[Validation of the SERVPERF tool for quality analysis in emergency care with clinical ultrasound].

{"title":"[Validation of the SERVPERF tool for quality analysis in emergency care with clinical ultrasound].","authors":"Laura Carbajo Martín, Ignacio Párraga-Martínez, Juan Jesús Rosado Cabral, Luis Matías Beltrán Romero, Máximo Bernabeu Wittel","doi":"10.1016/j.aprim.2024.103204","DOIUrl":"https://doi.org/10.1016/j.aprim.2024.103204","url":null,"abstract":"<p><strong>Objective: </strong>To analyze the reliability and validity of the SERVPERF questionnaire for assessing the quality of care from the patient's perspective in emergency departments for abdominal pain patients undergoing clinical ultrasound, as well as the influence of sex.</p><p><strong>Design: </strong>Prospective study from March 2023 to April 2024 involving patients treated for abdominal pain who underwent clinical ultrasound.</p><p><strong>Setting: </strong>Emergency department.</p><p><strong>Participants: </strong>Adults over 18 years old with abdominal pain.</p><p><strong>Interventions: </strong>Conducting clinical ultrasound and collecting the adapted SERVPERF questionnaire.</p><p><strong>Main measurements: </strong>Overall score of the questionnaire, sex, age, length of stay in the emergency department, referral origin, discharge destination, ultrasound results.</p><p><strong>Results: </strong>A total of 115 patients were included, with a mean age of 58.2 years (SD: 19.4) and 66.1% being women. To validate the questionnaire, the Bartlett's test of sphericity yielded a value of 0.000 (P<.05) and the KMO (Kaiser-Meyer-Olkin) test resulted in a value of 0.855 (>0.5). The reliability of the questionnaire was assessed using Cronbach's alpha: 0.928 (95% CI: 0.908-0.946). The mean score on the scale was 124.57±9.30. Men showed a higher overall mean score than women, approaching statistical significance (P=.05). Using multiple linear regression, no variable was independently associated with a higher perception of quality in care, although sex was marginally significant.</p><p><strong>Conclusions: </strong>The SERVPERF questionnaire is shown to be reliable and valid for determining the perceived quality of service. Additionally, the perceived quality of care using clinical ultrasound in the emergency department was very high. The causes and associated risk factors are analyzed, with more than 90% of low back pain cases being nonspecific and closely related to biopsychosocial context, as well as the role of imaging tests, whose findings have not been proven to be the source of low back pain. SITE: the work was carried out in Barcelona.</p><p><strong>Participants: </strong>The design of the sample was theoretical, and the participants were segmented taking into account the socioeconomic level and whether or not they had suffered any relapse.</p><p><strong>Method: </strong>It was set up six discussion groups of 6 to 12 participants each. A thematic analysis of the literal transcription of the audio files of the focus groups was done.</p><p><strong>Results: </strong>It has been found that the consequences of suffering breast cancer are not the same for those women who are self-employed as for those who work for someone else. However, tasks that require physical effort are difficult to continue doing, but those that require concentration and more intellectual effort are also difficult to perform too. It is considered a serious disease due to the presence of pain and reduced functional capacity. Moreover, there are no disease-modifying drugs or curative treatment at present; the basic or first-line treatment is non-pharmacological, based on education and physical exercise. The presence of degenerative changes in imaging tests is present in a high percentage of the general asymptomatic population, i.e. in those who have no pain or functional limitation, and this percentage increases progressively with age. In short, osteoarthritis is part of the normal aging process of the population. As with low back pain, it is proposed to apply a salutogenic model of health, paying special attention to the terms used when informing and advising patients. SITE: Primary Care (two Health Areas in Extremadura).</p><p><strong>Participants: </strong>A total of 1,422 patients were included (mean age 45.6 years; 53.2% women) who had the ICPC-2 diagnostic code for \"confirmed SARS-CoV-2 infection\" recorded in their clinical history during the first six months of the pandemic.</p><p><strong>Interventions: </strong>Not necessary.</p><p><strong>Main measurements: </strong>Clinical and sociodemographic characteristics as well as outcome of patients (hospital visits, admissions and mortality).</p><p><strong>Results: </strong>The mean age (50.8 vs. 42.3 years, p<0.001), and prevalence of most of the comorbidities, dependent patients (13.8% vs. 4.0%), and residents in social care institutions (15.4% vs. 3.1%) were higher in the Don Benito-Villanueva area than in Badajoz. The predominant age group was 19-49 years (44.4%). 41.4% of patients were actively employed, mainly in National Classification of Occupations groups 2, 5, and 9, while 16.5% were social healthcare professionals. 16.5% of patients visited the hospital, 13.4% required hospitalization. Among those who consulted in hospital emergency departments independently, 46.2% were hospitalized, compared to 78.8% of those referred from primary care (p=0.000). The overall mortality rate was 2.0% (3.1% in Don Benito-Villanueva versus 1.3% in Badajoz; p=0.016) increasing to 8.9% among hospitalized patients.</p><p><strong>Conclusions: </strong>Sociodemographic and clinical differences were noted between the two health areas.
