{"title":"The Healthcare Cost of Migraine: A Retrospective Cohort Study from Alberta, Canada.","authors":"Phuong Uyen Nguyen, Huong Luu, Helen So, Khanh Vu, Karen J B Martins, Werner J Becker, Farnaz Amoozegar, Thilinie Rajapakse, Lawrence Richer, Tyler Williamson, Scott W Klarenbach","doi":"10.1017/cjn.2025.40","DOIUrl":"10.1017/cjn.2025.40","url":null,"abstract":"<p><strong>Introduction: </strong>The economic burden of migraine is substantial; determining the cost that migraine imposes on the Canadian healthcare system is needed.</p><p><strong>Methods: </strong>Administrative data were used to identify adults living with migraine, including chronic migraine (CM) and episodic migraine (EM), and matched controls in Alberta, Canada. One- and two-part generalized linear models with gamma distribution were used to estimate direct healthcare costs (hospitalization, emergency department, ambulatory care, physician visit, prescription medication; reported in 2022 Canadian dollars) of migraine during a 1-year observation period (2017/2018).</p><p><strong>Results: </strong>The fully adjusted total mean healthcare cost of migraine (<i>n</i> = 100,502) was 1.5 times (cost ratio: 1.53 [95% CI: 1.50, 1.55]) higher versus matched controls (<i>n</i> = 301,506), with a predicted annual incremental cost of $2,806 (95% CI: $2,664, $2,948) per person. The predicted annual incremental cost of CM and EM was $5,059 (95% CI: $4,836, $5,283) and $669 (95% CI: $512, $827) per person, respectively, compared with matched controls. All healthcare cost categories were greater for migraine (overall, CM and EM) compared with matched controls, with prescription medication the primary cost driver (incremental cost - overall: $1,381 [95% CI: $1,234, $1,529]; CM: $2,057 [95% CI: %1,891, $2,223]; EM: $414 [95% CI: $245, $583] per person per year).</p><p><strong>Conclusion: </strong>Persons living with migraine had greater direct healthcare costs than those without. With an estimated migraine prevalence of 8.3%-10.2%, this condition may account for an additional $1.05-1.29 billion in healthcare costs per year in Alberta. {"title":"Biomarkers of Endothelial Activation in Delayed Cerebral Ischemia after Aneurysmal Subarachnoid Hemorrhage: A Prospective Cohort Study.","authors":"Maartje F Serlé, Maud A Tjerkstra, Bert A Coert, Rene Post, W Peter Vandertop, Dagmar Verbaan, Nicole P Juffermans","doi":"10.1017/cjn.2025.32","DOIUrl":"10.1017/cjn.2025.32","url":null,"abstract":"<p><strong>Background: </strong>Endothelial cell activation seems to be an important process in the multifactorial pathophysiology of delayed cerebral ischemia (DCI) and subsequent poor clinical outcome after aneurysmal subarachnoid hemorrhage (aSAH).</p><p><strong>Aim: </strong>To assess the association between biomarker levels of endothelial activation and the occurrence of DCI and poor clinical outcome six months after aSAH.</p><p><strong>Methods: </strong>Between October 2018 and November 2020, 75 aSAH patients were included. Blood samples were taken on admission, days 3-5 and days 9-11 after aSAH. Ten patients with unruptured intracranial aneurysms served as controls. Poor outcome was assessed at six months, defined by a modified Rankin Scale score of 4-6. The cohort was dichotomized into patients with and without DCI and good and poor outcomes. Biomarker levels of von Willebrand factor (vWF), E-selectin, thrombomodulin, syndecan-1 and matrix metalloproteinase (MMP-9) were analyzed and compared between groups by a <i>T</i>-test or Mann-Whitney <i>U</i> test, depending on the normality of the data.</p><p><strong>Results: </strong>Twelve (16.0%) patients developed DCI, and 39 (41.9%) patients had poor outcomes at six months post-aSAH. None of the biomarkers showed significant differences between patients with and without DCI. vWF and syndecan-1 were elevated on admission and on days 9-11 in patients with poor outcomes (<i>p</i> < 0.05 and <i>p</i> = 0.02, respectively).</p><p><strong>Conclusion: </strong>Levels of vWF, E-selectin, thrombomodulin, syndecan-1 and MMP-9 were not associated with the occurrence of DCI, although higher levels of vWF and syndecan-1 were associated with poor outcome at six months. Once implanted, the electrical stimulation parameters at each electrode contact can be modified. Iteratively adjusting the stimulation parameters enables testing for the optimal stimulation settings. Due to the large parameter space, the currently employed empirical testing of individual parameters based on acute clinical response is not sustainable. Within the constraints of short clinical visits, optimization is particularly challenging when clinical features lack immediate feedback, as seen in DBS for dystonia and depression and with the cognitive and axial side effects of DBS for Parkinson's disease. A personalized approach to stimulation parameter selection is desirable as the increasing complexity of modern DBS devices also expands the number of available parameters. This review describes three emerging imaging and electrophysiological methods of personalizing DBS programming. Normative connectome-base stimulation utilizes large datasets of normal or disease-matched connectivity imaging. The stimulation location for an individual patient can then be varied to engage regions associated with optimal connectivity. Electrophysiology-guided open- and closed-loop stimulation capitalizes on the electrophysiological recording capabilities of modern implanted devices to individualize stimulation parameters based on biomarkers of success or symptom onset. Finally, individual functional MRI (fMRI)-based approaches use fMRI during active stimulation to identify parameters resulting in characteristic patterns of functional engagement associated with long-term treatment response.
