Therefore, given the probable scenario of sustained community transmission of SARS-CoV-2, and to meet the demand for care at all levels, it is essential to ensure the response capacity of the health system. We face an over-demand at the pre-hospital level, and, foreseeing an eventual saturation of the other levels of care, the following recommendations are made regarding material resources, health personnel, diagnostic studies, and specific antiviral treatment.","PeriodicalId":51931,"journal":{"name":"Revista Medica del Uruguay","volume":"83 1","pages":""},"PeriodicalIF":0.2,"publicationDate":"2020-04-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"80645210","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Presenta alta letalidad en estadios avanzados. En nuestro pais el promedio anual de casos seria de 129 en hombres, 117 en mujeres, con 50 y 30 muertes anuales respectivamente.Objetivo: describir y analizar el perfil clinico-epidemiologico y sobrevida de una cohorte de pacientes portadores de melanoma cutaneo asistidos en dos centros de referencia en Montevideo en el periodo comprendido entre 2008 y 2017.Metodo: se realizo un estudio descriptivo, retrospectivo, observacional de cohortes historicas en base a revision de historias clinicas de los pacientes con diagnostico de melanoma cutaneo asistidos en el Servicio de Oncologia del Hospital de Clinicas y en la Unidad de Oncologia cutanea del Instituto Nacional del Cancer en el periodo comprendido entre 2008 y 2017, con un seguimiento de los casos hasta el 31 de julio de 2018.Resultados: se analizaron 173 pacientes, mediana de edad 61,2 anos (14-89), 50,3% de sexo masculino y 49,7% de sexo femenino. La forma anatomo-clinica nodular fue la mas frecuente (32,7%), seguida de la extensiva superficial (28,9%), siendo el resto menos frecuentes. No fue alcanzada la mediana de sobrevida global (SVG), siendo la tasa de SVG a cinco anos de 68,5% y a diez anos de 54,5%. La mediana de SVG en EIV fue de 12 meses (IC 95% 8,5-21,5).Conclusiones: los resultados de nuestra serie en SVG son similares a los reportados a nivel mundial, al igual que en la mayoria de las caracteristicas clinico-epidemiologicas. Se trata del primer trabajo con reporte de sobrevida en melanoma cutaneo en nuestro pais.","PeriodicalId":51931,"journal":{"name":"Revista Medica del Uruguay","volume":"1 1","pages":""},"PeriodicalIF":0.2,"publicationDate":"2020-04-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"81984063","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

CT is the most frequently used study and PET/CT is graduallly more requested in the assessment of different neoplasms. This study aims to assess performance of CT and PET/CT for diagnosis of peritoneal carcinomatosis in digestive and gynecological tumours.Method: retrospective, observational study of 11 cases operated on peritoneal carcinomatosis after a CT and/or PET/CT had been performed. The results of both image studies were analysed separately and altogether and they were subsequently compared to intraoperative and pathology studies.Results: CT matched findings in 8 out of 10 cases (sensitivity 78%, specificity 100%, positive predictive value 100%, negative predictive value 33%), whereas PET/TC matched in 9 out of 11 patients (sensitivity 100%, PPV 81%). When considered together, sensitivity was 100% y el PPV de 82%.Conclusions: the results of the study confirm imagenology studies overestimate peritoneal carcinomatosis based on the characteristics of nodules and its uptake pattern. In CT/PET, SUV> 6 could increase the probability of diagnosing carcinomatosis. This small series of cases enables our suggesting CT and 18F-FDG PET-TC to be requested and analysed together to increase its efficiency, although surgical exploration continues to be the most effective method to diagnose and assess peritoneal carcinomatosis.","PeriodicalId":51931,"journal":{"name":"Revista Medica del Uruguay","volume":"185 1","pages":""},"PeriodicalIF":0.2,"publicationDate":"2020-04-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"80587090","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

El objetivo de este estudio es describir y evaluar la experiencia de un servicio de cirugía universitario con un modelo de entrenamiento simulado de anastomosis intestinal laparoscópica. Método: tres residentes de cirugía general completaron un programa de entrenamiento de cuatro semanas. Se utilizó un modelo biológico ex vivo en box trainer, evaluando objetivamente la realización de una anastomosis entero-entérica. Resultados: el tiempo de ejecución del procedimiento se redujo en una media de 15 minutos, con una mejoría significativa del desempeño según la escala OSATS. Discusión: la implementación de un programa validado y modificado de simulación en laparoscopía avanzada permitió obtener resultados positivos, utilizando para ello solo el 4% de la carga horaria semanal curricular. El modelo tiene una alta fidelidad, bajo costo y es fácilmente reproducible. Conclusiones: el entrenamiento simulado en laparoscopía es una herramienta obligatoria y beneficiosa durante la formación del cirujano general.","PeriodicalId":51931,"journal":{"name":"Revista Medica del Uruguay","volume":"11 1","pages":""},"PeriodicalIF":0.2,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"78966472","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

They may correspond to infectious or non-infectious skin or systemic pathologies. Lipschutz ulcer and Behcet disease stand out among non-infectious conditions. Objective: to describe four clinical cases of adolescents with non-infectious severe vulvar ulcers hospitalized in two institutions of the national integrated health system in Montevideo, with the purpose of providing professionals with tools for the adequate diagnostic and therapeutic approach. Discussion: a detailed anamnesis questionnaire and a thorough physical examination are of the essence for an adequate diagnostic and therapeutic approach. The clinical analysis should include considering the patient’s age, initiation of sexual intercourse, the number, topography and characteristics of the ulcers including finding out whether there is pain or not, as well as the association of systemic manifestations. It is important to learn about this rather unusual condition in girls and adolescents, whose diagnostic and therapeutic approach depends on the etiological orientation based on well-defined epidemiological and clinical criteria. This will contribute to improving the quality of assistance and minimizing biological and psychological effects.","PeriodicalId":51931,"journal":{"name":"Revista Medica del Uruguay","volume":"43 1","pages":""},"PeriodicalIF":0.2,"publicationDate":"2019-11-04","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"86622862","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Pelvic x-rays are necessary to identify this condition.Objectives: to assess the x-ray follow-up in these patients and the early detection of this complication in  our hospital. Method: the medical records and x-rays of 17 GMFCS III-V patients who were between 2 and 8 years old at the time of index pelvic x-ray were reviewed, measurement of the hip migration percentage according to Reimer´s index, the cervical-diaphyseal angle and calculation of the CPUP hip score for each hip. We assessed the follow-up x-ray a year later or later than that date, and in the even this had not happened, we would call the patients in for a control x-ray to identify the hips with an increase migration risk.Results: 3 (18%) out of 17 patients assessed underwent a new pelvic x-ray a year later. 6 patients (35%) underwent one later than one year after and prior to the control visit planned, 7 (41%) patients were never controlled and they were called for a new x-ray in 2018. One patient (6%) was lost during follow-up. One patient had an increased risk hip (CPUP Score 50-60%), the rest had normal migration percentages.Conclusions: few patients with PC GMFCS III-V underwent annual follow-up. Those monitored did not evidence progression of this condition. Recently its use was regularized in Uruguay.Objective: to analyze a preliminary therapeutic clinical experience with medicinal cannabis (CM) with a high content of cannabidiol (CBD).Method: an observational and retrospective epidemiological study of a cohort of 355 patients who attended to spontaneously consult CM in a private clinic between August 2016 and December 2017. During the first interview demographic data, clinical history, pharmacological, expectations and experience were collected previous with cannabis. Cannabis was indicated mostly with high CBD content (5.25% CBD and 0.2% tetrahydrocannabinol [THC]). In subsequent consultations, access to the CM was investigated and its response and adverse effects were assessed using analog scales. Descriptive statistics was used. Results: in the cohort studied, women with an average age of 67 years, tertiary education level predominated. The consultation pathologies were neurological (38%), rheumatic or arthro-degenerative diseases (37%), neoplasms (13%), psychiatric (4%) and miscellaneous (8%). The majority of these cases (60.6%) reported improvement of their symptoms and only 16.3% of the population studied presented mild side effects. The high costs and difficult management to achieve CM were causes for not starting or abandoning treatment. Conclusions: our preliminary study reflects a positive therapeutic response without significant adverse effects to CM with high CBD content. 60.6% of treated patients reported improvement of their symptoms. The decisive factors for a successful treatment are aimed at facilitating access to the CM, improving management to obtain it and reducing costs for greater accessibility.","PeriodicalId":51931,"journal":{"name":"Revista Medica del Uruguay","volume":"46 1","pages":""},"PeriodicalIF":0.2,"publicationDate":"2019-11-04","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"87551706","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Although in the last decade there has been a great regional and international effort to agree and standardize the test, in our country the laboratories show disparity in the techniques used and the initial dilution of screening. Likewise, in clinical practice, the indication of the test is often not well directed. In the Uruguayan Congress of Clinical Pathology of 2016, a Continuing Medical Education Course on the Use of ANA was given and once again the need to standardize both clinical and laboratory practices around the test was recognized. In order to generate a 1st Uruguayan Consensus on ANA, a working group was formed to discuss the issues to be agreed upon.","PeriodicalId":51931,"journal":{"name":"Revista Medica del Uruguay","volume":"20 2","pages":""},"PeriodicalIF":0.2,"publicationDate":"2019-11-04","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"72573698","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
