Therefore, many scoring systems have been developed to help with this decision making process. We aim to achieve a modified HEART value by combining the VAS value with the HEART score.</p><p><strong>Materials and methods: </strong>Data were collected on age, sex, duration of the symptoms, pain severity using a 10-point visual analog scale (VAS), and the presence of a major adverse cardiac event (MACE). The HEART score was calculated and modified (mHEART) by adding 1 point to the total HEART score for a VAS score of ≥7.</p><p><strong>Results: </strong>During the study period, 4781 patients were admitted, and 293 participants were analyzed. Of the patients, 34(11.6%) experienced MACE within a month after the encounter. The mean VAS scores were 5.65±1.44. However, 77(26.3%) patients had VAS scores ≥7. Taking 3 as the threshold, 42(14.3%) patients had HEART scores of 4 and above, where 47(16.0%) had mHEART scores ≥4. The mHEART scoring demonstrated better test indicators than the HEART score. According to the HEART score, 6(2.3%) of the 251 patients predicted as negative would develop MACE, but this number decreased to 1(0.4%) in 246 using the mHEART score.</p><p><strong>Conclusion: </strong>Although the HEART score performs reasonably well in discriminating patients who are MACE negative, it is possible to further improve the score by adding the VAS item. {"title":"Destructive Effects of Acidic Blood on the Intestines: Experimental Study.","authors":"Ozgur Caglar,&nbsp;Erdem Karadeniz,&nbsp;Binali Firinci,&nbsp;Muhammed Enes Aydin,&nbsp;Onur Ceylan,&nbsp;M Dumlu Aydin,&nbsp;Mecit Kantarci","doi":"10.5152/eurasianjmed.2021.20035","DOIUrl":"https://doi.org/10.5152/eurasianjmed.2021.20035","url":null,"abstract":"<p><strong>Objective: </strong>This study aimed to investigate the destructive effects of acidic blood on the intestinal structures, which has been reported as the most hazardous biochemical result of vagosympathetic autonomic imbalances in intensive care unit patients with subarachnoid hemorrhage (SAH).</p><p><strong>Materials and methods: </strong>In total, 27 hybrid rabbits were used; 5 rabbits were used as a control group, 7 as the SHAM group into whom 1 cc saline was injected into the cisterna magna, and the remaining 15 were used as the study group. These animals received 1 cc of autologous arterial blood injection into the cisterna magna to create the SAH group. Blood pH values were recorded before, during, and after the experimental procedures. Computed tomography was performed to examine the intestinal morphology. Normal and degenerated epithelial cell densities of the intestine were estimated by stereological methods. The relationship between pH values and intestinal tissue changes was analyzed statistically using the Mann-Whitney U test.</p><p><strong>Results: </strong>The mean blood pH values were 7.364±0.042 in the control group, 7.326±0.059 in the SHAM group, and 7.23±0.021 in the study group. Intestinal epithelial cell injury, desquamation of villus, and cell loss were observed. Most interlaminar epidural steroid injection (ESI) procedures are performed with a blind technique known as loss-of-resistance (LOR) without an imaging guide. This study aims to evaluate the success rate of the LOR technique in interlaminar epidural steroid injection under fluoroscopic control.</p><p><strong>Materials and methods: </strong>Patients who underwent interlaminar ESI owing to a history of at least 3 months of chronic low back and leg pain not responding to medications and physical therapies were included in an observational trial. Participants' age was between 27 and 88 years, and they had an American Society of Anesthesiologists physical status of I-III. The patients were placed in a prone position, and a Tuohy needle was introduced at the level of the L5-S1 interlaminar foramen using fluoroscopic image with an anteroposterior view. A lateral view was obtained when the LOR was felt. The procedures that achieved epidural spread by contrast agent in the first attempt were deemed successful. Those that did not and those that had false positive LOR were regarded as unsuccessful.</p><p><strong>Results: </strong>Interlaminar ESİ was administered to 150 patients. The procedure's success and failure rates were 76% (114 patients) and 24% (36 patients), respectively. A total of 58.3% (21 patients) of patients who underwent an unsuccessful procedure had a false LOR, whereas 41.6% (15 patients) of the same group exhibited other causes. Sex, age, and body mass index (BMI) showed no statistical significance in terms of procedural success. There were statistically significant differences in the distance between the skin and the epidural space according to the body mass index groups.</p><p><strong>Conclusion: </strong>The LOR technique identified the epidural space in most epidural procedures. However, in some cases, LOR was shown to be inadequate. This study aimed to investigate sequential organ failure assessment (SOFA) score, systemic inflammatory response syndrome (SIRS), quick SOFA (qSOFA), and qSOFA + lactate criteria (qSOFA+L) in the diagnosis and prognosis of sepsis.</p><p><strong>Materials and methods: </strong>A retrospective study was performed that included all patients diagnosed with sepsis between January 1, 2013 and December 31, 2017 in Izmir Tepecik Training and Research Hospital Infectious Diseases and Clinical Microbiology Clinic.</p><p><strong>Results: </strong>A total of 976 patients diagnosed with sepsis (mean age 72.5±13.7 years, 52.7% women) over five years were included in this study. Of all patients admitted to the emergency department and diagnosed with sepsis, 37.4% (n=365) were hospitalized and 52.3% (n=191) of these patients died. Emergency department mortality was 12.5% (n=122). The mortality rate was higher in patients with qSOFA and qSOFA+L criteria ≥2 in the emergency department. There was no statistically significant difference in terms of SIRS, qSOFA, or qSOFA+L criteria among patients who died in the hospital. The SOFA score (area under receiver operator characteristic curve, AUC=0.89) was highly discriminative in predicting sepsis. When the SOFA score was>11, its sensitivity and negative predictive values were both 100%. The SOFA score (AUC=0.75 and 0.72, respectively) was also highly discriminative in predicting emergency and in-hospital mortality. When the SOFA score was>11, the sensitivity and specificity of predicting emergency department mortality were 63.5% and 78.8%, respectively. The sensitivity was 65.8% and the specificity was 75.5% when describing in-hospital mortality for SOFA scores>9.</p><p><strong>Conclusion: </strong>The SOFA score was highly sensitive and predictive in the diagnosis of sepsis. In this study, we aimed to address the impact of gender on symptoms and comorbidities in patients with OSA.</p><p><strong>Materials and methods: </strong>This cross-sectional study was conducted among 1,317 consecutive patients, who were admitted to the Sleep Apnea Clinic of the Marmara University Hospital between November 2015 and October 2018, and who completed questionnaires and a sleep study with cardiorespiratory polygraphy. OSA was defined as Apnea Hypopnea Index (AHI) ≥15/hour.</p><p><strong>Results: </strong>In all, 1,042 patients (334 women) fulfilled the inclusion criteria. OSA was observed in 589 patients (56.5%). Women were older than men (50.2±12.5 versus 45.6±15.1 years) and had lower AHI (22.1±20.1 versus 26.8±21.9 events/h). In the OSA group, women were older (53.7±11.5 versus 47.8±12.8 years) and more obese (BMI 34.6 versus 31.8 kg/m<sup>2</sup>). Symptoms were categorized as frequent/very frequent, and women with OSA complained more about daytime fatigue (74.6% versus 63.7%), nocturia (69.7% versus 51.8%), headache in the morning (50.0% versus 28.4%), depressive mood (49.0% versus 19.5%), and restless legs symptoms (43.1% versus 17.2%), than did men (all p values <0.05). Comorbidities were observed more in women than in men (lung disease [25.4% versus 13.7%], hypertension [29.6% versus 15.0%], diabetes [20.3% versus 11.3%], and hypothyroidism [14.0 % versus 4.1%]). In regression analysis, age (OR 1.03, p<0.001), BMI (OR 1.13, p<0.001), and male sex (OR 2.08, p<0.001) were significantly predictive for OSA while history of tonsillectomy was protective (OR 0.48, p=0.033).</p><p><strong>Conclusion: </strong>Fatigue, nocturia, headache, depressive mood, restless leg, and comorbidities were observed more in women. OSA-related symptoms develop late and/or the referral of women for diagnostic evaluation of OSA is delayed. Different imaging algorithms and mathematical calculations have been developed for the evaluation of LVF in cardiac magnetic resonance imaging (MRI), and numerous studies are still being carried on this. In our study, LVF was calculated by two different measurement methods in MRI and were compared with transthoracic echocardiography (TTE) to assess the correlation and the consistency of these with TTE.</p><p><strong>Materials and methods: </strong>In this study, 31 patients with left ventricular dysfunction due to different etiologies were evaluated with simultaneous TTE and MRI. In the Cine MR images, LVF parameters of ejection fraction, end-diastolic volume, end-systolic volume, and myocardial mass were measured using short axis images (short axis method) and short axis plus four chamber and two chamber images (combined method). The results were compared with the results from TTE.</p><p><strong>Results: </strong>We found that the combined and the short axis-based calculations of ejection fraction, end-diastolic volume, end-systolic volume, and myocardial mass in cardiac MRI showed correlation and consistency with those calculated via echocardiography. This retrospective study aimed to evaluate the effect of shakuyakukanzoto on chemotherapy-induced metoclopramide-uncontrolled hiccups.</p><p><strong>Materials and methods: </strong>We retrospectively investigated the medical records of all the consecutive patients with lung cancer who were prescribed with shakuyakukanzoto (Tsumura Co. Tokyo, Japan) for chemotherapy-induced hiccups in our hospital from September 2013 to November 2017. In the medical record from the initiation of shakuyakukanzoto until the start of the next chemotherapy, when there was description of \"complete or partial disappearance of hiccups\" after the prescription of shakuyakukanzoto, it was judged as a complete or partial response. A statement of \"unchanged,\" no description of improvement, or exacerbation was judged as no change.</p><p><strong>Results: </strong>Of the 49 chemotherapy courses in 15 patients with lung cancer, 93.9% had a \"partial or complete\" response within a few hours after the administration of shakuyakukanzoto. The effect of shakuyakukanzoto was observed irrespective of the pathological type of lung cancer, platinum-containing or non-platinum chemotherapy, and with or without other anti-hiccup drugs. No adverse event was observed.</p><p><strong>Conclusion: </strong>Shakuyakukanzoto may offer an effective pharmacological approach to treat chemotherapy-induced metoclopramide-uncontrolled hiccups. In addition, NF ‐κ B is overexpressed in viral infections. Up-regulated NF ‐κ B plays a crucial role in the production of pro-inflammatory cytokine storms and triggers a variety of cellular responses and func-tions, including dendritic cell maturation, chemotaxis, and cell phagocytosis. NF- κ B signaling is also a key regulator of lipopolysaccharide-induced pulmonary inflammation [4]. There is a strong crosstalk between NF ‐κ B and cytokine activation and production, and cytokines and pro-inflammatory mediators activate NF ‐κ B, after which NF ‐κ B activates the cytokine production. This cycle can enter an uncontrolled, positive feedback loop, resulting in the production of a cytokine storm. It is suggested that inhibition of NF ‐κ B up-regulation results in the mitigation of pro-inflammatory cytokine signaling. In addition, several pre-clinical studies have demonstrated the beneficial effects of NF -κ B-targeting inhibitors in the prevention and early treatment of lung injury induced by toxic agents in pre-clinical models [4-7]. We suggest that attention be paid to the development of anti-inflammatory drugs targeting NF -κ B to prevent or treat pneumonia induced by SARS-CoV-2 virus causing COVID-19. Since interventions for airway management are carried out in the intensive care unit (ICU), contamination risk of healthcare personnel within an ICU with Covid-19 is high. Even though ICU workers are fighting against the disease at the frontline, we have not encountered any physicians, nurses, or co-workers who have been infected with Covid-19 since the beginning of the pandemic on the 11th of March 2020 in our 14 ICUs, where each unit consists of 16 beds, with a total of 224 beds.
