Documentary research, key informants, focus groups and in-depth interviews were used. A descriptive analysis of the proposed suggestions was carried out stratified by country and year.</p><p><strong>Results: </strong>In all countries, the introduction or improvement of strategies and mechanisms that ease interaction and mutual knowledge between professionals from different care levels is noted, especially joint cross-level meetings, direct communication mechanisms and strategies to strengthen the use of the referral and back-referral form. Less frequently, other organisational strategies are suggested aimed at strengthening the primary care model, access to secondary care and the coordination of the network's management.</p><p><strong>Conclusions: </strong>The results allow to generate recommendations for the improvement of clinical coordination in different health systems drawn from the suggestions made by doctors, a perspective that is seldom considered in the design and implementation of interventions in healthcare services. Suicidal deaths are estimated at 800,000 people each year. This is why its study has gained relevance to the extent that it is necessary to understand the phenomenon, its causes and theoretical positions that guide decision-making and the design of prevention strategies. However, the hegemonized explanation justifies the occurrence of the phenomenon from the interaction of multiple risk factors, on which various explanatory models with a biomedical-centric approach are based, which postulate suicidal behavior as a consequence of individual psychopathology. Socioeconomic and anthropometric data, obstetric history and general medical history was collected from 300 pregnant women aged between 18 and 37. BMI was calculated at the beginning and end of pregnancy. Statistical analysis of multiple linear regression was performed.</p><p><strong>Results: </strong>An increased BMI at the beginning and end of gestation positively correlated with age (Coefficient = 0.156; p=0.013, Coefficient = 0.153; p=0.011), diagnosis of gestational diabetes (Coefficient = 2.264, p=0.018, Coefficient = 0.153; p=0.011) and concern about weight gain during pregnancy (Coefficient = 1.226; p=0.038, Coefficient = 1.568; p=0.004). A low BMI correlated negatively with Intrauterine Growth Restriction (Coefficient = -3.208; p=0.005). Furthermore, a higher final BMI positively correlated with a diagnosis of hypertensive disorder (Coefficient = 2.733; p<0.001) and negatively with socioeconomic status (Coefficient = 2.239; p=0.045).</p><p><strong>Conclusion: </strong>Excessive weight gain before and during pregnancy is a predictive factor in the appearance of gestational diabetes and hypertensive disorders, differentially affecting women from low socioeconomic backgrounds. {"title":"[Characterization of infectious conjunctivitis in a fourth-level care center, Bogota, Colombia].","authors":"Tatiana Urrea-Victoria, Mariana Daza-Betancourt, Hannia Paola Barrios-Bermúdez, María Fernanda Jaimes-Escobar","doi":"10.15446/rsap.V25n6.110422","DOIUrl":"10.15446/rsap.V25n6.110422","url":null,"abstract":"<p><strong>Objective: </strong>The purpose of this study is to describe the characteristics of the populations that consulted to the emergency service due to conjunctivitis from March 1 of 2019 to March 1 of 2021 and evaluate the frequency of presentation of contagious ocular pathologies like is the case of bacterial conjunctivitis.</p><p><strong>Methods: </strong>A descriptive cross-sectional study was conducted in a historical cohort in the Department of Surgery, Ophthalmology Unit of the Hospital Universitario San Ignacio, Bogotá D.C. from March 2019 to March 1, 2021. The study included patients with a diagnosis of bacterial conjunctivitis and patients with a diagnosis of superinfected viral conjunctivitis, and excluded all patients with a positive result for COVID-19 and patients with symptoms suggestive of SARS-infectionCoV-2. Of the patients, 629 met the selection criteria.</p><p><strong>Results: </strong>Between March 2019 and April 2020, 534 consultations were registered in the emergency department of the ophthalmology unit, and from April 24, 2020 (when resolution 666 on biosecurity measures started) until March 2021, 95 consultations were registered. Demographic (age, sex, skin colour), clinical (blood pressure and acanthosis nigricans), anthropometric (weight, height, waist circumference and body mass index) and laboratory (basal and oral glucose tolerance test-stimulated blood glucose and insulinaemia) variables were analysed. The insulin resistance index was calculated. The abdominal hypertension-obesity phenotype was defined as the presence of systolic pressure ≥130 mm Hg and/or diastolic pressure ≥80 mm Hg or treated hypertension, waist circumference ≥80 cm in women and ≥90 cm in men. Sensitivity, specificity and predictive values of the abdominal obesity hypertension phenotype were calculated to identify dysglycaemia and insulin resistance.</p><p><strong>Results: </strong>Individuals with the hypertension-abdominal obesity phenotype showed a higher proportion of impaired glucose metabolism and insulin resistance than those without the phenotype (p<0.0001). A qualitative study with a descriptive phenomenological approach was carried out using face-to-face semi-structured qualitative interviews (N=8). We evaluated the perspective and experience of healthcare professionals caring for a deaf patient through semi-structured interviews (supported by field notes) which were audio recorded, transcribed, and analyzed using the Colaizzi Method.</p><p><strong>Results: </strong>Ninety-four significant statements emerged and were grouped into two themes: \"professional-patient interaction\" and \"conditioners of care\", each with three categories. The interactions were influenced by factors such as the tools and strategies used by the health professional, their emotions, and the patient's conditions; on the other hand, within the conditioning factors, the professionals described the procedural aspects, environmental and structural factors linked to care and access.</p><p><strong>Conclusions: </strong>The health professionals' experience impacts both themself and the quality of patient care. The study's objective was to characterize the implementation of CHCP in the department of Cundinamarca in 2021.</p><p><strong>Methods: </strong>Qualitative and cross-sectional design based on semi-structured individual ¡nterviews with representatives of health sector entities at the national, departmental, and municipal levels. The information was coded and categorized through thematic analysis.</p><p><strong>Results: </strong>CHCP and its operational model were readjusted with the arrival of the new national government in 2018. Additionally, in 2020, with the new local leaders, CHCP had to reposition itself in their political agendas. Due to the pandemic, many activities were suspended, including CHCP, because of epidemiological restrictions. The operational model of CHCP lacks guidance to direct its implementation; there are only some specific norms for comprehensive health care routes and integrated health service networks, which facilitated some progress. There is a poor coordination between CHCP and other national and local planning instruments.</p><p><strong>Conclusions: </strong>The implementation of CHCP in Cundinamarca has a still incipient process, characterized by circumstances at the political (changes in government), epidemiological (Covid-19), and even operational levels (lack of specific guidelines). [Coca leaf chewing as a risk factor for oral health].

The sample consisted of 200 participants, divided into two groups: 100 coca leaf users and 100 non-coca leaf users. To assess oral health, an instrument validated by five experts was used, where three items were evaluated: Temporomandibular joint, oral cavity and coca leaf chewing characteristics.</p><p><strong>Results: </strong>In the population studied, it was identified that the female gender is the big-gest consumer of coca leaf. In addition, it was observed that coca leaf consumption does not produce dental wear, fracture or mobility; however, its consumption gene-rates alteration of the gum (OR:42.67). According to the evaluation of muscle pain, it was observed that coca leaf chewing significantly affected the right insertion masseter muscle, compared to those who did not consume it (OR:17.47). Cardiac failure can shorten people's lives and severely affect their quality of life. There are not many alternatives to treat patients with advanced cardiac damage. Recently, it has been proposed that implantation of devices that provide mechanical circulatory support may help Chagas' patients with advanced IC to improve its quality of life.</p><p><strong>Objective: </strong>To analyze the case of a patient with Chagas disease who was not treated in a timely manner due to failures in primary care, the consequences on his health, and the cost of late treatment that could have been avoided.</p><p><strong>Materials and methods: </strong>Diagnostic methods revealed damage at the functional and structural level of the heart, classifying the condition as CHF, which led to the implantation of a technological medical device (DMT).</p><p><strong>Results: </strong>We present here the results of implanting such a device (Heart Mate 3) in a 54-year-old Colombian male patient with a 2 years diagnosis of advanced (IC) as a consequence of chronic Chagas infection.</p><p><strong>Conclusion: </strong>Chagas disease represents one of the oldest communicable diseases in the region of the Americas.
