Transgender (i.e. trans) youth initiating gender-affirming hormone treatment (estrogen for birth-assigned males, testosterone for birth-assigned females) after progressing through their endogenous puberty may experience impairment in gonadal function leading to infertility or biological sterility. Gamete cryopreservation (i.e. egg/sperm “freezing”) offers youth pursuing gender-affirming hormone treatment the potential for future biological parenthood. Aims: Given the dearth of qualitative research on trans adolescents considering fertility preservation measures, we sought to explore trans youth’s initial reactions to and opinions of currently existing technologies and their importance in medical decision-making. Methods: We conducted intensive semi-structured interviews with trans adolescents and young adults (n = 18) ages 15–24 about their gender-related medical care and decision-making processes around existing fertility preservation options. Results: In the course of analysis, three major themes recurred: (1) youth discussed a desire to decouple their reproductive biology from their gender, (2) youth felt limited by existing reproductive technologies and discussed imagined parenting futures enabled by anticipated scientific advancements, and (3) youth emphasized the role of their individual identity, personal experiences, and family relationships in decision-making about preserving eggs or sperm for future biological parenthood. Discussion: These findings suggest trans youth may have complex experiences with limitations presented by existing fertility preservation technologies. In these cases, healthcare experiences can have a significant impact on the transgender person’s perception of healthcare and future interactions with the healthcare system. Aim: To explore and systematically review the literature related to transgender peoples’ experiences and perspectives of general healthcare. Methods: A systematic search of MEDLINE, CINAHL, Scopus, Web of Science, Informit, and PsycINFO was performed according to established search strategies. Results were screened for eligibility in two parts. Titles and abstracts were screened for relevance, and then full-texts were retrieved and screened using eligibility criteria, resulting in 20 articles of primary research included in the review. Data were extracted from the studies and thematically analyzed to synthesize findings. Results: Four main themes were identified relating to healthcare provider (HCP) knowledge, HCP communication, relationship dynamics, and healthcare systems. These reflected a subtheme of perceived underlying cisnormativity and transphobia which influenced transgender patients’ interactions with the healthcare system. Participants also provided suggestions for improved care experiences. Discussion: Findings from prior studies and reviews about other transgender healthcare related topics describe similar themes and recommendations from participants. This reflects an impetus towards shared dialog and compromise between HCPs, policy makers and transgender patients to listen to each other’s experiences and work collaboratively to improve transgender healthcare. Although historically transgender people almost uniformly experienced discrimination from families of origin, recent research suggests that growing numbers of transgender people are supported by their families. Aims: The study reported in this article sought to examine the relationships between family support and discrimination, and psychological distress and resilience. Methods: A convenience sample of 345 transgender people living in North America completed an online questionnaire constructed by the authors. The questionnaire included demographic questions and single items questions about emotional closeness to family, gender-related support from family, and discrimination from family. The questionnaire also included standardized measures of gender-related discrimination, resiliency, social support, and psychological distress. Results: Participants reported moderate levels of gender-related family support, with non-binary participants reporting the lowest levels of gender-related family support. Participants whose families provided greater gender-related support reported greater resilience and lower levels of psychological distress; however participants who reported higher levels of gender-related discrimination from their families reported greater psychological distress. The findings suggest that emotional closeness to family may help mitigate the effects of general discrimination on psychological distress. Coleman, Leonardo Candelario Pérez","doi":"10.1080/15532739.2018.1498267","DOIUrl":"https://doi.org/10.1080/15532739.2018.1498267","url":null,"abstract":"Over the last couple of years, the World Professional Association for Transgender Health (WPATH) Standards of Care (SOC) have been translated from English into other languages (Arabic, Chinese, Croatian, French, German, Hindi, Italian, Japanese, Korean, Norwegian, Persian, Portuguese, Russian, Serbian, and Spanish). These translations were completed following the publication of Version 7 of the SOC and are available on the WPATH website (Coleman et al., 2011). The Chinese and Spanish translations of the WPATH SOC were among the first languages to be completed. The original English version limited the worldwide dissemination and application of the SOC. Thus, these translations (now numbering 15) are an important tool to promote transgender health in many areas of the world. While the Chinese translation is terribly important because of the size of the Chinese speaking world, the Spanish translation published in this issue of the International Journal of Transgenderism is equally important. It has been estimated that there are 427 million Spanish speaking people in the world, with Mexico having the largest Spanish speaking population followed by the United States and Spain (Simons, 2017). By 2050, the United States is estimated to have the world’s largest Spanish speaking population (Instituto Cervantes, 2016). Latin America has the largest number of countries that speak Spanish and have it as their official language (Simons, 2017). In addition to the size of the population and given the current environment and rapid changes occurring in Spanish speaking countries, this translation has the potential of scaling up transgender health care in an exponential manner. While professional transgender health care workers are reasonably well organized in the United States and Europe (including Spain), they are not as well organized in Latin America. WPATH has only a handful of members from Spain or Latin America. It is understandable that there are not many members from Latin America as WPATH holds its biennial conferences in English without any simultaneous translation and has never held one of its conferences in a Spanish speaking country (however, WPATH will hold its next conference in Argentina in 2018). Regional affiliates of WPATH have formed in Europe, Australia, Canada, and the United States, and most recently in Brazil. Unfortunately, there is not a Spanish speaking professional organization devoted to transgender health to provide a network of professionals working in this field, to develop professional skills, or to advocate for transgender health care. As a result, many professionals providing gender related care are probably not very familiar with the WPATH Standards of Care. There are however, a number of Latin American activist organizations that have fought effectively for transgender rights, including the right to health care (see Table 1 for a list of some of the major organizations). The present study aims to assess the relationship between psychopathological symptoms, defensive style, personality traits and negative attitudes toward transgender people. Methods: Participants (N = 165) were asked to complete several questionnaires assessing their psychopathological symptoms (SCL-90-R), defensive style (REM-71), maladaptive personality traits (PID-5-BF), antitransgender attitudes (ATTI), and sociodemographic characteristics. A four-step hierarchical multiple linear regression analysis was carried out to investigate the predictive role of such variables on antitransgender attitudes. Results: We found a significant negative effect of mature defenses and a positive effect of antagonistic traits on trans-negative attitudes. Political and religious beliefs predicted trans-negative attitudes, whereas no effect was found for the other predictors. We also found a significant effect of age and an interaction effect between sexual orientation and gender, with heterosexual men showing higher levels of trans-negativity than non-heterosexual men. Conclusions: The study highlights the importance of both personality and social constructs in predisposing to negative attitudes toward transgender people.
