It is required to re-evaluate and to develop alternatives, particularly those using the patient's own tissue (autologous techniques). We report the case of a 53-year-old patient who underwent surgical implantation of the semitendinosus tendon in the mid-urethral space via a retropubic approach, as the patient refused the implantation of conventional synthetic material. The tendon was harvested from the left knee via an anterior approach using a technique well-known by orthopedic surgeons and implanted as a retropubic MUS. The procedure, performed under general anesthesia in an outpatient procedure, was performed without peroperative complication. Postoperative bladder outlet obstruction symptoms disappeared after one week. The functional outcome and the patient reported outcome at 6 months follow-up were successful (PGI-I=1, USP=0/6/0, ICIQ-UI-SF=0).</p>","PeriodicalId":516865,"journal":{"name":"The French journal of urology","volume":" ","pages":"103137"},"PeriodicalIF":1.2,"publicationDate":"2026-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148102316","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

While several treatment options exist for moderate SUI, women with severe incontinence - particularly due to intrinsic sphincter deficiency (ISD) or a fixed urethra - often have limited effective options after failed procedures. The artificial urinary sphincter (AUS) is the gold standard for severe incontinence in men, but its use in women remains limited due to surgical complexity, higher complication rates, and restricted availability to specialized centers. The procedures were performed by two andrologists at a university center. Defined as chronic pelvic pain, pressure, or discomfort perceived in relation to the bladder, accompanied by at least one urinary symptom, IC/BPS encompasses significant clinical and pathophysiological heterogeneity. A thorough medical history, targeted physical examination, and validated questionnaires such as the O'Leary-Sant score (ICSI/ICPI) form the foundation of initial evaluation. Patient phenotyping - distinguishing in particular the bladder-centric from the sensitization phenotype - is essential to guide both diagnostic and therapeutic strategies. Complementary investigations include a voiding diary, urine culture, imaging (ultrasound as first-line, CT scan and MRI as second-line modalities), and cystoscopy (preferably with hydrodistension under general anesthesia), which allows identification of Hunner's lesions, measurement of anatomical bladder capacity, and targeted biopsies. Urodynamic testing is reserved for cases of diagnostic uncertainty or associated voiding dysfunction. These recommendations aim to standardize the diagnostic approach in order to optimize individualized management in patients suffering from IC/BPS.</p>","PeriodicalId":516865,"journal":{"name":"The French journal of urology","volume":" ","pages":"103166"},"PeriodicalIF":1.2,"publicationDate":"2026-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148498672","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
