Mond","doi":"10.1185/135525706X56673","DOIUrl":"https://doi.org/10.1185/135525706X56673","url":null,"abstract":"Objectives: The study's aims were to gain a better understanding of the bulimia nervosa mental health literacy (BN-MHL) of general practitioners (GPS), namely their beliefs about bulimia nervosa (BN) and its treatment, and to \u0000compare these perceptions with those found in community women. \u0000Methods: A postal survey was sent to 304 city GPs. The survey comprised a fictional vignette of a woman with BN followed by questions about perceived prevalence, severity, management, aetiology and prognosis. \u0000Results: Of the 149 (49%) respondents, 95% identified the problem as an eating disorder, 51% specifying EN. Eighty percent considered it a very distressing problem and 70% considered it uncommon in their practice and very difficult \u0000to treat. Nearly half regarded a psychiatrist as the most helpful clinician. Eighty percent thought discrimination would occur if others knew about the problem. \u0000Conclusion: GPs correctly identified BN and specialist treatment was favourably regarded. As found previously in surveys of the general population most GPs considered EN severe, distressing, and difficult to treat. Design of study: Needs assessment using in-depth semi-structured qualitative interviews. Setting: A single general practice in eastern England. Methods: Nine patients with depression were identified from the practice register using purposeful sampling and interviewed by a practice nurse who had not previously provided any care to them. Ritchie and Spencer's Framework approach was used to analyse the qualitative data. Results: Participants welcomed the opportunity to provide feedback on their experiences. They told many positive stories which affirmed the overall accessibility and acceptability of the existing service. They reported symptoms that were distressing and difficult to live with, even when their depression was classified by health professionals as 'mild'. Participants considered they had good access to care. Despite this, many participants identified gaps in the current service such as access to information and concerns about discontinuing medication in the future. Hypochondriac disorders have less prevalence, although they have been calculated at between 3% and 6%. States of health anxiety are characterised by recurrent, highly emotive thoughts about the possibility of suffering or contracting a serious illness. The patient frequently shows characteristic behavioural patterns such as checking his/her body, seeking medical reassurance or behaviour aimed at avoiding external stimuli and thoughts/images related to the illness. Such behavioural patterns have also been regarded as important factors in perpetuating these problems. Hypochondriac worries, on the other hand, are associated with emotional disturbances and disorders, more frequent use of health services and affectation of quality of life. Clinical and research interest in health anxiety is a result of the high personal and health-care costs associated with it, given the prevalence of the problem in medical contexts.
