{"title":"Sociodemographic factors in the development of Burnout Syndrome in South and Central American physicians.","authors":"Sandy N Vásquez-Navas, Enrique Gea-Izquierdo","doi":"10.15446/rsap.V25n2.105798","DOIUrl":"10.15446/rsap.V25n2.105798","url":null,"abstract":"<p><strong>Objectives: </strong>To analyze the prevalence of Burnout Syndrome and identify sociodemographic and extra-occupational factors that influenced the development of this syndrome in medical personnel in Latin American countries during 2009-2020.</p><p><strong>Methods: </strong>Bibliographic research and a descriptive study approach included 35 articles that consider the Maslach Burnout Index to diagnose BOS. The relationship between the sample size, the prevalence and the corresponding subscales was carried out by means of a correlation analysis.</p><p><strong>Results: </strong>The presence of both sexes was balanced in most of the studies. Twenty-five studies addressed the relationship of sociodemographic and extra-occupational factors and their relationship with the development of BOS, while 14 of them identified one or more of these factors that were found to be influential or not at the time of diagnosis of the syndrome. The prevalence of BOS ranged from 2.4-83.3 %. There was no correlation between sample size and BOS (p=0.5993); identifying a positive correlation in the prevalence of the three subscales of the MBI, with values of r2 =0.72 between AE and DP (p<0.001); 0.39 for AE and RP (p=0.029), and 0.53 for DP and RP (p=0.002).</p><p><strong>Conclusions: </strong>Sociodemographic and extra-occupational factors are considered significant in predicting and diagnosing burnout. It is estimated that the factors studied should be linked to the diagnosis of BOS in early stages. In 1919, the Treaty of Versailles, conceived at the end of World War I, created the International Labor Organization. After the Great Depression in 1929 and the World War II in 1939, followed a period of economic growth known as the Welfare State (circa 1945), ending with the world oil crisis in 1973, from which it is believed to have emerged Neoliberalism. China and the Union of Soviet Socialist Republics (USSR) are also studied as non-capitalist social organization systems. The sample consisted of 130 women deprived of liberty in the Cartagena District Jail, using convenience sampling. A questionnaire was applied to assess sociodemographic aspects, perceived health status and conditions of confinement. The information was analyzed using SPSS software version 27.</p><p><strong>Results: </strong>The inmates reported being 30-39 years old (28.5%), having children (91.5%), being from stratum one (74.6%), and having a high school diploma (53.8%). Among the physical aspects of the women, a normal BMI was highlighted (40%), no STD reported (96.2%), health status perceived as good (48.5%). Environmental conditions, few discomforts (41.5%), stress often (50%) and excellent interpersonal relations (48.5%).</p><p><strong>Discussion: </strong>Unlike other studies, the findings obtained by the research show a different panorama in relation to the biopsychosocial conditions of a group of women deprived of liberty; especially for the results of diseases, consumption of alcohol, drugs and tobacco; however, it is necessary to develop actions to prevent cardiovascular alterations and sexual and reproductive health. On the other hand, the perception of good health and few discomforts (food, violence, companions, etc.) was found, as well as a good interpersonal environment with prison guards and staff. Regarding obstructive pulmonary diseases, the scientific literature is diverse; finding variation in the frequency of these and therefore their relationship with COVID-19. The available evidence only details hospitalized patients, with insufficient information referring to health workers, so analyzing the main comorbidities in this group is helpful for the implementation and improvement of preventive programs. The objective of this study was to describe the prevalence and association of comorbidities with SARS-CoV-2 infection in health care workers.</p><p><strong>Methods: </strong>Analytical cross-sectional study, in workers of the main national reference center for respiratory diseases in Mexico, who came to care to rule out SARS-CoV-2 infection through a preventive program; calculation of proportions was performed for the sample size. Summary and association measures were calculated.</p><p><strong>Results: </strong>The prevalence of COVID-19 was 22.9 %. The most frequent comorbidities were: overweight obesity, smoking, hypertension, allergic rhinitis and asthma, with prevalences of 29.0 %, 13.2 %, 8.6 %, 5.5 %, 3.9%, 2.8 % respectively. Sociodemographic characteristics, dietary, ethnicity, child labour factor, and region-level rurality were considered as independent variables. A two-level structural equation model was used to assess the effect of individual- and state-level variables. Analyses were stratified by academic domains and global score.</p><p><strong>Results: </strong>Health-related determinants, including belonging to an ethnic minority and child labour were associated with a reduction in global scores (20.07, 95 % CI 19.81-20.33 and 10.62, 95 % CI 10.49-10.76 points, respectively), whereas the youth from higher socioeconomic status achieved a 2.21 points increase. COVID-19 pandemic and rurality did not implied significant changes in the scores, however, rurality was associated with a reduction of 0.01 points in foreign language score (English).</p><p><strong>Conclusions: </strong>Health determinants not only affect the common outcomes in health but also explain educational inequalities in Colombian youth. Beyond an increased risk of morbidity or mortality, as reported elsewhere, belonging to a minority, coming from a lower socioeconomic stratum and be in need to work, put in risk the personal fulfilment of youth, which entail poor future health performance. The Prisma checklist was used. Embase via Ovid, Lilacs, Medline via PubMed and Central were searched for randomized controlled trials. The Cochrane collaboration guidelines were followed and reported using the Preferred Reporting Items for Systematic Reviews and MetaAnalyses (Prisma) statement. The Revman program was used to consolidate the data. A sensitivity analysis was carried out.</p><p><strong>Results: </strong>A total of 17 randomized controlled trials met eligibility criteria. {"title":"Equations based on body mass index for body composition estimations of women presenting grade III obesity.","authors":"João Felipe Machado, Flávia Lúcia Conceição, João Régis Carneiro, Valéria Bender Bráulio, José Fernandes Filho","doi":"10.15446/rsap.V25n2.105516","DOIUrl":"10.15446/rsap.V25n2.105516","url":null,"abstract":"<p><strong>Objective: </strong>To develop and validate predictive equations to estimate the body composition of women with grade III obesity, using the body mass index (BMI) as a predictive variable.</p><p><strong>Methods: </strong>This cross-sectional study involved 104 patients treated at the hospital of the Universidade Federal do Rio de Janeiro randomly divided into two groups, the Equation Group, used to generate regression equations, and the Validation Group, used to validate the equations. Body fat mass (BFM), body fat percentage (BFP), skeletal muscle mass (SMM), fat-free mass (FFM) and total body water content (TBW) were valuated employing the bioimpedance method (InBody® 230).</p><p><strong>Results: </strong>Polynomial equations exhibited the best fit and a general trend of results normalized by height squared presenting higher coefficients of determination (r<sup>2</sup>) was noted, positively affecting equation validations. Only one exception was observed, since the body fat percentage index (BFPI) resulted in an even lower correlation with BMI. Only these variables exhibited low r<sup>2</sup> (0.11 to 0.29), while r<sup>2</sup> values ranged from 0.51 to 0.94 for the other results.</p><p><strong>Conclusion: </strong>Except for the BFP and BFPI, body composition can be estimated by the application of predictive BMI-based models.
