Patients receiving midazolam for the first surgery received clonidine at the second surgery and vice versa. The anxiolytic efficacy was evaluated with a visual analogue scale (VAS) upon admission and 30, 50 and 60 min after administration of the medication. Patient satisfaction was recorded on a VAS after surgery and 7 days postoperatively.</p><p><strong>Results: </strong>As soon as 30 min after administration of midazolam (p < 0.03) and clonidine (p < 0.02), an anxiolytic effect was recorded. Most patients are male adults presenting clinical symptoms and signs similar to those of pulmonary tuberculosis. The second group is formed by immunocompetent children with localized cervical lymphadenitis. Although cervical adenitis is the main extrapulmonary manifestation of M. malmoense, virtually all cases of cervical disease were documented in children. Disseminated disease is rare and has been reported in patients with severely impaired immunity.</p><p><strong>Case report: </strong>We report a case of a 47-year-old immunocompetent man with a cervical abscess, in whom we identified a M. malmoense pulmonary disease with multiple cervical, thoracic and abdominal adenopathies.</p><p><strong>Conclusion: </strong>Extrapulmonary infection due to M. Malmoense needs to be considered on the differential diagnosis of cervical masses and adenopathies, not only in pediatric patients but also in adults with no impaired immunity. The injection discomfort associated with three volumes of infiltration anesthesia was also assessed.</p><p><strong>Materials and methods: </strong>A total of 10 subjects received 0.6 mL (group 1), 0.9 mL (group 2), and 1.2 mL (group 3) of the anesthetic buccal to the upper canine. Test teeth were assessed with electrical stimulation to determine onset and duration of pulpal anesthesia. Soft tissue anesthesia was assessed by pin-prick test, and injection discomfort was assessed using a visual analogue scale. The statistical analysis of the data recorded was carried out with one-way ANOVA and post-hoc tests.</p><p><strong>Results: </strong>The 1.2-mL dose induced faster onset of pulpal anesthesia, a higher success rate, and a longer duration of soft tissue/pulpal anesthesia than was achieved with 0.6 mL (P < 0.005). No differences in injection discomfort were observed between treatment groups. Group 3 where 1.2 mL of local anesthetic was injected showed faster onset and longer duration of action of articaine. Group 3 also had longer soft tissue anesthesia as compared to groups 1 and 2.</p><p><strong>Conclusion: </strong>Maxillary infiltration anesthesia with articaine and epinephrine has a faster onset, a greater success rate, and a longer duration when a volume of 1.2 mL is used than when volumes less than 1.0 mL are used. {"title":"Malignant neoplasms of the sinonasal tract: report of 71 patients and literature review and analysis.","authors":"Bijan Khademi,&nbsp;Azadeh Moradi,&nbsp;Sara Hoseini,&nbsp;Mohammad Mohammadianpanah","doi":"10.1007/s10006-009-0170-8","DOIUrl":"https://doi.org/10.1007/s10006-009-0170-8","url":null,"abstract":"<p><strong>Background: </strong>The present study aimed to report the characteristics, prognostic factors, and treatment outcomes of 71 cases of malignant neoplasms of the sinonasal tract and literature review and analysis of major report series.</p><p><strong>Methods: </strong>Seventy-one consecutive patients diagnosed with primary malignant neoplasm of the sinonasal tract that were treated and followed up at a university hospital between May 2000 and March 2008 were selected for the present study. Thirty-four patients were treated with surgery followed by a combination of chemotherapy and radiotherapy, 15 with surgery alone, 14 with combined radiotherapy and chemotherapy, six with radiotherapy alone, and two with surgery followed by radiotherapy. To find out the major series of related studies over the last 20 years, a literature review of PubMed was performed. In all, we found 42 major series including 8,164 patients with malignant neoplasms of the sinonasal tract.</p><p><strong>Results: </strong>There were 35 women and 36 men ranging in age from 5 to 80 years, with a median age of 55 years at diagnosis. The primary sites included were paranasal sinuses in 51 and nasal cavity in 20. There were one case of stage I, 20 of stage II, 27 of stage III, and 23 of stage IV. Epithelial tumors constituted 65% of all neoplasms. After a median follow-up of 39 months for surviving patients, 33 patients are alive and without disease, eight are alive with disease, and 30 patients died due to disease. Local recurrence was the most frequent treatment failure. The 5-year disease-free, local control, and overall survival rates were 42.1%, 59.5%, and 54.5%, respectively. On univariate analysis, cervical lymph nodes involvement, primary tumor size, histologic type, response to therapy, and stage of disease were independent prognostic factors for overall survival. In the literature review and by analyzing the data collection from 42 major reported series, the median age was 57 years and male/female ratio was 1.8. Epithelial tumors consisted of 69% of all malignant neoplasms of sinonasal tract and stages III and IV disease constituted 74.7% of all stages. Local recurrence was the dominant treatment failure in nearly all series. Five-year local control and overall survival rates were 56% and 45.5%, respectively.</p><p><strong>Conclusions: </strong>In this review and by analyzing the large data collection of recent major reported series, we found that malignant neoplasms of the sinonasal tract tend to present at locally advanced stage, with a high frequency of local failure and a moderate to poor outcome. The disease exhibits a dominant autosomal hereditary trait, with implication of the human homologue of the Drosophila segment polarity Patched (PTCH) gene. BCNS is diagnosed on the basis of clinical and radiological criteria and can be confirmed by genetic study. Miniplates in the frontozygomatic area are traditionally applied to the lateral face of the orbital rim, but with some undesirable effects, such as palpability, visibility, and risk of penetration into the anterior cranial fossa. The aim of this study was to perform an anatomical study to validate the use of miniplates on the temporal face of the frontozygomatic region.</p><p><strong>Methods: </strong>Osseous thickness measurements were performed in 30 skulls, on four points above and four below the suture, at 3-mm intervals, perpendicular to the bone surface.</p><p><strong>Results: </strong>There is enough bone thickness to apply the screws, ranging between 4 and 6.5 mm. The first hole over the frontozygomatic suture should receive the smallest screws and the other areas can receive screws up to 6 mm. All drillings are made from the temporal fossa to the orbit, and its contents should therefore be protected during the perforations. The advantages and disadvantages of this technique are discussed.</p><p><strong>Results: </strong>The surgical protocol for inferior alveolar nerve transposition, followed by implant placement, presented excellent results, with complete recovery of the sensitivity within 6 months after the surgical procedure.</p><p><strong>Discussion: </strong>Inferior alveolar nerve transposition is an option for prosthetic rehabilitation in cases of moderate or even severe bone reabsorption for patients that do not tolerate removable dentures.</p><p><strong>Conclusions: </strong>It is concluded that inferior alveolar nerve transposition can be safely and predictably performed with low risk to the mental nerve sensibility. Each patient should be advised of the chance of permanent nerve deficit throughout the distribution of the mental nerve. {"title":"Mandibular fractures associated with endosteal implants.","authors":"Bruno Ramos Chrcanovic,&nbsp;Antônio Luís Neto Custódio","doi":"10.1007/s10006-009-0171-7","DOIUrl":"https://doi.org/10.1007/s10006-009-0171-7","url":null,"abstract":"<p><strong>Purpose: </strong>The purpose of this study is to report four cases of mandibular fractures associated with endosteal implants and to discuss prevention and treatment of these types of fractures.</p><p><strong>Discussion: </strong>To evaluate whether the patient's anatomy allows insertion of implants, radiological exams that demonstrate the height and the labial-lingual width are needed. To reduce the potential fracture problem, the mandible can be restrengthened with bone grafting techniques. The treatment of a fracture in an atrophic mandible is always a challenge because of the diminished central blood supply, the depressed vitality of the bone, and the dependence on the periosteal blood supply. The basic principles in fracture treatment are reduction and immobilization of the fractured site for restoration of form and function.</p><p><strong>Conclusions: </strong>If implants are placed in severe atrophic mandible, iatrogenic fracture of the mandible may occur during or after implant surgery because implant placement weakens the already-compromised mandible. A few millimeters of cortical bone should remain on both the labial and the lingual sites after the hole for insertion of an implant has been drilled. The removal of projectiles is recommended; in some cases, however, this is a controversy. The reproduction of a projectile image can be difficult if it is not adjacent to an anatomical landmark. Therefore, navigation systems give the surgeon continuous real-time orientation intraoperatively. The aim of this study was to report our experiences for image-guided removal of projectiles and the resulting intra- and postoperative complications.</p><p><strong>Patients and methods: </strong>We investigated 50 patients retrospectively; 32 had image-guided surgical removal of projectiles in the oral and maxillofacial region. Eighteen had surgical removal of projectiles without navigation assistance.</p><p><strong>Results: </strong>There was a significant correlation (p = 0.0136) between the navigated surgery vs. not-navigated surgery and complication rate, including major bleeding (n = 4 vs. n = 1, 8% vs. 2%), soft tissue infections (n = 7 vs. n = 2, 14% vs. 4%), and nerval damage (n = 2 vs. n = 0, 4% vs. 0%; p = 0.038) and between the operating time and postoperative complications. A high tendency between operating time and navigated surgery (p = 0.1103) was shown. When using navigation system, we could reduce operating time.</p><p><strong>Conclusion: </strong>In conclusion, there is a significant correlation between reduced intra- and postoperative complications, including wound infections, nerval damage, and major bleeding, and the appropriate use of a navigation system. In all these cases, we could present reduced operating time. She had a CD4+ count of 487 cells/mm3. The urinary vanillylmandelic acid assay and echocardiography were normal. Subsequently, she had complete tumour excision through a medial maxillectomy and has remained without a recurrence 12 months after.</p><p><strong>Conclusion: </strong>To our knowledge, this is the first report in the world literature of sinonasal paraganglioma in a HIV-positive patient and the first reported case of sinonasal paraganglioma in Africa.
