Both cohorts had recorded data on iNO treatment. General cognitive ability was tested with IQ tests. Classification and Regression trees analysis was used to identify prenatal, perinatal and neonatal, clinical and social-environmental predictors of IQ.</p><p><strong>Results: </strong>In both cohorts, treatment with inhaled nitric oxide was not associated with IQ at age 5-6 years. Analysis identified maternal educational level, gestational age at discharge from hospital, intraventricular haemorrhage and maternal country of birth as important factors associated with IQ scores.</p><p><strong>Conclusion: </strong>Treatment with inhaled nitric oxide was neither negatively nor positively associated with IQ at age 5-6 years. {"title":"Variability and Need for Uniformity in Paediatric Care Guidelines in Belgium: A National Survey.","authors":"Elise Ruysschaert, Thomas Antychin, Eline Vermeiren, Levi Hoste, Anne Rochtus, Amber Van Baelen","doi":"10.1111/apa.70105","DOIUrl":"https://doi.org/10.1111/apa.70105","url":null,"abstract":"<p><strong>Aim: </strong>To determine the current variability in the use of clinical care guidelines across hospitals and evaluate the potential for uniform guidelines throughout Belgium.</p><p><strong>Methods: </strong>A survey, consisting of 48 questions covering demographics, use of evidence-based medicine, and opinions on guideline standardisation, was conducted among paediatric trainees and paediatricians. Statistical analysis included paired t-tests (significance p < 0.05) and thematic analysis to explore variations in guideline use across hospitals, departments, and doctors.</p><p><strong>Results: </strong>A total of 124 participants responded, with an even distribution in years of training. Sixty-three percent were currently working at a university hospital. Common sources for guideline implementation included UpToDate (80%) and local guidelines (58%). The largest differences reported were in the choice of treatment options. Trainees generally consider university hospitals to have a higher number and more up-to-date guidelines. Experience-based medicine is more commonly practised than evidence-based medicine. Ninety percent of participants were convinced that uniform guidelines would enhance patient care, improve the quality of training, and promote evidence-based treatment.</p><p><strong>Conclusion: </strong>This national study indicates significant differences between hospitals, departments and practitioners in guidelines used. We aimed to study the association of oxygenation instability of VLBW premature infants during the first week of life, as documented by the SpO<sub>2</sub> histogram, with short-term neonatal morbidities.</p><p><strong>Methods: </strong>VLBW infants requiring respiratory support were included. 24-h SpO<sub>2</sub> histograms were documented once daily. A classification system, based on the distribution of the histogram, was used to classify the histograms into stable/unstable categories.</p><p><strong>Results: </strong>Seventy-seven infants with mean (SD) gestational age-28.5 (2.1) weeks and birth weight-1040 (243) g were included in our study. 2063 histograms were recorded. Mean (SD) length of follow-up-27.5 (20.1) days per infant. 62% of infants remained stable during the follow-up period, while 38% experienced instability. 52% entered the unstable period during the first week of life. These infants had a higher risk of bronchopulmonary dysplasia (BPD)/death after adjusting for gestational age (OR [95% CI] 17.5 [4.0, 77.2]; p < 0.001), and for a variety of demographic and clinical variables (OR [95% CI] 32.4 [2.5, 418.5]; p = 0.008).</p><p><strong>Conclusion: </strong>VLBW premature infants demonstrate a distinct instability pattern. Entry into the unstable phase during the first week of life is associated with BPD/death, making it a potential predictive tool. A meta-analysis was also carried out.</p><p><strong>Results: </strong>Five studies with 2055 participants from the USA, Brazil, the Netherlands and China met the inclusion criteria. They were published from 2019 to 2023: three were prospective observational studies and two were retrospective cohort studies. Asymptomatic viral respiratory tract infections were detected in 17% of the participants. In one study, asymptomatic viral respiratory tract infections were associated with an increased risk of respiratory complications, and they were associated with an increased risk of mortality in another study. The pooled odds ratios had wide 95% confidence intervals and showed no statistically significant associations with respiratory complications, reintubation or mortality. The meta-analysis was limited by the low number and heterogeneity of the studies.</p><p><strong>Conclusion: </strong>This meta-analysis showed that asymptomatic viral respiratory tract infections were common among surgical paediatric patients. {"title":"Four-Fold Increase of the Number of Children Diagnosed With Paediatric Feeding Disorder in Northern Sweden in 2012-2022.","authors":"Elsa Gustavsson, Martina Kölgran, Maria Jonsson, Kajsa Lamm, Anna Chmielewska","doi":"10.1111/apa.70096","DOIUrl":"https://doi.org/10.1111/apa.70096","url":null,"abstract":"<p><strong>Aim: </strong>The aim of this study was to investigate the annual prevalence and characteristics of Paediatric Feeding Disorder (PFD) in Västerbotten county, Sweden, during the years 2012-2022.</p><p><strong>Methods: </strong>In this retrospective study, medical records of patients attending paediatric clinics with possible feeding problems were identified by applying pre-defined diagnostic and procedure codes. Diagnostic criteria of PFD were verified. Additional data including nutritional status and enteral feeding tube placement were retrieved.</p><p><strong>Results: </strong>Annual prevalence of PFD increased from 1.2 (95% CI, 0.9-1.5) per 1000 children in 2012 to 5.0 (95% CI, 4.4-5.6) per 1000 children in 2022. In 2022, the prevalence in children < 5 years of age was 10.2 (95% CI, 8.7-12.0) per 1000. Of those, 28% had thinness grades 2 or 3 (moderate or severe thinness), and 31% were fed via nasogastric tube or gastrostomy.</p><p><strong>Conclusion: </strong>Prevalence of PFD has increased in the last decade, most notably for children under the age of five. High rates of malnutrition and enteral feeding in our study population indicate that we identified only the most severe cases of PFD. AERP and mismatch responses to changes in duration or frequency in pure tones were measured at term-equivalent age using EEG. Neurocognitive outcome was assessed at 2 years of corrected age with Bayley scales of infant development. At 12 years of age, the subgroup of children born before 27 GWs (n = 18) were assessed with Wechsler Intelligence Scale for Children Fifth Edition (WISC-V).</p><p><strong>Results: </strong>Larger negative AERP amplitudes after 250 ms from stimulus onset to the standard and a negative rather than positive mismatch response to the frequency deviant were associated with higher Bayley score in univariable analysis. after adjustment for perinatal factors, only the duration mismatch response was associated with mental developmental index at 2 years of age. AERPs were not predictive of WISC-V test results at 12 years of age.</p><p><strong>Conclusion: </strong>AERPs at term in very preterm infants have a low prognostic value for short-term outcome. All parent-infant dyads were eligible for inclusion. The self-reported time present in the unit, lap holding, and skin-to-skin care (SSC) were used to measure parental closeness.</p><p><strong>Results: </strong>833 dyads of parents and infants were included (365 OBU, 140 mixed, 328 SFR phase). Parental presence increased for mothers from 210 min/day in the OBU to 420 min/day (p = 0.000) in the SFR, and for partners from 180 to 360 min/day (p < 0.001). Only for mothers, lap holding time increased from 100 min/day in the OBU to 120 min/day in the SFR phase (p = 0.046). SSC increased from 120 to 160 min/day (p = 0.002) for mothers and from 110 to 130 min/day (p = 0.004) for partners. These findings were independent of gestational age.</p><p><strong>Conclusion: </strong>Moving from an OBU to SFR setting increased parental closeness in the NICU.
