[Planning for death with dignity: an international clinical and educational approach under the umbrella of citizens' knowledge].

{"title":"[Planning for death with dignity: an international clinical and educational approach under the umbrella of citizens' knowledge].","authors":"José Manuel Jiménez-Rodríguez","doi":"10.15446/rsap.V25n5.109354","DOIUrl":"10.15446/rsap.V25n5.109354","url":null,"abstract":"<p><strong>Objective: </strong>To identify university students' knowledge and perception of the process of dignified death.</p><p><strong>Methods: </strong>Cross-sectional descriptive study involving postgraduate students of the International University of La Rioja (Spain), enrolled during the academic year 2020-2021. A simple random sampling was carried out with N=215. Inclusion criteria were to be aged between 25-65 years and have a clear command of Spanish. A self-adminis-tered questionnaire was used for data collection. A descriptive and bivariate analysis of the data obtained was carried out.</p><p><strong>Results: </strong>136 individuals participated. Knowledge of the right to a dignified death and its regulations represented an average of 5.43. The 6.56 think that in their environment peo-ple talk openly about death, and 6.59 believe that in their city the responsibility for the patient's health-illness process lies with the doctor. [Prevalence of burnout syndrome in family physicians working in Colombia and associated factors].

{"title":"[Prevalence of <i>burnout</i> syndrome in family physicians working in Colombia and associated factors].","authors":"Jorge L Ramirez-Rios, Juan F Pardo-Báez, Cesar J Fuentes-Estepa, José D Toledo-Arenas","doi":"10.15446/rsap.V25n5.100694","DOIUrl":"10.15446/rsap.V25n5.100694","url":null,"abstract":"<p><strong>Objective: </strong>To determine the prevalence and factors associated with the presence of burnout syndrome in practicing family physicians in Colombia in the year 2021.</p><p><strong>Methods: </strong>An analytical cross-sectional study was developed in which, at a given moment in time, data was taken from a series of subjects (Family Medicine professionals) and a survey and the Maslach questionnaire were carried out at the same time. This population was described. Next, the previously mentioned questionnaire was used, the group was divided into people with Burnout Syndrome and those without Burnout Syndrome, and with this information 2x2 tables were constructed, where the respective OR were obtained for each risk factor or protective. A point prevalence was obtained. To estimate parameters, confidence intervals were calculated and each of the hypotheses was tested with a p of 0.05.</p><p><strong>Results: </strong>A total of 206 surveys were analyzed. The data were analyzed considering a health systems performance evaluation framework.</p><p><strong>Results: </strong>Survey participants rated health system governance an average of 2.6/5.0, corresponding to low quality of governance. Despite the poor rating, leaders perceived improvements during the pandemic in the performance of purposes such as strategic vision and system supervision, and regarding they considered improvements in capabilities of risk management and information management.</p><p><strong>Discussion: </strong>This study provides an approach to the governance of the system, considering the subfunctions proposed in the evaluation framework, which constitutes an opportunity to know the failures and aspects that must be improved, even more, after going through the pressures of the covid-19. The system must learn from these aspects to be prepared for the next external shock. The disruption caused by the Covid-19 pan-demic revealed the fragility of healthcare systems around the world. Those countries with capacities in the development and manufacture of vaccines promoted the increase in financing for the development of possible vaccines. In the case of Mexico, once the world leader in vaccine production, it exhibited decades of neglect and little response capacity to meet the needs brought about by the pandemic. This paper analyzes the evolution of the capacities of the vaccine production industry in Mexico. The methodology makes a critical review of the literature regarding the evolution of the productive and technological trajectory of the vaccine industry. {"title":"[A cross-sectional study of suicide attempt reports in Colombia during the covid-19 pandemic].","authors":"María Ana González-Alvarez, Viviana A Rodríguez-Romero, Diana C Acuña, Isabel Del Socorro Moreno-Luna","doi":"10.15446/rsap.V25n4.108002","DOIUrl":"10.15446/rsap.V25n4.108002","url":null,"abstract":"<p><strong>Objective: </strong>To characterize the population that reports cases of attempted suicide during the confinement period and immediate post-confinement in Colombia according to the official reports collected by the epidemiological surveillance system of the National Institute of Health (Sivigila) and to determine if there are patterns of presentation and variations concerning the characteristics of the population that reported cases in 2019.</p><p><strong>Methods: </strong>A serial cross-sectional descriptive observational study and multiple correspondence analysis on epidemiological surveillance sheets of \"suicide attempts\" collected in Colombia from March 25 to December 31, 2020, and during the same period in 2019.</p><p><strong>Results: </strong>42,102 reports of \"Attempted Suicide\" were analyzed (23,745 in 2019 and 18,357 in 2020), and there was a reduction in approximately 23% of the number of reports in the year 2020 compared to the year 2019. Informed consent and assent were completed, anthropometric measurements were taken, the Fitnessgram battery tests were carried out, and a physical exercise program was implemented.</p><p><strong>Results: </strong>Post implementation of the physical exercise program, weight gain and waist circumference were reported in girls. Schoolchildren from 10 to 13 showed a decrease in cholesterol and triglycerides. Men performed better in the Fitnessgram battery, aerobic capacity and strength tests. In the Trunk Lift, and Sit and Reach tests, women exhibited greater flexibility.</p><p><strong>Conclusions: </strong>It is important to evaluate the physical condition of schoolchildren, as a strategy for the prevention of chronic non-communicable diseases. Science Direct , Scopus , Embase, BIREME OPS - OMS data bases were used to search the following words: \"Older adults/adulto mayor\", \"elderly/tercera edad\", \"poisoning/intoxicación\", \"overdoses/sobredosis\". The search was restricted to published articles from 2000 and cases related to poisonings among older adults (> 60 years old).</p><p><strong>Results: </strong>There is a need to strengthen a positive ageing model including strategies that handle social risks associated with the identified inequities among older adults. One of the risks that face older adults is intentional or unintentional poisonings related to drugs. Univariate analysis was per-formed to calculate frequencies and percentages. QGIS v.3.24 was used to establish the geospatial location of the cases.</p><p><strong>Results: </strong>A total of 937 cases were reported (55% classified as megacolon, with ICD-10 code K593). The years 2013 and 2009 were the years with the highest and lowest number of cases reported (145 vs. 40 cases). Seventy percent of the cases were of diagnostic impression (n=652) and only 10% corresponded to definitive cases (n=98). The 75.7% occurred in minors and 85% (n=782) corresponded to cases in the department of Caldas, mainly in Manizales (n=643).</p><p><strong>Conclusions: </strong>Between 2009 and 2019, a high prevalence of megacolon cases was evidenced in the department of Caldas, with a reduced trend in recent years. 7 out of 10 cases are reported as diagnostic impression and in this same proportion, it occurred in minors. Only 15% of the cases come from outside the department. For the design of the protocol, a literature search in indexed databases and validation of the content by experts in the field were carried out. Thirteen experts in the field of rehabilitation were convened to evaluate the content of the protocols in terms of adequacy, pertinence and relevance. Subsequently, a consensus was reached to define the protocols used in the software. A Likert-type scale with a score from 1 to 5 was used, where 1 refers to the lowest rating and 5 corresponds to the highest, scores that were then translated into three categories where 1 and 2 became \"Not important\", 3 and 4 were classified as \"Useful, not essential\", and 5 was classified as \"Essential\", for the calculation of the content validity index. {"title":"[Epidemiológical surveillance systems in the working environment: a perspective in Latin American countries].","authors":"Martín Gracia-Socha, Juan C Cerón-Arcos, Alejandro Nocua-Salazar, Juan J Vaca-González","doi":"10.15446/rsap.V25n4.99325","DOIUrl":"10.15446/rsap.V25n4.99325","url":null,"abstract":"<p><p>Through a review of scientific articles, information was identified on the implementation of Epidemiological Surveillance Systems (EVS) managed by employers within the framework of Occupational Health and Safety Management Systems (SG-SST) in Colombia and Latin America between 2010. and 2021. PubMed, Science Direct, SciELO, Scopus, Dialnet, and Gale databases were used to identify research that used employer-implemented EVS data. The topics covered, economic sectors, publication dates, countries of origin, populations studied, scope and quality parameters were described. It was evident that only 3 studies used EVS information derived from SG-SST to monitor workers' exposure to epidemiological risks and prevent occupational diseases and accidents. Of the 24 studies examined, 21 did not use EVS information, being considered negative findings. The lack of adherence to standards, the lack of EVS, the lack of knowledge of different instances of EVS and the absence of adequate studies result in poor coverage of workers exposed to occupational risks. Most studies highlight the need for EVS in the workplace. They also highlight the importance of generating massive, systematic, reliable and comparable data from primary sources such as companies and productive units to support public policies that guarantee the social security and health of workers.
