It protects both mothers and children against various infections and diseases.\u0000Objective: To identify the demographic variables related to breast feeding practices among mothers of below five years children in a semi-urban area of Bangladesh.\u0000Materials and methods: It was a descriptive type of cross sectional study conducted among purposively selected 180 mothers of below five years children. The study was conducted at Safipur, Kaliyakor, Gazipur in Bangladesh from 12th to16th February, 2019. Data were collected with a semi structured questionnaire by face to face interview. Statistical package for social science 19.0 programme was used for data analysis.\u0000Results: Most of the respondents (91.67%) knew about colostrum and among them (94.55%) fed their child colostrum immediately after birth. Majority of the respondents (95.56%) had knowledge about exclusive breastfeeding and most of them (65.70%) knew that the duration of exclusive breastfeeding is up to 6 months. Almost all of the respondents (94.44%) knew about weaning practices and most of them (70.59%) started weaning at the age of 6 months. Almost all respondents (89.44%) could breastfeed their child properly. Majority of the respondents (76.67%) knew that breast milk provides immunity to their children against certain diseases. Most of the respondents (73.33%) gathered knowledge about breastfeeding practices from their family members.\u0000Conclusion: Appropriate awareness about exclusive breast feeding among the mothers will help in enhancing the growth and development of children and also in saving them from many disease.\u0000Delta Med Col J. Khanom","doi":"10.3329/dmcj.v8i2.66620","DOIUrl":"https://doi.org/10.3329/dmcj.v8i2.66620","url":null,"abstract":"Background: Hypothyroidism is a common hormonal disorder affecting various organs including lungs and pulmonary dysfunction in hypothyroid patients has been noted. \u0000Objective: To observe maximum voluntary ventilation (MVV) in hypothyroid female patients. \u0000Materials and method: This cross-sectional study was carried out in the Department of Physiology, BSMMU, Dhaka, Bangladesh, from 1st July 2008 to 30th June 2009 on 60 hypothyroid female patients of 30-50 years old. Based on treatment, hypothyroid patients were divided into B1 (untreated patients on their 1st day of diagnosis) and B2 (patients treated for at least 12-18 months). For comparison, 30 age and BMI matched apparently healthy subjects (Group A) were also studied. The study group was selected from the Out Patient Department of Endocrinology, BSMMU, Dhaka, Bangladesh, and the control group was selected by personal contact. Serum TSH and serum FT4 levels were measured by Microparticle Enzyme Immunoassay (MEIA) principle in AxSYM system. Maximum voluntary ventilation (MVV), which is a lung function parameter, was measured by a digital MicroDL spirometer. Data were analyzed by one way ANOVA test, independent sample t-test and Pearson’s correlation coefficient test. \u0000Results: The mean percentage of predicted values of lung function parameter of MVV in control subjects and treated hypothyroids were within normal ranges. However, this value was significantly lower in untreated hypothyroid subjects in comparison to those of control and treated hypothyroid subjects. In addition, MVV showed negative correlation with serum TSH level and positive correlation with serum FT4 level and these relationships were statistically significant in control group and treated hypothyroid subjects. \u0000Conclusion: This study reveals that MVV may be lower in untreated hypothyroid female patients compared to control and treated hypothyroids and the deterioration may be positively correlated with serum FT4 level and negatively correlated with serum TSH level. Treatment of hypothyroids may reverse these changes. \u0000Delta Med Col J. Maitra","doi":"10.3329/dmcj.v8i2.66625","DOIUrl":"https://doi.org/10.3329/dmcj.v8i2.66625","url":null,"abstract":"Background: Diabetic foot is a common and potentially disastrous complication that can rapidly progress to irreversible septic gangrene, necessitating foot amputation. \u0000Objectives: To assess the clinical outcome of treatment of diabetic foot in diabetic patients based on random blood glucose (RBG) and HbA1c level. \u0000Materials and method: This prospective observational study was conducted in the Department of Surgery, BIRDEM General Hospital, Shahbag, Dhaka, Bangladesh, from July 2018 to December 2018. A total of 350 patients with diabetic foot were enrolled in the study based on inclusion and exclusion criteria. The patients were divided into four groups based on RBS and HbA1c level. A complete history was taken, thorough clinical examination was done and relevant investigation reports were collected. Collected data were classified, edited, coded, and entered into the computer for statistical analysis using SPSS version 23. \u0000Results: Mean age was 47.6±13.3 years in group A, 48.2±12.7 years in group B, 46.7±13.1 years in group C, and 49.9±12.5 years in group D. Male to female ratio was 2.3:1 in group A, 3.5:1 in group B, 1.5:1 in group C and 2.9:1 in group D. One hundred eight patients were found to have Wagner ulcer grading I. Among them, 2(20.0%) were in group A, 9(33.3%) were in group B, 12(36.4%) were in group C, and 85(30.4%) were in group D. Twenty-two patients were found to have Wagner ulcer grading IV. Among them, 1(3.0%) of group C and 21(7.5%) of group D. The difference was statistically significant (p<0.05) among the four groups. Two hundred eighty-eight patients had wound healing time of 2-6 weeks. Among them were 1(10.0%) in group A, 25(92.6%) in group B, 29(87.9%) in group C, and 233(83.2%) in group D. The mean wound healing time was found to be 1.5±0.7 weeks in group A, 3.1±1.3 weeks in group B, 3.6±1.2 weeks in group C and 4.9±1.8 weeks in group D. The difference was statistically significant (p<0.05) among four groups. One hundred six patients underwent amputation at a different level. Among them, 1(10.0%) in group A, 5(18.5%) in group B, 13(39.4%) in group C and 87(31.1%) in group D. The difference were not statistically significant (p>0.05) among four groups. \u0000Conclusion: Elevated HbA1c was associated with slower and incomplete foot healing in diabetic patients. Random blood glucose and HbA1c parameters can be used as dependable predictors of foot ulcer healing in the diabetic patients. \u0000Delta Med Col J. The route of transmission of the virus and the nature of dental service has put dental professionals at risk of being exposed and infected by the virus and at the same time making dentistry itself a part of the infectious chain of COVID-19. The World Health Organization (WHO) has advised dental professionals across the world to limit dental practice to emergency cases only and postpone all non-essential dental services. According to Bangladesh Dental Society, about 10,000 practicing dentists and near about 20,000 supporting staff are involved in dental health practice in the country. There are about 4,000 dental clinics only in Dhaka city. A dental practice guideline has been developed by Bangladesh Dental Society to employ by dental professionals during COVID-19 pandemic. However, we feel that efforts to explore the real scenario of proper safety precaution and maintenance of public health measures in COVID-19 is less evident in the manuals in the context of public health and patient safety in Bangladesh. Hence, we proposed this review paper through an extensive literature and guidelines search to explore the risks of COVID-19 transmission inside dental settings, explore recommended safety protocols to minimize those risks and address the challenges associated with resuming regular dental and oral health practice. \u0000Delta Med Col J. Currently, controversy remains about the best therapeutic alternatives such as complementary and alternative medicine (CAM) and conventional medicine (CM) for this disease when evaluated from a cost effectiveness viewpoint. Objective: To examine the cost effectiveness of CAM and CM in the treatment of RA. Materials and method: The data were collected from Dhaka Medical College Hospital and Government Unani and Ayurvedic Medical College Hospital from August 2008 to February 2009 by primary sources in the study. Total 60 patients were included who had been treated for six months. Patients were assessed with EuroQol 5D (EQ-5D) tool. Results: The outcome is more effective in CAM than CM by EuroQol 5D. The EQ-5D average outcome of CAM is 0.239 and CM is 0.202. The cost of CAM is also lower than that of CM. Pain on propofol injection still remains a considerable concern for the anaesthesiologist. Objective: Aim of this study was to observe the efficacy of metoclopramide as pretreatment for the prevention of pain caused by the propofol injection in patients undergoing elective surgery under general anaesthesia. Materials and method: A total of 80 patients were taken up in the study in the age group of 20 to 50 years of either sex, ASA grade I/II, scheduled for routine elective surgical procedure under general anesthesia with endotracheal intubation and using propofol as induction agent. The patients enrolled were divided randomly into two groups of 40 patients each. Group A received 10 mg metoclopramide IV diluted in 5 ml saline. Group B received 5 ml of normal saline as placebo before propofol injection. The patients were asked to report their pain according to the scale provided to them in the form of none, mild, moderate and severe after injection of propofol. Results: The overall incidence and severity of pain were significantly less in Group A (metoclopramide group) than in group B (placebo group) (p< 0.05). The incidence of mild and moderate pain in Group A versus group B was 15% vs 45% and 5% vs 25% respectively (p<0.05). The incidence of score ‘0’ (no pain) was higher in Group A (80%) thanGroup B (25%) (p< 0.05). Morphology of tendo Achilles is an important tool for its assessment which can be done by measuring tendon thickness, cross-sectional area and length of tendon. Objective: The aim of the study was to correlate length of tendo Achilles with stature in several age groups of sedentary people as measured by ultrasonography. Materials and method: This cross sectional analytical type of study was conducted in the department of Anatomy, Dhaka Medical College Hospital, Dhaka, Bangladesh from July 2013 to June 2014. The present study was performed on 200 tendo Achilles of 100 people (50 male and 50 female) randomly selected from the patients who came to the Radiology department of Dhaka Medical College Hospital for ultrasonography of any region of their body other than leg. Results: In this study length of tendo Achilles was measured in both legs of adult people and was correlated with stature. Highly significant correlation was found between length of tendo Achilles with stature (p<0.001). PCOS is associated with many comorbidities and also has a number of long-term metabolic and other consequences. The prevalence is quite high and is increasing day by day. It is a syndrome to be prevented by awakening awareness both in health workers and patients. There are many areas of controversies starting from its diagnosis, pathogenesis, consequences and treatment modalities. This review is an attempt to summarize the evolution of the diagnosis and current management guidelines and also to look into the future approaches.
