of stay and total cost of care.This procedure is worth promoting in China.","PeriodicalId":63159,"journal":{"name":"中华临床医师杂志(电子版)","volume":"5 1","pages":"2254-2257"},"PeriodicalIF":0.0,"publicationDate":"2011-06-18","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70278344","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods　76 STEMI elderly patients with high thrombus burden in culprit vascular undergoing primary percutaneous coronary intervention(PCI) were divided randomly into two groups-aspiration group and tirofiban group.All patients in two groups were treated by primary PCI,patients in tirofiban group were given tirofiban intravenous and Diver CE catheter was used to aspirate the thrombus in 39 patients of therapy group. Between two groups,the basline characteristics,angiographic results were compared.Results　In aspiration group and tirofiban group,the rate of slow-reflow phenomenon,TIMI frame count(TFC),myocardial blush grade(MBG),ST-segment resolution within two hours after operation was respectively 2.6% vs. 16.2%,48.1±10.3 vs. 63.2±14.3,2.59±0.48 vs. 1.68±0.43,84.6% vs. 62.2%. Patients in the aspiration group achieved better myocardial reperfusion. Methods　Cultured astrocytes of rats were divided into five groups randomly，normal control group， thrombin group and aprotinin pretreatment group. After twenty-four hours， the activity of lactate dehydrogenase was determined by chemistry reaction in the culture system. The viability of astrocytes (OD value) was measured by methylthiazolyl tetrazolium (MTT). The apoptotic cells in different aprotinin concentrations were detecte by TUNEL.Results　the OD values in the intervention groups of aprotinin (0.67±0.11， 0.78±0.07 and 0.80±0.13) were higher than the thrombin group (1.05±0.09) (P<0.05)； The LDH activities in the intervention groups of aprotinin (154.31±10.74， 101.38±13.59 and 92.72±11.92) were significantly lower than the thrombin group (267.09±25.35) (P<0.05).The apoptotic cells in the intervention groups of aprotinin (13.20±1.52， 9.10±1.20 and 4.80±0.89) was significantly lower than the thrombin group (26.10±1.82) (P<0.05).Conclusions　Aprotinin can alleviate the astrocytes damage induced by thrombin by increasing viability and decrease apoptosis of astrocytes， to achieve its protective effect.","PeriodicalId":63159,"journal":{"name":"中华临床医师杂志(电子版)","volume":"5 1","pages":"408-412"},"PeriodicalIF":0.0,"publicationDate":"2011-03-19","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70278424","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
