{"title":"Report on Advances for General Medicine in 2019: Neurosurgery, Urology, Gynecology and Obstetrics, and Internal Medicine.","authors":"Hakan Doneray","doi":"10.5152/eurasianjmed.2020.040620","DOIUrl":"https://doi.org/10.5152/eurasianjmed.2020.040620","url":null,"abstract":"Neurosurgery The use of graft materials in dural defects (cranial or spinal) are still a problem for neurosurgeons because they may lead to some serious complications such as meningoneural adhesions, scar tissue, pachymeningitis or soft tissue infections, the leakage of cerebrospinal fluid, neural herniations, and tethered cord syndrome [1]. Although most graft materials have been implicated in the cranial region, there are a few studies regarding spinal duraplasty [2]. One paper published in the Eurasian Journal of Medicine drew attention to an aspect that has not been studied in depth. That experimental and controlled study conducted by Calikoglu et al. investigated the effectiveness of collagen matrix in spinal duraplasty in terms of cellular inflammatory response, fibroblastic proliferation, foreign body reaction, and capsule formation. They showed that the collagen sponge might be suitable for spinal duraplasty and might be a useful agent in patients whose dura cannot be closed primarily because of retraction, shrinkage, or excision [2]. In the experimental femur fracture control group, we created only experimental femur fracture. In the ebselen treatment group, we administered ebselen treatment with creating an experimental femur fracture. We administered ebselen intraperitoneally at 5 mg/kg once daily for 1 month after the 1st day of experimental femur fracture in the ebselen treatment group. We evaluated the recovery status of fractured femurs at the end of 1st month with radiographic, histopathological, and immunohistochemical methods.</p><p><strong>Results: </strong>According to the radiographic fracture healing scores, ebselen treatment increased the extent of new bone formation and fracture cartilage callus significantly compared to the control group. According to the histopathological recovery scores, ebselen treatment significantly improved healing scores compared to the control group. Ebselen treatment increased the expression scores of bone healing markers in the ebselen treatment group, such as vascular endothelial growth factor and osteocalcin, compared to the control group.</p><p><strong>Conclusion: </strong>We demonstrated that ebselen treatment increases the formation of new bone in the femur in an experimentally created femoral fracture model. The aim of this review is to analyze all reported cases of neuraxial or perineural bleeding after performance of a locoregional technique since the last guidelines update in 2018. We included articles indexed by MEDLINE, Scopus, and Google Scholar. We analyzed the patient's age, surgical procedure, pre-operative anticoagulant and antiplatelet therapy, type of anesthetic procedure, vertebra level of the procedure, diameter and point type of the needle, hematoma type (spinal, subdural, epidural), signs and symptoms, time to imaging, and time to treatment and outcome. During our bibliographic research, we identified 5637 unique articles that were eligible according to our protocol criteria, identifying 18 separate cases of neuraxial bleeding. The purpose of this study was to adapt the Diabetes Empowerment Scale-Short Form (DES-SF) to old people and to the Turkish language, and to determine its validity and reliability.</p><p><strong>Materials and methods: </strong>This methodological type study was conducted between 1 January and 30 March 2017 with 106 old people fitting the criteria of acceptance in the study who attended the geriatrics clinic of a university hospital. In determining validity, language equivalence, content, and predictive validity were used, while reliability was tested with temporal invariance, Cronbach's alpha coefficient, and item total correlation. We examined its reliability and validity via item analyses, content validity (expert panel), confirmatory factor analyses, and construct validity (exploratory factor analyses).</p><p><strong>Results: </strong>The Cronbach's alpha of the scale was 0.883. The scale items were grouped under a single factor in accordance with the original. Confirmatory and exploratory factor analysis showed good fit signs.</p><p><strong>Conclusion: </strong>After conducting validity and reliability testing on the adaptation of the Diabetic Empowerment Scale-Short Form to old people and the Turkish language, it is was concluded that it is a reliable scale to determine the empowerment levels of old people. Based on the statistical analyses applied to evaluate the validity and reliability of the scale obtained by adapting the DES-SF to old people, it was inferred that it is an instrument with high validity and reliability. The scale is a short and practical instrument to evaluate the empowerment levels of old people. {"title":"Is There a Relationship Between Epitaxis and Anatomical Variations?","authors":"Ismail Salcan, Abdulkerim Olgun","doi":"10.5152/eurasianjmed.2020.19222","DOIUrl":"https://doi.org/10.5152/eurasianjmed.2020.19222","url":null,"abstract":"<p><strong>Objective: </strong>The aim of our study is to introduce the relationship between epistaxis and anatomical variations and present the current treatment approach.</p><p><strong>Materials and methods: </strong>Forty-five patients (28 males and 17 females) that presented to otorhinolaryngology clinics at Mengucek Gazi Training and Research Hospital with pre-diagnosed epistaxis between October 2018 and April 2019 were included in the study. Age, sex and structural causes and anatomical localization of epistaxis in patients were analysed.</p><p><strong>Results: </strong>The median age of the patients was 42, ranging from 20 to 60 years. Focus of bleeding was observed on the right in 16 cases (35.5%), left in 14 cases (31.1%) and bilateral in nasal cavity in 15 cases (33.3%). Bleeding was from the anterior septum in front of the maxillary ostium line. Anatomical variations were seen in 31 patients (68.8%) (septal deviation in 12, septal perforation in two, and concha hypertrophy in 17).</p><p><strong>Conclusion: </strong>Epistaxis is one of the most common ear, nose, and throat emergencies. We aim to report a 7-year-old boy diagnosed with NMLFS and moderate developmental delay. {"title":"Angiographic Findings and Outcomes of Bronchial Artery Embolization in Patients with Pulmonary Tuberculosis.","authors":"Orkun Sarioglu, Ahmet Ergin Capar, Melike Yuksel Yavuz, Umit Belet","doi":"10.5152/eurasianjmed.2020.19221","DOIUrl":"https://doi.org/10.5152/eurasianjmed.2020.19221","url":null,"abstract":"<p><strong>Objective: </strong>We aimed to evaluate the angiographic findings and outcomes of bronchial artery embolization in tuberculosis patients and to compare them with those of non-tuberculosis patients.</p><p><strong>Materials and methods: </strong>Patients who underwent bronchial artery embolization in a single interventional radiology department with hemoptysis were reviewed. A total of 89 patients (66 males and 23 females; mean age 52.71±15.37) were incorporated in the study. The patients were divided into two groups: tuberculosis group (n=36) and non-tuberculosis group (16 malignancy, 22 bronchiectasis, 6 pulmonary infection, 5 chronic obstructive pulmonary disease, 4 idiopathic; n=53). Angiography and embolization procedure were performed by interventional radiologists with 5, 10, and 20 years of experience. Angiographic findings were classified as tortuosity, hypertrophy, hypervascularity, aneurysm, bronchopulmonary shunt, extravasation, and normal bronchial artery. Chi-square test was used to compare angiographic findings between tuberculosis and non-tuberculosis patient groups.</p><p><strong>Results: </strong>Bronchopulmonary shunt was found to be significantly higher in the tuberculosis group as compared to that in the non-tuberculosis group (p=0.002). Neither of the groups showed a statistically significant difference with respect to recurrence (p=0.436).</p><p><strong>Conclusion: </strong>Bronchial artery embolization is a useful and effective treatment method of hemoptysis in tuberculosis.
