However, precariousness remains poorly defined and detected by general practitioners because too few tools adapted to private practice are available. General practitioners, as the first point of contact for these patients and often the future doctors of the children to come, take charge of them or refer them to other practitioners. Their medical and social practices in this context are mostly undocumented. The purpose of this practice survey is to understand the underlying rationale and frame of reference according to which GPs identify precarious pregnant women and the way they take care of them.</div></div><div><h3>Method</h3><div>This is a qualitative study based on semi-structured interviews. They were thematically analyzed using the inductive method of grounded theorizing. Private general practitioners carrying out pregnancy follow-ups were enrolled via GP networks and then sampled according to the “<em>respondent driven sampling”</em>.</div></div><div><h3>Results</h3><div>Twelve interviews were conducted between September 2020 and April 2021. The analysis led to explore 6 main themes, poverty identification, feelings about it, the care of precarious women and the attitude, resources and training of the GPs.</div></div><div><h3>Conclusion</h3><div>The definition of precariousness by GPs is directly linked to their own representation of it. They work in formal or informal networks to avoid being isolated in the care that they describe as complex. {"title":"Parcours professionnel des gynécologues-obstétricien(ne)s après leur internat","authors":"Anaïs Turcsak, Chrystèle Rubod, Damien Subtil, Charles Garabedian, Louise Ghesquiere, Nassima Ramdane, Véronique Debarge","doi":"10.1016/j.gofs.2025.02.005","DOIUrl":"10.1016/j.gofs.2025.02.005","url":null,"abstract":"<div><h3>Objectives</h3><div>To analyze the career paths of gynecologists-obstetricians to identify the factors that lead them to stop practicing obstetrics or on-call duties.</div></div><div><h3>Material and methods</h3><div>A questionnaire was sent to 162 gynecologists-obstetricians who began their residency in the Hauts-de-France region between 2003 and 2014. The questions covered their personal life and professional background.</div></div><div><h3>Results</h3><div>The respondents were predominantly women (83%) with a median age of 37 years. The respondents mostly worked in the public sector (64%), full-time (77%), and practiced obstetrics (80%). Participation in obstetric activities significantly decreases over time (<em>P</em> <!--><<!--> <!-->0.001): 95% practiced it in their first position, 43% did so in their fourth position. Eighty percent still participated in on-call duties, but 40% were considering stopping (almost half of them within the next 5 years). Significant factors associated with stopping obstetrics included older age, being male, and practicing surgery. The cessation of on-call duties was significantly associated with age, number of children, involvement in assisted reproductive technology, medical gynecology, or private practice.</div></div><div><h3>Conclusion</h3><div>The obstetricians and gynecologists in this study still predominantly practice obstetrics and participate in on-call duties after a median of 7 years in practice. This retrospective observational study evaluates the benefits of therapeutic education in vaginoplasty, focusing on hospital stay duration, follow-up rate, and vaginal stenosis incidence.</div></div><div><h3>Materials and methods</h3><div>This study included all patients who underwent vaginoplasty between October 2018 and April 2021. A therapeutic education program, conducted by a specialized nurse, was introduced in June 2020, providing a control group (pre-June 2020) and a therapeutic education (TE) group (post-June 2020). Data were retrospectively analyzed, with the primary outcome being the length of hospital stay post-surgery.</div></div><div><h3>Results</h3><div>A total of 153 patients were included, with 121 in the control group and 32 in the TE group. The TE group experienced a significantly reduced hospital stay (5.56 days) compared to the control group (9.98 days), <em>P</em> <!--><<!--> <!-->0.001. Although no significant increase in follow-up rates was observed, a decrease in loss to follow-up was noted in the TE group. No significant reduction in the vaginal stenosis rate was found.</div></div><div><h3>Conclusion</h3><div>Preoperative therapeutic education for vaginoplasty appears to reduce hospital stay and reduce the loss to follow-up rate. The objective of this study was to establish the status of this practice in France and to depict the ethical issues around this problematic.</div></div><div><h3>Study design</h3><div>A questionnaire drafted by Maternal and Fetal Medicine physicians and family planning teams of the University Hospital from Strasbourg was distributed to the 48 French Multidisciplinary Prenatal Diagnosis Centers, among which 28 answered (58,3%).</div></div><div><h3>Results</h3><div>Embryo reduction in twin pregnancy on maternal request has already challenged 71% of the centers; 29% have performed such a reduction. The overall position of the centers to these requests is negative (3.1/10), with very mixed levels of in-team agreement. The main arguments against this practice are that twin pregnancy is not a pathology, that embryo reduction exposes to the risk of loosing the entire pregnancy, the feeling of being held hostage with the alternative of abortion of the whole pregnancy, and the lack of legal framing. On the contrary, the arguments in favor of the reduction are: that the reduction can avoid an abortion, that this type of reduction can be related to a partial abortion, that it responds to women's rights and that mental health is an integral part of women's health.</div></div><div><h3>Conclusion</h3><div>There is no consensus about how to respond to patients requesting for embryo reduction in twin pregnancy.
