LF Beauty Manufacturing Indonesia is a Hong Kong-based Li & Fung group of companies engaged in manufacturing services. The Company has 4 regular customers, namely: PT JHHP, PT.Diversey, PT.Topindo and PT.Wavin. During 2015 the company got 12 defect complaints which 8 complaints came from PT. JHHP. This study aims to determine the cause of disability complaints of PT JHHP, provide improvements to existing problems so that it does not occur in the future and as a measure of company productivity to improve production quality. This research method uses Six Sigma with DMAIC stages (Define Measure, Analyze, Improvement, Control). The object of research is the production data of August 2015 in accordance with customer complaints data. The result of analysis of JHPP customer's complaint obtained the biggest defect of kiwi paste product according to the diagram is finish good leak. Factors affecting the defect complaints of the finish good leaked kiwi paste product is the operator often do autocapper engine wind adjusment, unavailability of space thermometer tool in the warehouse area and dry production area, unavailability of wind pressure gauge used to determine the number of bars used in the asemble process and the temperature of the storage area space exceed the standard set. The number of occupational accidents is increasing significantly from 96,314 in 2009 to 110,285 in 2015. The mortality rate in these cases is very high. This research aims to develop a daily fitness for duty index as a measurement to ensure that workers are able to work effectively and safely. Systematic literature review had been carried out through several stages, i.e. 1) determination of searching strategy, and searching process, 2) determination of searching criteria, 3) quality control for literature, and 4) conclusion. The experiment involved four participants. They divided into two groups: night shift workers and morning shift workers. Total sleep time, heart rate at rest, reaction time, and body weight were recorded during the experiment. The daily fitness for duty index has been developed that consider sleep sufficiency factor, circadian cycle misalignment factor, and individual factor. The daily fitness for duty index has a strong correlation with subjective work readiness measurement. X is a distributor of spare parts with 128 customers for Surabaya City and Sidoarjo City. Number of delivery vehicles owned by PT. X is 15. Determination of delivery route owned by PT. X is completed manually based on driver experience. This research is to make a model of determining the route with minimum cost by considering the travel time and the capacity of the vehicle. Model formation is accomplished in accordance with the PT X policy that each colleague can only be served by one vehicle and the maximum shipping time of 7 hours. The model formed is Heterogeneous Fleet Vehicle Routing Problem with Time Windows (HFVRPWTW). The settlement method used is Evolutionary Algorithm (EA) by reviewing 3 cases. The number of vehicles decreased by the model is  with an average difference of 1.67 vehicles. The shipping cost generated by the model is lower than the company for case 1 of 3,155 IDR and case 2 of 25,005 IDR. The achievement of labor productivity through activity sustainable done at one manufacturing companies who produce capacitor. In the packing process is done manually using bending tools that cause fatigue 90-95% tired on the right foot and right hand, with the average reject 14% and only meet 66% output target. The purpose of this research is make a bending machine to increase the capacity process of packing, so it expected to decrease reject mechanical and indirectly decrease the work load operator. The design of the bending machine is done by testing the T-test statistic (paired T-test) against the mechanical reject by making the machine system semi-automatic with motor and engine control on the machine table design using the average height of the operator seat with the aim that the standard time can be faster, The human workload based on the man machine system may decrease.  The result of bending machine design shows that the standard time after design is increased from 1.52 seconds to 1.02 seconds so the average output increase from 16578 capacitors to 24705 capacitors. The approval of the KRS (Card Study Plan), the management of transcripts of grades, and the taking of diplomas are the requirements of the students at BAAK, so in the registration condition of the students, especially at the end of the registration period, there is a solid queue that the majority of the students need is the endorsement of KRS. One factor could be due to lack of servers (service personnel). The purpose of this study to determine the needs of the number of servers that should be used BAAK UTP using queue theory. Transfer conveyor is an additional element in a conveyor belt circuit that serves to channel coal from unloading area until coal bunker (power plant). This study aims to find out the value of OEE Transfer Conveyor 17A which is used as the basis of proposed improvement based on the lowest value of the three parameters in OEE. The concept of research adopting OEE method with the value of quality is assumed 97,50% because the equipment that becomes the object of research does not produce the product. The result of calculation obtained by OEE value equal to 82,98% with ratio Availability 86,76% Performance Eficiency 98,09% and Rate of Quality  97,50%. The sudden impact of machine damage resulted in disruption of planned production performance so that it is necessary to identify and analyze the factors that cause the machine damage. Establish maintenance method using Autonomous Maintenance, Preventive Maintenance and Reliability Centered Maintenance (RCM) concept to prevent machine failure from the beginning, through fault tree analysis (FTA) method, failure mode effect, and analysis (FMEA), mean time between failure (MTBF) is an analytical activity for implementing RCM systems on machines and critical components, able to identify and detect symptoms of malfunction before the machine is damaged. The results of the research get better application of maintenance system so that identification of important components can be anticipated from the symptoms of damage. Overall, there is an increase in performance seen from the increase of Overall Equipment Effectiveness (OEE) value, still expected to continue to increase according to JIPM 85% standard value. The purpose of this study is to determine the possibility of the business plan for the development of food business with the limits of research on the assessment of financial aspects. Primary data collection is done through field observation, interview and questionnaire filling. Objects in interviews and surveys are business owners and residents around the place of business. Secondary data are obtained from literature studies. Data were analyzed by using Payback Period (PBP) method, Present Net Value (NPV) method, and Internal Rate of Return (IRR) Method. The result of the research shows the ability of the company to return all investment cost is for four years eight months. Cash funds collected from year to year if assessed at present with a positive value at the end of year 5 (five) and the ability of the company's business to provide annual average return/dividend amounted to 20.494%. A Chemical Company in Cilegon has a production cost reduction program. However, in the process of creation, still found some factors that cause high production costs, especially the cost of electricity in pumping equipment. This study aims to determine the causes of high electricity consumption in the drainage pump unit, provide suggestions for improvement and evaluate the results of revision. This research uses some new quality tools consisting of affinity diagrams, interrelationship diagrams, matrix diagrams, tree diagrams, and Process Decision Program Chart (PDPC). Based on the results of the research, the factors causing high electricity consumption in the drainage pump unit are DCS-Man forgot to stop at the G-1875 pump, the C-1851 sealing output goes to the slimy pit, and the G-1871 sealing output goes into the dirty hole. The remedial step is to move the C-1851 sealing output from the muddy ditch to the clean trench, moving the G-1871 sealing output from the filthy ditch to the clean ditch, fixing the leak in the sample product box, and the G-1875 pump made auto start-stop. The data used in this research are primary and secondary data. Primary data obtained through observations and interviews to the parties concerned in the mechanical and electrical parts. Secondary data in the form of data SOP maintenance and repair machine from company. Descriptive, comprative, and evaluative methods are used to analyze the actual condition and working procedures of maintenance and repair of machinery and to compare between the results obtained from this research on the implementation of the maintenance work process and the repair of machines there are still stages that are not executed according to the procedure. Using the Fishbone Diagram method, the most dominant type of damage occurring in the motorizer engine is a burning roll with a contribution rate of 53.125%.
