Atanassov","doi":"10.4236/ojoph.2021.114025","DOIUrl":"https://doi.org/10.4236/ojoph.2021.114025","url":null,"abstract":"Purpose: To determine the diagnostic value of the anterior segment optical coherence tomography (AS-OCT) indices in differentiating normal from ectatic corneas. Material and Methods: Two groups of patients—with corneal ectasia and normal controls were compared. Each group consists of 80 eyes of 43 age and sex-matched patients. All of them underwent corneal topography with OCULUS Keratograph 5M and corneal pachymetry with AS-OCT with RTVue-100. The indices generated by the AS-OCT pachymetric scans were analyzed. Results: There was a statistically significant difference for all the examined indices between the two groups with p values <0.001 and a confidence interval of 95%. The minimal corneal thickness (Min) was the best performing index according to the ROC analysis with an area under the curve of 0.976 and a combination of sensitivity and specificity of 0.925 and 0.911 respectively, and a “cut-off” value of 484 microns, followed by the indices of focal thinning—Min-Med and Min-Max with an area under the curve of 0.973 and 0.971 and sensitivity/specificity of 0.938/0.962 and 0.938/0.937 respectively. The rest of the examined parameters had an area under the curve in the range between 0.950 for the central corneal thickness and 0.814 for the outer superior segment. The external dacryocystorhinostomy with silicon intubation is the surgical technique with better long-term outcomes and performed in the institutions where this study was conducted. Can be performed under either general or loco-regional anesthesia, or a combination of both. Methods: This article presents a revision of 218 consecutive cases from December 2016 to June 2021 in two specialized centers with the same stan-dardized surgical and anesthetic technique. Results: 242 surgical prontuaries were selected, with 218 filling the inclusion criteria. 13% of the patients required additional anesthetic infusion before the beginning of the surgery; 2.7% of the patients required field anesthetic infusion during the surgery and 6.4% required supplementary sedation, thus meaning a positive outcome, as the number of patients with intraoperative complaints was low, and the overall comfort was high. During the follow up, no patient required pain medication after 24 hours. This condition affects young people between the ages of 20 - 50 years. The disease is seen predominantly in men as compared to women. We report the cases of two (02) patients received in consultation at IOTA-Teaching Hospital between July 2019 and February 2020. Case Presentation: We report two cases of Idiopathic stress central serous chorioretinopathy. The first case is a 37-year-old man that we received in consultation for visual acuity decrease from a sudden installation for 1 hour. The clinical and paraclinical arguments permitted us to retain the diagnosis of Idiopathic stress central serous chorioretinopathy. The patient benefited from laser treatment and the clinical evolution was good. The second case is a 42-year-old man, who consulted for a sudden reduction of visual acuity that had been evolving for 3 weeks. The clinical and paraclinical contexts were in favour of the diagnosis of Idiopathic stress central serous chorioretinopathy. The patient was treated with spironolactone. The clinical evolution was favourable. Conclusion: Our study confirms the existence of central serous chorioretinopathy (CSC) in Black African patients. It is highly challenging and often poses a diagnostic dilemma, causing delay in diagnosis and treatment. We report herewith the clinical and histopathology findings of a patient with an atypical presentation of acanthamoeba keratitis in Bahrain. The patient is a 16-year-old Bahraini teenager who was a cosmetic contact lens wearer. She presented with clinical signs and symptoms of microbial keratitis, which was initially misdiagnosed elsewhere as a case of herpetic corneal infection. Her corneal biopsy confirmed the clinical diagnosis as acanthamoeba keratitis. The patient was started on anti amoebic treatment. The infection got eradicated. The cornea healed with a central scar. Eventually, she underwent penetrating keratoplasty. This case report serves to raise awareness of this rare condition. Clinicians should have a high index of suspicion when diagnosing such cases among contact lens wearers. Articles in English and French were \u0000selected from 2010 to 2020. Results: We selected 05 articles on the epidemiology and management of DED in \u0000sub-Saharan francophone Africa. The prevalence of DED in sub-Saharan \u0000francophone Africa in the articles ranged from 7.5% to 90.4%. DED affects more \u0000women than men. The risk factors found were: age over 50 years, diabetes, meibomian \u0000gland dysfunction, prolonged use of computers, and glaucomatous anti-glaucoma \u0000drugs. The Test of Breack Up Time (TBUT) was commonly used for the clinical \u0000diagnosis of dry eye disease in Sub-Saharan Francophone Africa. The therapeutic \u0000strategy for DED was not mentioned in any article. Conclusion: DED, a multifactorial and blinding eye disease, is insufficiently \u0000explored by eye health professionals in sub-Saharan Francophone Africa. The main \u0000aim of this registry was to improve cataract surgery outcomes. Aim: The \u0000aim of this project is to offer a tool for benchmarking through the establishment of a reference database in Germany \u0000where surgeons could record and analyze their own outcomes. Methods: Data \u0000were collected between January 2018 and December 2018. The analyzed data \u0000included parameters of surgical technique, implanted intraocular lens (IOL), \u0000refractive and visual outcomes. Results: During the year of 2018, the \u0000German registry included 10,035 lens exchange surgeries and the complete \u0000follow-up was available for 9882 lens extractions. Approximately one third of \u0000the patients were between 76 and 80 years old and 60.6% of the cases had a \u0000best-corrected distance visual acuity (CDVA) before surgery of 0.5 or better. \u0000Parabulbar anesthesia was reported in 60.4% of the cases and \u0000phacoemulsification with implantation of a posterior chamber IOL was the chosen \u0000method in 92.2% of the cases. The power of the implanted IOL was between 21.0 - 22.9D in 29.4% of eyes. Surgical complications were \u0000reported in only 413 cases. After surgery, a CDVA \u0000of 0.5 or better was achieved in 90.8% of the cases. Most of the operated patients (64%) had a residual refractive error within ±0.5D (95% confidence \u0000interval 63.2 - 65.1). This study aimed to review and determine \u0000the distribution of biometry components, such as axial length (AL), anterior chamber depth (ACD), and lens \u0000thickness (LT) of pre-operative cataract patient in Indonesian \u0000population. Methods: A cross-sectional study data were obtained from \u00002015-2018 cataract surgery at Prima Husada Citra Hospital, Surabaya, East Java, \u0000Indonesia. A total of 1295 eyes \u0000with cataract was included in the study based on their medical record. We analyzed the \u0000data using descriptive analysis and correlated each variable using the Spearman’s Rho analysis. Results: Mean AL, LT and ACD was 23.81 ± 1.46 mm, 4.49 ± 0.55 mm, 3.25 ± 0.70 mm respectively. Male has longer AL and deeper \u0000ACD with (p p = 0.005). The increase \u0000of AL was accompanied by ACD (r = 0.457; p r = -0.101; p Conclusions: LT is increased with age while AL and ACD are \u0000shortened with age. Laser photocoagulation continues \u0000to be the treatment of choice in conjunction with the use of steroids and anti-angiogenics, \u0000but these treatments include possible ocular complications. The nutritional supplement \u0000Alzer (whose primary active ingredient is Ginkgo \u0000biloba, a powerful antioxidant that acts on vascular factors and oxidative damage, \u0000which are two of the mechanisms implicated in the pathogenesis of diabetic macular \u0000edema), which has been used on other non-diabetic macular conditions, along with \u0000the Diamel nutritional supplement has been shown to be effective on glycemic control \u0000and could represent a treatment alternative for mild to moderate macular edema by \u0000reducing the thickness of the macular retina and preventing the progression of other \u0000more advanced clinical presentations that are harder to treat. Objective: Identify \u0000the effect of Alzer along with Diamel in reduction of the thickness of the macular \u0000retina among patients with mild to moderate diabetic macular edema. Materials \u0000and Methods: A phase II double-blind clinical trial was conducted in 64 patients \u0000with non-severe diabetic macular edema over the course of non-proliferative diabetic \u0000retinopathy, who attended the ophthalmology service of the Institute of Endocrinology \u0000of Havana from January 2016 to December 2016. The treatment was randomly assigned \u0000to two groups: one received Alzer plus Diamel (n = 32) and the other group received Alzer placebo + Diamel \u0000placebo (n = 32). All patients \u0000were given an initial clinical evaluation, blood testing and ophthalmological evaluation \u0000at the start of treatment and after one year of follow-up. Results: There \u0000was a clinical improvement in the macular thickness upon the conclusion of the study \u0000in the patients treated with Alzer and Diamel. This decrease in thickness was statistically \u0000significant in the left eye. There was no decrease \u0000in visual acuity one year after treatment. Adverse events were mild and \u0000uncommon. Conclusions: Severe macular edema did not evolve in the Alzer \u0000and Diamel group. The clinical, but not statistically significant, success \u0000obtained in the experimental group proves the protocol hypothesis regarding the \u0000efficacy of the product being researched. The positive results in this small sample \u0000lead to the suggestion of performing larger-scale studies (Phase III). The cause of this type of cataract is \u0000preventable penetrating or blunt ocular injury. Lens extraction can improve the \u0000visual acuity but it also depends on the \u0000extent of the injury to other ocular structures. Objective: To describe the features of \u0000pediatric traumatic cataract presenting at a tertiary eye center in Indonesia. Methods: This study is a descriptive study and the data were collected retrospectively from the medical records \u0000of the patients who were diagnosed as pediatric traumatic cataract over the \u0000period of January 1st 2019 to December 31st 2019. \u0000Demographic data, trauma characteristics, clinical features, management, and \u0000pre- and post-operative Best Corrected Visual Acuity (BCVA) were reviewed \u0000retrospectively. Results: A total of 37 patients were diagnosed as \u0000pediatric traumatic cataract. Among these patients, 78.38% were boys, with the \u0000mean age of 9.14 ± 3.77 years old. Open globe injury was the \u0000mechanism of injury for 54.05% patients. Besides lens aspiration, additional \u0000procedures were membranectomy, anterior vitrectomy, primary posterior \u0000capsulotomy, and synechiolysis. Eighty-one percent patients had unilateral \u0000blindness preoperatively and 23.80% patients still had unilateral blindness on \u0000three months of follow-up. Conclusion: In pediatric patients, traumatic cataract occurred predominantly in boys \u0000while playing outside the house. The children who had ocular trauma still have \u0000the risk of blindness even after the surgery.
