Les manifestations rhumatologiques sont des myalgies, arthralgies, souvent diffuses, associées à de la fatigue. Il est nécessaire d’écarter une complication de la phase aiguë, une décompensation de comorbidité et une autre cause que la Covid-19. L’étape du diagnostic différentiel est importante. La prise en charge est pluridisciplinaire avec une rééducation et des exercices physiques adaptés, le traitement symptomatique des douleurs, la prise en charge des symptômes associés cognitifs, anxiodépressifs, de stress post-traumatique.</p></div><div><p>The incidence of post-COVID-19 syndrome has been reported between 6 and 30% of patients after SARS-CoV-2 infection. The pain syndrome pattern includes arthralgia, myalgia, frequently widespread pain and fatigue. The post-COVID-19 syndrome remains a diagnosis of exclusion, not explained by acute phase complications, decompensation of comorbidities and other causes unrelated to COVD-19. The temporal model includes 5 stages: phasic activation of the locus coeruleus noradrenaline system, salient stimuli, threat coding of salient stimuli, central sensitization, and neuronal degeneration. The treatment model includes three elements: reducing afferent visceral input; shifting humoral inflammatory activity away from the brain and the body; and reducing oxidative stress. The use systemic manual therapy protocols (SMT) are proposed to address these components.</p></div><div><h3>Design</h3><p>Cohort retrospective multivariate analysis using a modified adaptive platform design.</p></div><div><h3>Methods</h3><p>Improvement in knee pain and overall symptoms was measured in 467 patients after provision of 364 SMT protocols or protocol combinations, and across episode of care. The outcome tool used was the patient identified problem scale (PIP).</p></div><div><h3>Results</h3><p>Knee pain was improved on average by 1.28/9 (<em>P</em> <!--><<!--> <!-->0.001), overall improvement was 8.20/90 (<em>P</em> <!--><<!--> <!-->0.001). Thirty-five combinations containing 15 distinct protocols passed the HPTSAT criteria. These protocols included four with direct effects; the six proposed for their effects on central desensitization; and five protocols with either regional or desensitization effects.</p></div><div><h3>Conclusion</h3><p>An effective treatment for knee pain is possible if in addition to direct intervention over the knee, regional contributors and CS are addressed. The four conditions specified to test the temporal model and treatment hypothesis were met. All three types of interventions to address CS were more effective than oSOC (optimal standard of care)</p></div><div><h3>Historique</h3><p>Le but de cette étude est de tester l’hypothèse du modèle temporel pour le mécanisme et le traitement de la sensibilisation centrale (SC) chez les patients souffrant de douleurs au genou. Le modèle temporel comprend 5 étapes: activation phasique du système de noradrénaline du locus coeruleus, stimuli saillants, codage de la menace des stimuli saillants, sensibilisation centrale et dégénérescence neuronale. Le modèle de traitement comprend trois éléments: réduire l’apport viscéral afférent; déplacer l’activité inflammatoire humorale loin du cerveau et du corps; et réduire le stress oxydatif. Ils sont plus fréquents et plus importants dans le SDRC. À l’heure actuelle, les hypothèses physiopathologiques ne sont pas élucidées. L’échelle de Bath fournit de nombreuses informations qualitatives et quantitatives sur les troubles de la perception corporelle. Son originalité réside dans le dessin de l’image mentale du membre affecté, qui nécessite une coopération entre le patient et l’évaluateur. Les prises en charge thérapeutiques sont encore peu développées. Elles impliquent des stratégies visant des modifications, stimulation, et entraînement de perceptions multisensorielles.</p></div><div><p>Body perception disturbances in Complex Regional Pain Syndrome (CRPS) comprise both alterations in the sensorimotor representation of the limb (changes in the perception of shape, size, and proprioceptive representation) and in the conscious perception of the limb (negative emotions, aversion to appearance or the feeling of disownership). They are more frequent and more significant in CRPS. At present, the pathophysiological hypotheses have not been elucidated. The Bath CRPS Body Perception Disturbance Scale provides a wealth of qualitative and quantitative information. Its originality lies in the drawing of a mental image of the affected limb, which requires cooperation between the patient and the assessor. Therapeutic treatments are still underdeveloped. De la labellisation à la réforme du modèle de financement des SDC, ces travaux mettent en lumière l’évolution du paysage des SDC et soulignent l’engagement continu envers l’innovation et l’efficacité de ces structures.
