Bangladesh, a rapidly developing country, faces progressive urbanization and the adoption of a westernized lifestyle, factors which contribute to the rising burden of cardiovascular disease. \u0000Objective: To predict the risk of developing cardiovascular diseases among diabetic patients in Bangladesh. \u0000Materials and method: This cross sectional study was done among the diabetic patients attending the outpatient department of MARKS Medical College &Hospital, Dhaka, Bangladesh from February 2018 to July 2018. Patients suffering from any cardiovascular disease were excluded from the study. The purpose of the study was explained and informed written consent was obtained from each subject who volunteered for the study. Data was collected with a pre tested structured questionnaire. Cardiovascular risk was assessed based on Framingham scoring, calculated by using online calculator and expressed as percentages. Based on the total risk score calculated, subjects were categorized to be at high (≥20%), intermediate (10-19%), and low (<10%) risk of cardiovascular disease. Data analysis was done in SPSS software version 16. \u0000Result: Among the diabetic patients, 32.62% are in high and 22.64% are in intermediate risk category based on Framingham scoring system for developing cardiovascular disease. A statistically significant association was found between cardiovascular risk and factors like gender, occupational status, smoking and hypertension. \u0000Conclusion: This study reveals that the diabetic populations are at higher risk of developing cardiovascular diseases. Hence awareness about the risk should be created and appropriate intervention at early stages should be implemented at primary health care level. \u0000Delta Med Col J. Quite a number of women with these anomalies can be totally asymptomatic. In some women, anomalies may be first detected during pregnancy or delivery. Here a case of incidentally diagnosed bicornuate uterus is reported in a lady of 48 years while being treated for breast carcinoma. The patient attended with a lump in her left breast at Delta Medical College Hospital, Dhaka, Bangladesh. She was totally asymptomatic previously and her three children were born preterm without any difficulty. Along with other investigations, abdominal ultrasonogram was done which showed two separate fundal and uterine cavities with single cervix. Bicornuate uterus was confirmed by endovaginal ultrasonogram. Breast carcinoma was treated accordingly. \u0000Delta Med Col J. Sarker","doi":"10.3329/dmcj.v8i1.58957","DOIUrl":"https://doi.org/10.3329/dmcj.v8i1.58957","url":null,"abstract":"Background: Optic nerve sheath fenestration (ONSF) is commonly used in idiopathic intracranial hypertension (IIH) with visual deterioration and visual improvement is observed post operatively in these patients. \u0000Objective: Retrospective evaluation of MRI observations in IIH patients who underwent optic nerve sheath fenestration (ONSF) with postoperative visual improvement. \u0000Materials and method: Optic nerve sheath fenestration (ONSF) was done in twenty four (24) patients with IIH having severe visual impairment. Post operatively all patients improved in their visual functions. The recorded pre operative MRI (of brain and orbit) of patients were reviewed and evaluated retrospectively. \u0000Results: Total number of MRI studied was 24. Male female ratio was 1:11. Posterior globe flattening was found in all 24 (100%) cases. Other findings were horizontal tortuosity of optic nerve (92%), optic nerve head protrusion (87.5%), optic nerve sheath distension (87%), and vertical buckling (62.5%). Ventricular size was normal in 92% cases and slit ventricle was found only in 8% cases. On magnetic resonance venogram (MRV) one sided transverse sinus abnormality was found in 25% cases. There was empty sella in 79% cases, more marked in three cases. \u0000Conclusion: The MRI findings in IIH patients with positive post ONSF result are of variable degree and prevalence except posterior globe flattening which was found in all cases. \u0000Delta Med Col J. Shermin","doi":"10.3329/dmcj.v8i1.58958","DOIUrl":"https://doi.org/10.3329/dmcj.v8i1.58958","url":null,"abstract":"Background: Leukaemias are neoplastic proliferations of haematopoietic stem cells and form a major proportion of haematopoietic neoplasms that are diagnosed worldwide. \u0000Objective: To differentiate between morphological and immunophenotypic analysis in the diagnosis of acute leukemia. \u0000Materials and method: This cross sectional study was conducted in the department of Haematology, Armed Forces Institute of Pathology (AFIP), Dhaka, Bangladesh from January 2008 to December 2008. Total 50 patients were included after fulfilling inclusion and exclusion criteria. \u0000Results: The total of 50 bone marrow samples from suspected cases of acute leukaemia were included in the study. Out of 50 samples, 48 cases were diagnosed as either acute myeloid leukaemia (19 or 38%) or acute lymphoblastic leukaemia (29 or 58%) and 02 (04%) cases were morphologically indistinguishable. All 50 cases were subjected to immunophenotypic study. Out of 50 cases immunophenotypically 14(28%) were acute myeloid leukaemia (AML), 32(64%) were acute lymphoblastic leukaemia (ALL), and bi-phenotypic leukaemia and acute undifferentiated leukaemia were 02(04%) each. In this study Male: Female ratio was 1.3:1. Out of 19(38%) cases of AML, 29(58%) cases of ALL and 02(04%) cases of indistinguishable diagnosed morphologically, 14(28%) were found to be AML, 32(64%) ALL, 02(04%) bi-phenotypic and 02(04%) were acute undifferentiated leukaemias on immunophenotyping respectively. Out of 29 cases identified as ALL on morphology 25(86.2%) were confirmed as ALL, 02(07%) turned out to be AML, 01(3.4%) was bi-phenotypic and 01(3.4%) was undifferentiated. \u0000Conclusion: In this study, acute lymphoblastic leukaemia was the commonest type of leukemia followed by acute myeloid leukaemia with male predominance seen in all types of leukaemia. \u0000Delta Med Col J. The healthcare workers are facing challenges in reducing the severity and mortality across the world. Patients with severe form of COVID_19 are generally treated in the intensive care unit, while mild or non_severe patients are treated in the isolation ward or at home. It is important to identify early and give treatment to reduce the disease severity and improve the outcomes. There is no credible and convenient biomarker to predict the severity of the disease. Therefore, the exploration of biomarkers is deemed necessary. We aimed to review the diagnostic and early prognostic value of C-reactive protein (CRP) in COVID-19 patients. In a number of studies, CRP showed different distribution feature and differences existed in various ages, clinical types and outcomes of COVID-19 patients. Bangladesh is known to have a high prevalence of skin diseases. Considering the economic burden of the skin diseases treatment and its high prevalence, it is important to study the drug prescribing patterns in skin diseases. \u0000Objective: To evaluate the prescribing pattern of drug according to World Health Organization (WHO) prescribing indicators among the patients attending Dermatology outpatient department in two teaching hospitals. \u0000Materials and method: A cross sectional descriptive study was conducted in outpatient department of Dermatology in Dhaka Medical College Hospital (DMCH) and Sir Salimullah Medical College & Mitford Hospital (SSMC & MH) from July 2015 to June 2016. A total number of 600 prescriptions (300 from each hospital) were enrolled in the study. The general information and data were collected in the questionnaire and subjected to evaluation using WHO prescribing indicators. \u0000Results: The average number of drugs per prescription was 2.41 in DMCH and 2.38 in SSMC & MH. Only 23.72% drugs in DMCH and 27.79% drugs in SSMC & MH were prescribed by generic name. The percentage of prescriptions with antibiotics was 46% in DMCH and 45.66% in SSMC & MH. The percentage of prescriptions with an injection encountered was 0.67% and 0.33% in DMCH and SSMC & MH respectively. This study also revealed that, the percentage of drugs prescribed from the Essential Drug List (EDL) of Bangladesh was 56% in DMCH and 57.68% in SSMC & MH. \u0000Conclusion: There is need to emphasize to keep the average number of drugs as low as possible and encourage prescribing by generic name and from Essential Drug List (EDL). \u0000Delta Med Col J. This syndrome is characterized by varying pattern of low back pain, sciatica, lower extremity sensorimotor loss, saddle anaesthesia and bowel and bladder dysfunction. \u0000Objective: The objective of this study was to evaluate clinical and functional outcome and operative complications in case of CES who underwent surgical decompression by laminectomy and discectomy. \u0000Materials and method: This prospective interventional study was carried out at National Institute of Traumatology and Orthopaedic Rehabilitation (NITOR) and different private hospitals in Dhaka from January 2017 to December 2019. Patients were selected on the basis of history, clinical examinations and Magnetic Resonance Imaging (MRI) findings. Patients with fracture and compression due to other than disc prolapse were excluded from the study. \u0000Results: Out of 23 patients, 18 patients (78%) were male and 5 patients (22%) were female, age ranged from 20-60 years, with the average age 37.5 years. Twenty one patients (91%) had single level and 2 patients (9%) had two level disc herniation. The mean follow up duration was 9 months (range 6-12 months). Patients were evaluated with respect to age, time to surgery and scoring system for CES before and after operation. Those who were treated within 48 hours and those after 48 hours showed significant difference in outcome (p <0.05). Complete recovery was documented in 12 patients (52%). There was infection in 01 case (4%), 03 patient (13%) had persistent low back pain, 02 patients (9%) had sciatica, 01 patients (4%) had bowel and bladder dysfunction and 01 patients (4%) had saddle anaesthesia in delayed operative group. Evaluation of final outcome was satisfactory in 17 patients (74%). \u0000Conclusion: Laminectomy and discectomy is an effective, safe and acceptable modality of treatment in CES. \u0000Delta Med Col J. Tuberculosis (TB) of the musculoskeletal system is an uncommon infection caused by tuberculous bacilli and constitutes 1% - 5% of all forms of TB. About 30% of skeletal TB involves the joints, the knee being the 3rd most commonly affected after the spine and the hip. Knee tuberculosis commonly presents as synovitis or arthritis. We are presenting a case of a young female with clinico-radiological features suggestive of Baker's cyst. Complete excision of Baker's cyst and histopathology was done. Histopathological examination revealed multiple epitheloid granulomas pointing toward a tubercular etiology. Standard ATT protocol with rehabilitation was followed. The patient was asymptomatic at 1st year with complete resolution of symptoms and full range of motion. This case highlights the need to maintain high index of suspicion in cases hailing from endemic region with unusual intra-operative findings; also, the importance of routine histopathological examination. \u0000Delta Med Col J. Therefore, a change in practice with opportunistic salpingectomy (OS) at the time of hysterectomy has been advocated for prevention of ovarian cancer in a low-risk population. \u0000Objective: The aim of this study was to analyzethe current trend of performing opportunistic salpingectomy during gynaecological surgery for benign indicationsin a tertiary care hospital of Bangladesh. \u0000Materials and method: This retrospective observational study was conducted in the Department of Obstetrics & Gynaecology, BIRDEM Women and Children Hospital, Dhaka, Bangladesh from October 2019 to March 2022. All relevant demographic and clinical information of 255 patients undergoing hysterectomy for benign gynaecological disease without involving the ovaries or adnexae were documented from hospital records after exclusion of patients with any gynecologic cancer or having any ovarian or adnexal pathology, and those who underwent hysterectomy due to pregnancy related conditions. \u0000Results: Mean age of the patients was 48.71 ± 8.31 years and the highest proportion of patients (49.0%) were in the 41-50 years of age group. Bilateral salpingo-oophorectomy was done in 131 cases (51.4%) with mean age of 51.95 ± 7.99 years and opportunistic salpingectomy was done in 72 cases (28.2%) with mean age of 42.68 ± 3.97 years. The rate of opportunistic salpingectomy statistically increased from 17.39% in 2019 to 36.17% in 2022. \u0000Conclusion: As seen throughout the world, the practice of opportunistic salpingectomy for prevention of ovarian cancer has also increased in Bangladesh in recent years. \u0000Delta Med Col J.
