The aim of this study was to evaluate the effect of umbilical cord derived Mesenchymal Stem Cell (UC-MSC) implantation on severity of Retinitis Pigmentosa (RP). Methods: This single-center, clinical study included data of 138 eyes of 92 patients who had a confirmed diagnosis of RP and received stem cell implantation to the suprachoroidal area with a surgical procedure. Patients were evaluated before and 1 year after the surgery regarding to the outcome measures of Best Corrected Visual Acuity (BCVA), Optical Coherence Tomography (OCT) and Visual Field (VF) tests. BCVA, VF width and ellipsoid zone (EZ) width on OCT were recorded for each patient and a scoring criterion was established for each variable varying from 0 to 5 depending on its distribution. The cumulative score (from 0 to 15) was used to classify disease severity from grade 0 to 5. Results: All of the patients completed 12-month follow-up period. The median age of the patients was 40.8 years, 46% were female, 77% had been diagnosed within 10 years and 41% had a family history. 79% of the patients with family history had autosomal recessive inheritance pattern. There were statistically significant improvements in the mean BCVA and VF scores during the study (p < 0.05). The mean score and the mean grade of the disease also improved after the treatment (p < 0.05). There was a negative correlation between BCVA improvement and scoring and grading of the disease. Conclusions: This study demonstrated beneficial effect of suprachoroidally applied UC-MSCs on BCVA, VF and the severity score and grade of the disease during 12-month follow-up period. Cell mediated therapy based on the secretion of Growth Factors (GFs) seems to be an effective and safe option for the treatment of degenerative retinal diseases. Multiple respiratory pathogens such as bacteria, viruses and fungi are potentially coexisted. A 60-year-old male with left eye post corneal transplantation developed acute severe pneumonia caused by Pneumocystis jiroveci (PJP) coinfection with Nocardia spp. and Cytomegalovirus (CMV). He was hospitalized due to acute respiratory failure. Chest radiographs and chest Computed Tomography (CT) revealed extensive ground-glass opacities. PJP was diagnosed from Bronchoalveolar Lavage Fluid (BALF). The pneumonia was persistent despite of receiving intravenous cotrimoxazole. Tracheal aspirate showed faint gram-positive filamentous beaded branching organisms. Consequently Nocardia spp. was proven. Intravenous cotrimoxazole was continued and intravenous imipenem was added. After a course of dual antibiotics, pneumonia was gradually improved. A week after, he developed the worsened acute respiratory failure. The bronchoscopy was performed. The new pathogens were not detected from BALF microbiology. The BALF cytology was unremarkable. PJP was detected by Polymerase Chain Reaction (PCR) from BALF. CMV antigenemia was detected from BALF and blood. Intravenous ganciclovir was given. This report describes PJP coinfected with Nocardia spp. and CMV in post corneal transplantation patient suffering from severe pneumonia. This technique is preferred because it has a simple procedure. Iris-claw IOL was originally designed to be fixated on the anterior chamber. The use of retropupillary fixation is increasing because the location is more physiologic and it shows less risk to corneal endothelial damage. Purpose: To describe safety and efficacy of iris-claw Artisan IOL in correcting aphakia without sufficient capsular support. Methods: This is a descriptive retrospective study of patients with aphakic iris-claw Artisan IOL implantation in National Eye Center Cicendo Eye Hospital, Indonesia from July 2017-July 2019. Patients were divided into prepupillary and retropupillary group. The Uncorrected Visual Acuity (UCVA), Best-Corrected Visual Acuity (BCVA), Safety Index (SI), Efficacy Index (EI), and complications were recorded. The procedure is safe if SI value ≥ 1.0 and effective if EI value ≥ 1.0. Results: There were 54 eyes in the retropupilary group and 17 eyes in the prepupillary group. In the prepupillary group, there were 94.11% eyes with SI ≥ 1.0, the mean SI was 1.79 ± 1.02, 50% of eyes with EI ≥ 1.0, and the mean EI was 0.77 ± 0.20. In the retropupillary group, there were 96.29% eyes with SI ≥ 1.0, the mean SI was 2.49 ± 2.23, 74.07% of eyes with EI ≥ 1.0, and the mean EI was 1.75 ± 1.64. Postoperative UCVA and BCVA were improved significantly compared to preoperative visual acuity in both groups (p < 0.05) Conclusion: Prepupillary and retropupillary iris-claw IOL implantation are safe. Brandt","doi":"10.4236/OJOPH.2021.112014","DOIUrl":"https://doi.org/10.4236/OJOPH.2021.112014","url":null,"abstract":"Purpose: To investigate the foveal avascular zone (FAZ) in obese by optical coherence tomography angiography (OCT-A) and to evaluate the findings of structural optical coherence tomography (OCT) and their relations with comorbidities. Methods: It was included 35 obese (study group) and 30 normal individuals (control group). Patients with retinal diseases and retinal treatments were excluded. The images were obtained using the Topcon ® . Results: The mean areas of FAZ in superficial plexus (FAZ-SP) and deep plexus (FAZ-DP) were significantly greater in the study group: FAZ-SP was 405.0 ± 136.4 µm 2 in the obese group and 307.3 ± 78.6 µm 2 in the control group and in the left eye (LE) 477.1 ± 124.4 µm 2 in the obese group and 384.0 ± 88.7 µm 2 in the control group. This difference was statistically significant (RE: p = 0.0014 and LE: p = 0.0012). The mean area of the FAZ-DP was 491.0 ± 124.4 µm 2 (Right eye—RE) in the obese group and 384.4 ± 88.7 µm 2 in the control group and in the left eye (LE) was 497.9 ± 124.1 µm 2 in the obese group and 484.9 ± 92.7 µm 2 in the control group. There were no correlations regarding FAZ-SP and FAZ-DP in both eyes with fasting blood glucose, glycated hemoglobin, total cholesterol and fractions and triglycerides. A significant association between enlargement of FAZ-DP and type 2 diabetes mellitus (p = 0.0160) was observed. Conclusion: The FAZ areas in superficial and deep plexus achieved significantly greater values in the study group. There was a significant association between a larger deep FAZ area and type 2 diabetes mellitus. Nosé","doi":"10.4236/ojoph.2021.114028","DOIUrl":"https://doi.org/10.4236/ojoph.2021.114028","url":null,"abstract":"Background: The implantation of the intraocular lens (IOL) is still subject to error and complication, as it can result in traumatic opening of the IOL lead-ing to rupture of the posterior capsule and zonular dialysis, it takes time to train paramedic teams to assemble such IOLs with the manual injectors. More-over, there is a potential risk of comtamination and endophthalmitis as there is manipulation of the IOL and cartridge. The preloaded IOLs tend to reduce those unwanted results and may optimize the surgical time. Purpose: The aim of this study is to compare the effectiveness and implantation time between three injectors and three intraocular lenses, two pre-loaded and one conventional. Methodology: Videos of thirty patients undergoing cataract surgery from December 2019 to December 2020 at the Hospital Oftalmológico de Brasília (HOB), Brasília, Brazil were included in this observational, analytical retrospective study, non randomized. All patients had their surgeries recorded, from which the time of injection and openi ng of the intraocular lens (IOL) was extracted, 20 eyes were implanted with preloaded intraocular lens, and 10 eyes with conventional IOL implant. The patients were time of the IOL) in relation to Hoya and Clareon lenses (39.50 s vs 19.60 s, p < 0.001); the total time of the Sensar IOL was 31.30 s. The opening time of the IOL optics was significantly longer for the Sensar One and Hoya groups compared to Clareon group, and the total time of Hoya group was significantly longer compared to the total time of the Clareon group. Conclusion: The study demonstrated that the choice of injector and IOL set can significantly affect the total time of IOL implantation. Face masks have been linked to dry eye complaints in recent studies. Purpose: To evaluate the changes in tear-film status and Ocular Surface Disease Index (OSDI) score following prolonged use of face mask. Design: Cross-sectional study. Method: Patients between 18 to 70 years using masks regularly for at least 2 hours/day for at least 5 days/week from different eye-care centres in Dhaka were included. All subjects were divided into 4 groups. Mask use lasted ap-proximately 2 hours/day in Group 1, 4 hours/day in Group 2, 6 hours/day in Group 3 and 8 hours or more/day in Group 4. Evaluation of symptoms, tear-film stability and tear volume was done by Ocular Surface Disease Index (OSDI) scores, Tear-film Break-Up Time (TBUT) and Schirmer-1 test. Average of right & left eye’s Tear-film Break Up time (TBUT) and Schirmer-1 value were noted. Results: Total 100 patients were enrolled (n = 100). The TBUT (p < 0.001) and Schirmer-1 measurement (p = 0.01) were significantly lesser and Ocular Surface Disease Index (OSDI) score were significantly higher in patients using face-masks for longer time (p < 0.001). It may involve the cornea, iris, zonula, ciliary body, choroid, retina and optic nerve. The incidence of this syndrome is 0.7 per 10,000 live-births [1]. The aim of this observation is to present the first case of bilateral coloboma of the iris [1]. This case was associated with an ametropia causing a decrease in visual acuity. Case Presentation: This was a clinical observation concerning a young 12-year-old patient who consulted for blurring of vision which had progressed for approximately 2 years. The ophthalmologic examination revealed an ametropia with a bilateral notch of the pupillary rim suggesting a bilateral coloboma. There was no association with another coloboma such as chorioretinal coloboma which is quite common and is accompanied frequently by visual symptoms. Conclusion: A visual impairment of the child can indicate ametropia. Methods: A structured Google form based cross-sectional survey was conducted among 678 employees compris-ing of medical, nursing, students and paramedical staff. The responses were evaluated in an Excel spreadsheet and tabulated. Results: 86% of the employees had awareness about eye donation, however, their knowledge regarding the same was either inadequate or incorrect. Only 63% had pledged their eyes and of the remaining, about 63.5% were willing to pledge. Conclusion: Awareness levels are high among the hospital workers but the lack of basic information is alarming.
