However, we are surprised that DPTs with iodinated contrast media (ICM) are now discouraged. This statement contradicts a previous proposal of the same Committee, which recommended DPTs with ICM when studying hypersensitivity reactions to these drugs. This advice was included in the \"Practical Guide for the Diagnosis and Management of Hypersensitivity Reactions to Contrast Media\" published in JIACI in 2016 [2]. In the new guideline, the contraindication to DPTs with ICM is based on potential toxicity. However, in our opinion, this is not a compelling reason. In fact, the same authors consider that DPTs with potentially more toxic drugs, such as nonsteroidal anti-inflammatory drugs, biologics, and cytostatic drugs, are indicated. Kidney damage is the most frequent toxicity reaction produced by ICM. However, the existence of contrast-induced nephropathy is currently questioned [3] and, in any case, is avoidable using a nephroprotective protocol adjusted to a renal risk score [4,5]. In other words, while many drugs are potentially toxic, the toxicity of ICM can be prevented using nephroprotective measures, especially if the patient is properly hydrated. In recent years, the lack of efficacy of premedication in preventing hypersensitivity reactions to ICM [6] has led to allergy studies including DPTs. These studies were performed by influential research groups, several of which were Spanish, with successful results [7-9]. Moreover, DPTs with ICM were also recommended by the EAACI in a recent position paper [10]. Therefore, we think that DPTs with ICM are a safe and necessary tool, both for establishing a diagnosis of drug allergy (in the case of inconclusive results with ICM in skin tests) and for confirming tolerance to an alternative ICM with previous negative skin test results. Perhaps the greatest difficulty in generalizing the use of DPTs with ICM is the lack of standardized protocols. Therefore, further studies are necessary to search for the most appropriate methodology. This area is especially important today, since many patients are labeled as allergic to drugs, especially penicillins, without this being the case. This article reviews the importance of the clinical history in a patient with a hypersensitivity reaction to a drug and considers which data should be collected. Although diagnosis is clinical and disease course is mostly self-limiting, diagnostic tools and medications are overused by physicians, thus increasing the direct medical costs of the disease. Objective: The aim of the PROSINUS study was to quantify the direct medical costs of management of ARS in Spain.</p><p><strong>Methods: </strong>We performed a prospective observational study of 1610 patients with a clinical diagnosis of nonbacterial, uncomplicated ARS. According to the duration of symptoms by the European Position Paper on Rhinosinusitis and Nasal Polyps, patients were classified as having viral or postviral ARS with different levels of severity. Direct medical costs were calculated based on medical visits, use of diagnostic tools, and medications prescribed.</p><p><strong>Results: </strong>Overall, the mean (SD) direct medical costs per episode were €322.3 (301.2) vs €441.1 (344.3) for viral and postviral ARS episodes, respectively (P<.001). When viral and postviral disease were compared, the medical costs per episode were €245.0 (265.4) vs €328.4 (301.9) for medical visits (P<.001), €38.1 (64.0) vs €61.9 (78.8) for diagnostic tools (P<.001), and €39.2 (25.9) vs €50.8 (25.3) for medications prescribed (P<.001).</p><p><strong>Conclusion: </strong>The direct medical costs of postviral ARS episodes were higher than those of viral episodes (common cold). She subsequently tolerated linezolid. Skin biopsy of a macular lesion revealed superficial perivascular dermatitis of unspecific characteristics with a lymphocytic perivascular infiltrate limited to the epidermis. Eight weeks later, despite strict recommendations to avoid β-lactams, the patient took amoxicillin-clavulanic acid for a week with no adverse reactions. A detailed medical history compiled in the allergy department did not show any latex allergy or atopic disease. Written informed consent was obtained, and patch tests were performed with penicillin (10%), meropenem, ertapenem, and imipenem (5%). Readings at 48 and 96 hours yielded negative results. Skin prick tests (SPTs) and intradermal tests (IDTs) with immediate and delayed readings with benzylpenicilloyl octa-L-lysine, benzylpenilloate, penicillin, imipenem, and ertapenem yielded negative results. SPT with meropenem (100 mg/mL) was negative, whereas IDT with meropenem (1 mg/mL) became positive at 48 hours (Figure). All skin tests were performed on an area of previously inflamed skin. Given that the patient met the 5 clinical criteria [2] and had positive skin test results to meropenem, she was diagnosed with meropenem-induced SDRIFE, and carbapenems were forbidden. The positive result in the delayed IDT reading and the histopathological findings point to a type 4 hypersensitivity mechanism, as reported elsewhere [4,5].
