This study explored the lived experience of self-care among hospice nurses using an existential-phenomenological approach. Hospice nursing, characterized by its focus on comfort care and end-of-life support, presents unique emotional, physical, and ethical challenges that often lead to burnout and compassion fatigue. Phenomenological analysis revealed 4 central themes: Threads of Trust: Weaving Meaningful Connections; A Full Heart, an Empty Cup; When Caring Hurts: The Emotional Cost of Hospice Nursing (with 2 subthemes-The Cost of Grief and The Stigma and Misunderstood Nature of Hospice Work); and The System Forgot the Caregiver. Participants highlighted the emotional strain of hospice work. The participants shared their lived experiences, emphasizing the emotional demands of their work, sparse self-care practices, and a lack of organizational support. The findings suggest that practicing self-care must be augmented by other actions such as systemic changes, caseload limits, leadership training, and structured emotional support systems. This study highlights the urgent need for health care systems to prioritize hospice nurses' well-being and help sustain compassionate nursing care.
","PeriodicalId":54807,"journal":{"name":"Journal of Hospice & Palliative Nursing","volume":" ","pages":"187-193"},"PeriodicalIF":1.3,"publicationDate":"2026-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145710154","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Unmet PC needs can delay establishing goals of care, increase the length of stay, and decrease patient and family satisfaction. Nurses can best advocate for appropriate PC referrals when given adequate resources and tools. Nurse-driven screening results in more appropriate PC recommendations. The project was a descriptive pre/post evidence-based practice intervention design. An electronic survey on comfort and knowledge of PC was distributed to neurocritical ICU nurses before and after an educational intervention regarding the definition of PC and the use of the Center to Advance PC's ICU screening tool. Nurses' comfort when assessing for PC consults increased from 62.5% to 91.7%, and for requesting consults from a physician, from 58.4% to 75%. Nurses' knowledge of PC being compatible with aggressive treatment increased from 41.7% to 95.8%. Nurses recommended PC in 66.7% of patients based on 3 or more screening criteria selected. The screening tool highlighted that more than 62% of patients in the neurocritical ICU had a PC need on admission. Implementing a screening tool and educational intervention can increase nurses' comfort and knowledge in recommending PC consults. The screening tool effectively identified patients with PC needs, empowering nurses to advocate for appropriate PC referrals.
","PeriodicalId":54807,"journal":{"name":"Journal of Hospice & Palliative Nursing","volume":" ","pages":"E186-E194"},"PeriodicalIF":1.3,"publicationDate":"2026-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148266491","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Morphine sulfate remains the standard pharmacological treatment for refractory EOL dyspnea, effectively modulating central respiratory drive and alleviating breathlessness perception, distinct from its use in chronic stable breathlessness, where recent evidence shows limited benefit and potential harms. Many nurses, however, lack adequate knowledge and confidence in administering morphine appropriately due to misconceptions about addiction, respiratory depression, and hastening death. This quasi-experimental study evaluated a brief online training module on morphine administration for EOL dyspnea among registered nurses. Of 69 enrollees, 34 provided complete pre- and post-intervention data. Paired t tests revealed significant improvements in knowledge and self-efficacy. Understanding the factors that influence nurses' attitudes toward caring for the dying is essential, as these attitudes directly affect the quality of end-of-life care. The aim of this study was to examine the associations between death anxiety, spiritual care perceptions, and attitudes toward caring for the dying among palliative care nurses. This cross-sectional study was conducted in the palliative care clinics of 5 public hospitals in Turkey between May and November 2023. Data were collected using a participant information form, the Thorson-Powell Death Anxiety Scale, the Spirituality and Spiritual Care Rating Scale, and the Frommelt Attitude Toward Care of the Dying Scale. Participants demonstrated moderate levels of death anxiety, high levels of spiritual care perceptions, and positive attitudes toward caring for the dying. Spiritual care perceptions (β = 0.525), educational level (β = 0.245), end-of-life care training (β = 0.216), and experiences of patient death (β = -0.231) were significant predictors of attitudes toward caring for the dying ( R ² = 0.609, P < .001). To address these challenges, the Acute Basic Palliation Concept was developed to enable patients to return home safely and strengthen nurses' ability to provide quality terminal care in home settings. This study explored nurses' experiences with using the concept and its influence on home-based terminal care. Fifteen semi-structured interviews were conducted with nurses who had applied the concept, which included a discharge checklist, a medical supply package, nursing guidelines, a medication decision tool, and informational materials for patients and relatives. Data were analyzed inductively using thematic analysis. Three key themes were identified: Streamlining Workflows , enabling nurses to focus on patient-centered care; Effective and Timely Symptom Relief , supporting prompt symptom management; and Enhanced Communication and Information Flow , improving collaboration across care sectors and reducing stress for patients and relatives. Findings suggest the concept may support nurses in delivering more coordinated, person-centered terminal care at home by proactively addressing common barriers. However, further education in palliative symptom management may be needed to optimize its use.
","PeriodicalId":54807,"journal":{"name":"Journal of Hospice & Palliative Nursing","volume":" ","pages":"E162-E168"},"PeriodicalIF":1.3,"publicationDate":"2026-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145710135","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Utilizing Bowlby's Caregiving Theory as a framework, this study examined the impact of time spent with the baby on the patient's lived perinatal loss experience, a phenomenon with limited investigation. Using a descriptive phenomenological approach, data were gathered retrospectively through interviews with mothers from 2 inpatient facilities at 3 and 6 weeks postpartum. Four major themes were identified: a sense of hopefulness, lack of support (follow-up care) from nursing/health care providers, importance of time with baby, and the need as parents to protect, nurture, and socialize (honor) their baby. Five additional minor themes identified and unique to individual facilities included a perceived lack of anticipatory guidance, need for communication in the moment, importance of mementoes, personhood related to baby, and the perception of abandoning baby at the time of discharge. All but one of the participants spoke about the significance of time with their baby. Most parents have the need to parent even in death, and health care professionals greatly impact whether mothers are given the opportunity to parent while in the hospital.
","PeriodicalId":54807,"journal":{"name":"Journal of Hospice & Palliative Nursing","volume":"28 4","pages":"194-199"},"PeriodicalIF":1.3,"publicationDate":"2026-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148334536","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This case report describes the care of a 47-year-old man with recurrent terminal tongue cancer, demonstrating how Swanson's Caring Theory guided holistic and empathetic end-of-life nursing practice. Over a 16-day hospice admission in September 2020, assessment was based on Gordon's functional health patterns and interventions aligned with Swanson's 5 caring processes. Key issues included excruciating tumor pain (rated 10/10), a malodorous, bleeding oral wound, and anticipatory grief. Integrative strategies, including morphine titration, lidocaine mouthwash, aromatherapy, and life-review therapy, reduced the pain to 2/10, improved the wound odor, and helped the patient find emotional acceptance. Guided by Swanson's framework, the nursing team established trust, involved the caregiver, and supported the patient's spiritual needs. In conclusion, applying Swanson's Caring Theory enabled individualized, compassionate, multidimensional care, demonstrating the value of theory-based integrative approaches in improving the quality of dying for patients with terminal oral cancer.
","PeriodicalId":54807,"journal":{"name":"Journal of Hospice & Palliative Nursing","volume":" ","pages":"E169-E177"},"PeriodicalIF":1.3,"publicationDate":"2026-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"146068910","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, evidence on how to promote hope remains fragmented, limiting its integration into palliative care practice. The aim of this rapid review was to identify and synthesize the available evidence on interventions aimed at promoting hope in people with advanced illness, with particular attention to their components and reported effectiveness. The review was conducted following Cochrane recommendations for rapid reviews and included studies published between 2020 and 2025. A total of 12 studies with diverse methodological designs were included. Interventions based on Snyder's Hope Theory showed consistent improvements in hope, although effects on other outcomes were less clear. In addition, approaches not explicitly designed to target hope-such as psycho-spiritual interventions, communication strategies, and early palliative care-also appeared to support its development. Across studies, improvements in hope were reported more consistently than changes in anxiety, symptoms, or quality of life, suggesting that hope may be particularly responsive to these interventions. The findings indicate that hope is a dynamic and clinically modifiable construct, shaped not only by structured interventions but also by the relational dimension of care, reinforcing the central role of nursing.
","PeriodicalId":54807,"journal":{"name":"Journal of Hospice & Palliative Nursing","volume":" ","pages":"162-171"},"PeriodicalIF":1.3,"publicationDate":"2026-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148058348","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
