This article presents current treatment recommendations for IC/BPS, covering the full therapeutic spectrum from lifestyle and dietary measures to last-resort surgical options. Behavioral interventions, patient education, pelvic floor physical therapy, and psychological support form the cornerstone of first-line management. When these measures prove insufficient, pharmacological treatments - including conventional and atypical analgesics, as well as systemic oral therapies such as pentosan polysulfate sodium - along with intravesical instillations of glycosaminoglycans or dimethyl sulfoxide, may be considered. Interventional techniques, including hydrodistension, intravesical injections of onabotulinumtoxinA, and sacral neuromodulation, represent effective options for refractory patients. Excisional surgery and urinary diversion remain reserved for a carefully selected subgroup of patients presenting with severe, documented bladder-centric disease. Rigorous phenotypic patient selection is the cornerstone of all therapeutic decisions and the principal determinant of prognosis.
","PeriodicalId":516865,"journal":{"name":"The French journal of urology","volume":" ","pages":"103169"},"PeriodicalIF":1.2,"publicationDate":"2026-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148582003","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, compared to standard techniques such as transurethral resection of the prostate (TURP) and holmium laser prostate enucleation, it has certain limitations with respect to reintervention rates. The aim of our study was to identify the risk factors for reintervention due to recurrent obstruction within 12months following a photovaporization.

Methods: This retrospective, multicenter cohort study include patients treated with laser Greenlight 180W-XPS. The primary endpoint was the need for an early reintervention for obstruction. The identified causes for reintervention were: bladder neck sclerosis, urethral stricture, adenoma regrowth, managed by internal urethrotomy, PVP, or TURP.

Results: Data of 777 patients were analyzed and 32 reinterventions (4.1%) were recorded within 12months following surgery. Groups were comparable on preoperative data. The predictive factors for reintervention risk were: age (OR 0.92, 95% CI 0.88-0.96), laser usage time (OR 0.10, 95% CI 0.01-0.74), and energy delivered/prostate volume (OR 0.66, 95% CI 0.54-0.80). The objective of this study was to evaluate the impact of mFI-5 on the oncological prognosis of patients treated with a Radical Nephroureterectomy for non-metastatic upper tract urothelial carcinoma.

Patients and methods: In this multicentric, retrospective, study we collected all patients managed for non-metastatic upper tract urothelial carcinoma who underwent radical nephroureterectomy between 2015 and 2023. The mFI-5 was calculated based on the presence of the following five comorbidities: congestive heart failure within 30 days before surgery, diabetes mellitus, chronic obstructive pulmonary disease, partially or totally dependent functional health status at the time of surgery, and hypertension requiring antihypertensive treatment. Patients were grouped as not frail (mFI-5=0), intermediate (mFI-5=1), and severely frail (mFI-5≥2). Surgical outcomes were evaluated based on perioperative complications and length of hospital stay. Prognostic outcomes included progression-free survival (PFS) and overall survival (OS).

Results: We included 170 patients. From these patients, 47% (n=80) were not frail, 30.58% (n=52) were intermediate, and 22.35% (n=38) were severely frail. Frail patients were more likely to be older (P=0.03), current smokers (P=0.042), and to have chronic kidney disease (P=0.01). Patients with intermediate or severe frailty were more likely to experience postoperative acute kidney injury (P=0.012), thromboembolic event (P=0.003), significant blood loss (P=0.041), and require vasopressor support (P=0.01). Frail patients had a longer hospital length of stay (P<0.001) and unplanned readmission (P=0.02). The 2-year PFS rates were 45% in the severely frail group, 64% in the intermediate group and 87% in the low mFI-5 group, while the corresponding 2-year OS rates were 46%, 78%, and 92% respectively.

Conclusion: Frailty, as measured by the mFI-5, is a predictor of postoperative complications, functional decline, and poor oncological prognosis after RNU for UTUC. Its preoperative assessment is crucial for personalized patient management.
","PeriodicalId":516865,"journal":{"name":"The French journal of urology","volume":" ","pages":"103171"},"PeriodicalIF":1.2,"publicationDate":"2026-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148611546","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
