Our aim was to determine the prevalence of undiagnosed type 2 diabetes mellitus (T2DM) and to evaluate the performance of the Finnish Diabetes Risk Score (FINDRISC) questionnaire to identify type 2 diabetics in the urban population of Ouagadougou, Burkina Faso. Methodology: This was a cross-sectional study among volunteers aged 18 years and older recruited systematically from November 11 to 16, 2019, in the city of Ouagadougou, Burkina Faso, FINDRISC score was used. Analysis of the Receiver Operating Characteristic (ROC) curve was used to study the diagnostic performance of FINDRISC for the identification of type 2 diabetics. The optimal threshold was determined by the sum of the highest sensitivity and specificity. Multivariate logistic regression was used to analyze the association of each variable used in the calculation of the FINDRISC score. Results: A total of 1276 individuals were included in the analyses, of which 667 (52.27%) were women. The average age was 34.16 years (SD: ±12.42). The prevalence of T2DM was 10.74%. The mean FINDRISC score was 5.85 (SD: ±4.31). The majority (58.54%) of individuals had a low risk of diabetes according to the FINDRISC score 0 - 7, while 3.61% had a score ≥ 15. The FINDRISC score showed good performance (AUC = 0.70) in identifying undiagnosed type 2 diabetics. However, the variables in the score that best predicted the likelihood of being diabetic were age (p < 0.001), daily physical activity (p = 0.004), use of antihypertensive medication (p = 0.007) and waist circumference (p < 0.001). The optimal cut-off score ≥ 7 was the best predictor of the likelihood of having T2DM. Conclusion: The Finnish Risk Score (FINDRISC) is a good predictor of the risk of dysglycemia in Burkina Faso. It is a score to be promoted in daily clinical practice because it is easy to use, affordable and non-invasive. Further studies are needed to make modifications to the FINDRISC questionnaire in case it is applied to other ethnic groups.","PeriodicalId":60567,"journal":{"name":"内科学期刊(英文)","volume":"11 1","pages":"39-54"},"PeriodicalIF":0.0,"publicationDate":"2021-04-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48879065","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: North Dakota State University (Fargo, ND, USA) female and male competitive long-distance runners who agreed to participate during fall semester, 2019, completed an electronic survey containing the LEAF-Q and EAT-26 surveys and additionally completed a DXA scan, to screen for low energy availability and RED-S, disordered eating/eating disorders, and bone health, respectively. Participants were required to be 18 - 25 years old, not injured, training a mini-mum of five hours per week, and currently participating as part of a collegiate competitive team. Thirteen participants (male = 3) finished the study which also included energy intake and exercise expenditure self-reported with a three-day food diary and exercise log and estimated with ESHA Food Processor. Response rate was 42% of entire team; mean age 19.8. Results: The LEAF-Q significantly predicted risk for RED-S (p < 0.007). Correlations using the participants EAT-26 score, energy availability levels (p < 0.508), and whether they were at risk for RED-S (p < 0.208) or not were non-significant for all comparisons, although there was a moderate positive correlation between EAT-26 and RED-S risk (R = 0.454). Participants at risk for RED-S had higher occurrences of injuries (p < 0.022) and lower DXA measured Z-scores (p < 0.063) than those not at risk for RED-S. Conclusion: Long distance runners at risk for RED-S may have higher occurrences of injuries and lower bone mineral density, which can be easily measured using available screeners and DXA technology. What are the new findings: · Long distance runners at risk for RED-S may have higher occurrences of injuries and lower bone mineral density then age- and sex-matched individuals. How might it impact on clinical practice in the near future: · This is important for future clinical practice to be aware of the potential negative impacts on the bone development in young adults; · Particularly long-distance runners or athletes in leanness focused sports should be screened for energy availability which can impact performance, and future bone density.","PeriodicalId":60567,"journal":{"name":"内科学期刊(英文)","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-04-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"43931708","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We aimed to study the factors associated with prolonged poor diabetes control in the cohort of T2DM patients monitored and treated in the Department of Internal Medicine at the Yalgado OUEDRAOGO Teaching Hospital in Ouagadougou in order to optimize therapeutic education in these patients. Methodology: This was a descriptive and analytical cross-sectional study combining retrospective data collection from the last year of patient follow-up and prospective collection of some information. The study included all diabetic patients, aged at least 18 years old, followed and treated in the Department of Internal Medicine at the Yalgado OUEDRAOGO Teaching Hospital between January 1, 2010 and December 31, 2018 following a systematic random sampling with a sampling step of 10. The variables collected were sociodemographic, anthropometric, lifestyle, cardiovascular risk factors and diabetes-related characteristics. To determine the risk factors associated with prolonged poor glycemic control, we performed modeling using logistic regression. All variables associated with prolonged poor glycemic control, in bivariate logistic regression with a p-value less than 0.20 were included in the full model. Later, we used a stepwise descending method to obtain the final model, which was then tested by a receiver operating characteristic (ROC) curve. The significance threshold was set at 5%. Raw and fitted Odds-Ratio (OR) and 95% confidence interval were presented. Results: 270 patients were included. Prolonged poor control of diabetes mellitus was observed in 73.70%. The mean age was 55.97 years (SD: ±11.52) and the sex ratio was 0.6 in favor of female. The mean time since diabetes mellitus diagnoses was 5.85 years (SD: ±5.15). A monthly gain of 92.62 USD (50.74%) for average diabetes mellitus care expenditures of 55.82 USD (SD: 28.25) was reported. An overweight (55.92%) and hypertension (41.85%) were reported. Diabetes mellitus was complicated in 68.15%. Patients were supported by their families in the management of their diabetes mellitus in 85.19%. By extension, these strategies enhance the healthy development of the fetus. This study assesses the knowledge and practices of pregnant women towards intermittent malaria prevention strategies in the Juaben Government Hospital of Ghana. Methods: A cross-sectional, quantitative, descriptive survey was conducted among 120 pregnant women at the antenatal clinic of the Juaben Government hospital in the Ashanti region of Ghana using a simple random sampling technique in selecting the study participants. The participant response rate was 94.5%. Data were analyzed using Statistical Package for the Social Sciences (SPSS) version 22.0. Results: Majority (54.2%) of respondents were between 20 - 29 years of age. Majority of study respondents attended Antenatal visits in their first trimester of pregnancy. Majority (69.2%) have knowledge of malaria preventive strategies in pregnancy, with about 70% of respondents stating these strategies were designed only for pregnant women. Only 37.5% of study respondents could rightly identify the chemoprophylaxis indicated for malaria prevention in pregnancy. Whereas majority (93.3%) of respondents owned insecticide-treated mosquito nets, only 63.3% utilized these nets for their purpose. Barriers to utilization of insecticide-treated nets include difficulty in setting up the nets (26.7%), using mosquito coils (10.8%), and feeling uncomfortable sleeping in the nets (36.7%). Barriers to taking malaria prophylaxis include distance to health facilities (28.3%), thoughts of not being sick with malaria (55.8%), using herbs (10.0%), and not being aware of the essence of prophylaxis (7.5%). Conclusion: Knowledge of malaria preventive strategies in pregnancy is high. However, utilization of these strategies is low due to perceived human and institutional barriers. A comprehensive community-wide approach is required to improve the utilization of these services among pregnant women.","PeriodicalId":60567,"journal":{"name":"内科学期刊(英文)","volume":"11 1","pages":"27-38"},"PeriodicalIF":0.0,"publicationDate":"2021-03-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"42419089","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The objective of this descriptive study with retrospective collection was to describe the epidemiological and clinical profile of all autoimmune and au-to-inflammatory diseases in the department of internal medicine at the University Hospital Center of the Point G. Methods: This was a descriptive study with a retrospective survey of the records of patients hospitalized for autoimmune and auto-inflammatory diseases in the department of internal medicine at the CHU of Point G for a study period of 15 years from January 1, 2005 to December 31, 2019. We included in the study all patients hospitalized for autoimmune and auto-inflammatory diseases. Results: During the study period (January 31, 2005 to December 31, 2019), 6383 patients were hospitalized in internal medicine at the University Hospital Center of the Point G, of which 317 patients presented with autoimmune and/or auto-in-flammatory disease with an average annual hospital recruitment rate of 21 ± 7.87 cases per year. The female sex accounted for 64.98% with a sex ratio of 0.54. The mean age of patients was 35.27 ± 16.27 years and the extreme ages were 07 and 79 years. Out of the 317 medical records included according to our inclusion criteria, there were 07 cases of association between autoimmune disease and autoinflammatory disease, i.e. 14 cases of autoimmune and autoinflammatory diseases. A total of 331 autoimmune diseases and/or au-to-inflammatory diseases were collected, i.e. a frequency of 5.19%, including 291 cases of autoimmune diseases (221 cases of organ-specific autoimmune diseases and 70 cases of systemic autoimmune diseases) and 40 cases of autoinflammatory diseases (no case of monogenic forms, 08 cases of “systemic” polygenic forms and 32 cases of “organ-specific” polygenic forms). Organ-specific autoimmune diseases were dominated by type 1 diabetes (141 cases), Graves’ disease (48 cases) and systemic autoimmune diseases by systemic lupus erythematosus (43 cases), rheumatoid arthritis (16 cases). Among the auto-inflammatory diseases, the “systemic” polygenic forms were dominated by Horton’s disease (02 cases) and the “organ-specific” polygenic forms by gout (16 cases), ulcerative colitis (08 cases). Conclusion: It appears from our study that autoimmune and autoinflammatory diseases are characterized in internal medicine by their frequent occurrence in women and preferably between 25 and 44 years of age with very disparate distribution. We also observed a predominance of organ-specific autoimmune diseases over systemic ones, and “organ-specific” polygenic autoinflammatory diseases over “systemic” ones.","PeriodicalId":60567,"journal":{"name":"内科学期刊(英文)","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70664831","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
