This trend has important implications in pregnancy, as HCV infection can lead to adverse maternal outcomes, mother-to-child transmission, and adverse neonatal outcomes. The perinatal period is a critical time both to identify cases of HCV infection and to initiate treatment, as patients have greater access to care during this time. There has been a guideline shift from risk-based screening for HCV in pregnancy to a universal screening approach. Additionally, several clinical trials are evaluating the use of direct-acting antiviral therapy in pregnancy, with preliminary results demonstrating safety and efficacy. Inflammation that is not controlled, and even in instances when inflammation is controlled, often has a substantial impact on quality of life for individuals with Crohn's disease, affecting relationships, education, and work productivity. The concept of a therapeutic window of opportunity has long been recognized as a period within which to maximize effectiveness of interventions and potentially modify the course of disease. However, there has been considerable uncertainty over the years on when the window of opportunity is, or how early should early effective intervention be in Crohn's disease. In recent years, there has been a growing body of evidence supporting early intervention to maximize benefit and improve outcomes for patients, and several seminal studies have provided insights into the true window of opportunity. Since that time, 3 new first-line treatment options have been approved in the United States and knowledge of rising antimicrobial resistance (now above 30% for clarithromycin, levofloxacin, and metronidazole) has been accumulating. Key clinical updates in the 2024 guidelines include new recommendations on screening practices, selecting the most appropriate treatment regimens for initial <i>H pylori</i> infection, and using antimicrobial susceptibility testing in cases where the infection does not respond to initial therapy. This article summarizes these updates with an emphasis on how the current guidelines differ from the previous version.
