The scapula is surrounded and supported by a complex system of muscles that work together to help in the arm movement. If some condition or injury causes these muscles to become imbalanced or weak, the position of the scapula can be altered at rest or in motion. A change in the scapular positioning or motion can make difficulties in the arm movement, especially when performing overhead activities and may cause shoulder weakness. Acupuncture treatment as part of the Traditional Chinese Medicine is most commonly used treatment for treating musculoskeletal disorders. In the treatment of scapular muscle fasciitis the fire-needle acupuncture gave the fastest and best results in our practice. The aim of the treatment is to break up the scar tissue and the adhesions which commonly form at the site, to relax the levator scapula muscle and restore local normal blood and energy flow. In the research are included 13 patients, 6 male and 7 female, on age from 24 to 70. All patients had scapular pain and on all was performed fire-needle acupuncture. Most common age groups are 50 - 60, most patients have pain on the right scapula and most of them—9 have done only one treatment. Tumors arising from the first and the second part of the duodenum (DI and DII, respectively) can be wrongly diagnosed as pancreatic mass. We report a case of a 58-year-old male patient with the history of vomiting just after eating, loss of appetite, lethargic and weight loss for 25 days. CT scan of abdomen and pelvis showed a soft tissue mass with slight heterogeneous enhancement at the region of the uncinate process of the head of pancreas and (MPR) on CT enhanced showed the closely attached mass to the head of the pancreas. On operative exploration, the mass was found to be originated from duodenum. Then, surgical resection of the duodenal tumor was performed by wedge resection successfully. The clinical presentation of a kidney stone includes fever, nausea, vomiting, acute flank pain radiating to the groin or the back. The pain is often described as stabbing and there is tachycardia, with or without hematuria for the severe patient. For the triad for urinary or kidney stones, some people say they are fever, vomiting, and acute flank pain. So in acute setting analgesia is given with or without an antiemetic to prevent vomiting IV fluids administered carefully. Noncontrast computed tomography (CT) is the gold standard for diagnosis. Most of urinary stones get washouts spontaneously if it is less than 5 mm without any intervention. However, if intervention is required either it is done by elective or as soon as possible by the intervention. {"title":"The Effect of Neoadjuvant Chemotherapy on ER, PR and HER-2 Expression in Breast Cancer","authors":"Z. Nie","doi":"10.4236/YM.2019.33017","DOIUrl":"https://doi.org/10.4236/YM.2019.33017","url":null,"abstract":"Objective: Through comparativing the change of the ER, PR and HER-2 expression in tumor tissue before and after neoadjuvant chemotherapy (NAC) in locally advanced breast cancer patient, to discuss whether there is influence for NACon ER, PR, HER-2 expression for breast cancer. Also, the relationship of ER, PR, HER-2 and chemosensitivity is also discussed. Methods: The expressions of ER, PR and HER2 in paraffin sections of breast cancer tissues from 51 patients before and after NAC were detected by immunohistochemical method. The assessment for the relationships between ER, PR, HER-2 and the efficiently of NAC is done. Results: For the 51 peoples, there is no statistical significance for change of ER, PR and HER-2 expression before and after NAC (P > 0.05). There is statistical significance for the differences of chemotherapeutic effectiverate between ER negative and ER positive, PR negative and PR positive, HER2 over-expressed group and HER2 non over-expressed group (P < 0.05). Pathological complete response (pCR) was related to molecular classification and the number of chemotherapy cycles. There is statistically significant or prognisis difference about hormone receptor, HER2 over-expressed and three-negative breast cancer (P < 0.05). Conclusion: NAC maybe could not change the expression of ER and PR, and HER2 for breast cancer patients. The patients with HER-2 over-expressed are not sensitive to chemotherapy, and the patients with ER or PR negative get more benefits from chemotherapy. Methods: A total of 102 undergraduate nursing students who were interned in our department from July 2016 to February 2018 were assigned randomly to the experimental group and the control group, 51 for each. The traditional teaching method was used in the control group for the training of intravenous indwelling needle infusion skills, whereas the micro-teaching method was used in the experimental group. Before the end of the internship, the two groups of interns were given a score test of intravenous indwelling needle infusion skills and an anonymous questionnaire survey on teaching effects. Results: The results of the skill test showed that the total scores of the control group and the experimental group were (76.73 ± 1.67) and (84.12 ± 1.90), respectively, and the difference was statistically significant (p < 0.01). According to the questionnaire survey, the percentage of the students in experimental group who had a positive attitude toward teaching methods, interest in learning, self-learning ability, and learning efficiency, was higher than that in the control group, and there was a statistical significance (p < 0.05). How to overcome missed diagnosis of meniscus tear in clinical setting? This review article highlighted the importance of early diagnosis and treatment; fortunately, these processes have been vastly improved by advances in Magnetic resonance imaging (MRI) and Arthroscopy. Although partial, subtotal and total meniscectomy is the choice for different types of meniscal tears, there are certain criteria to be meet for undergoing surgical treatment. The most common criteria for meniscal repair include: 1) A vertical longitudinal tear more than 1 cm in length located within vascular zone, 2) A tear which is unstable and displaceable into the joint, 3) An active informed and cooperative patient but younger than 40 years old, 4) A stable knee or wound stabilized with a ligamentous reconstruction. Chronically deformed or degenerative menisci are bottom-out candidates for repair. Most investigators report that only 10% to 15% of meniscal tears can be repaired and that most such repairs are done in association with an anterior cruciate ligament reconstruction. However, early diagnosis is difficult. Different biomarkers for early diagnosis of AKI, such as NGAL, KIM-1, Cys-C, IL-18, β2-microglobulin have been proposed. But regarding premature infant, there is very little research that has been reported. Objectives: To investigate the role of urinary NGAL and Kim-1 biomarkers for early diagnosis of AKI in premature neonates. Methods: Eighty cases were divided into experimental group (60 cases) and controls group (20 cases). Of 60 cases, 24 patients met the criteria for AKI group; the remaining 36 were non-AKI group. Blood and urine samples, 1 ml each, were collected from all subjects at first day, second day, third day and seventh day of life. Urine NGAL and KIM-1 were measured by ELISA. Results: The urinary NGAL and KIM-1 are significantly higher in AKI group than Non-AKI group and normal control group in the first three days. They present earlier than classical Scr.
