This prospective study was carried out to determine the prevalence of carbapenem resistance in clinical isolates of Klebsiella species phenotypically and to confirm for the presence of bla NDM (New Delhi metallo-b-lactamase) and bla KPC (Klebsiella pneumoniae carbapenemase) genes in these isolates.\u0000 \u0000Materials and Methods: The 336 clinical isolates of Klebsiella species were tested for carbapenemase production by phenotypic tests; the Modified Hodge Test (MHT) and Combined Disc Test (CDT). The carbapenemase producers were further confirmed for presence of bla NDM and bla KPC genes using polymerase chain reaction (PCR). \u0000 \u0000Results: Resistance to carbapenem was seen in 34.52% of clinical isolates of Klebsiella species and majority of these isolates were from inpatient units (59.23%) of the Hospital. Maximum cases were seen in males (63.69%) and the positivity rate was high in > 61 years of age (27.08%). On genotypic characterization, bla NDM was the predominant (15.52%) gene detected as compared to bla KPC (10.34%) gene. However, these 2 genes could not be detected in some isolates suggesting other genes responsible for carbapenem resistance.\u0000 \u0000Conclusion: Emergence of resistance to carbapenem is a matter of concern. There was predominance of bla NDM gene in our hospital. Microalbuminuria when detected early, helps to delay progression to proteinuria and overt diabetic nephropathy. This study aims to find out the role of glycaemic control and development of microalbuminuria in type -2 diabetes.\u0000 \u0000Materials and Methods: This study was done at a South Indian tertiary care centre. Our case control study included 100 subjects with diabetes as cases and 100 age and sex-matched healthy individuals without diabetes as the controls. Blood samples were collected from both groups to assess fasting and post prandial blood sugar levels, as well as HbA1c levels. Urine samples were also analysed for microalbumin using midstream early morning samples. Our data was analysed using statistical methods such as Chi square test, unpaired Student’s t-tests, and linear regression method.\u0000 \u0000Results: The results displayed a striking rise in the average levels of urine microalbumin in individuals with diabetes, as compared to those without the condition. Furthermore, there was a considerable escalation in urine microalbumin levels observed with longer durations of diabetes. Additionally, there was a strong and significant connection between HbA1c and urine microalbumin levels amongst patients diagnosed with type-2 diabetes.\u0000 \u0000Conclusion: It is vital to screen for microalbuminuria in individuals with type-2 diabetes mellitus at an early stage, in order to lower the likelihood of developing nephropathy in diabetes and end-stage kidney failure. Additionally, suboptimal glycemic management in type-2 diabetes significantly raises the chances of developing microalbuminuria.

The culprit usually attempts self-annihilation due to extreme degree of guilt or attempt to escape from criminal punishment. An elderly couple’s dead bodies were found by the home nurse when she came for duty. Old lady was found on her bed with a pillow on her face and the husband of the lady was found hanging in the adjacent room. The lady was bedridden for the last 7 years due to a stroke and home nurses were employed for the care. The husband was the main caregiver. The rapid urbanization and children moving towards greener pastures leads to home alone elderly people all over the world. Old age homes are alarmingly mushrooming everywhere and not contributing much to alleviate the stress of geriatric groups. Psychological autopsy, crime scene investigation and other corroborative findings helped to conclude the case as Dyadic Death. The caregiver burnout syndrome is an important entity in this case.

Mangiferin and giloy are the important Ayurvedic medications. Both have effective control over diabetes glycemic status. It is possible that the treatment of certain herbal remedies led to an increase in intracellular Ca2+ levels or that glucose-stimulated insulin secretion was enhanced through activating glucose transporter-2 (GLUT-2) receptors. In our work, we rigorously examined the chemical and biological properties of giloy and mangiferin. Clinical study on the effect of mangiferin and giloy combined extract solution in type 2 diabetic subjects and determine the level of Fasting Blood sugar, insulin and HbA1c in type 2 diabetic subjects before and after therapy.\u0000 \u0000Materials and Methods: A randomized cross-sectional study was conducted on type 2 diabetic subjects. Study design with group 1 diabetic subjects (n=45) of FBS less than or equal to 160 mg/dl) and diabetic subjects group 2 (n=49) of FBS more than 160 mg/dl were included with regular antidiabetic drugs; in addition, for the period 30 days, twice a day, 500 mg mangiferin and 500 mg giloy extract was given to group 1 and group 2. The difference between the groups was calculated using the t-test and Mann-Whitney U test.\u0000 \u0000Results: It is evident from the statistical data a significant change seen in HbA1c levels , FBS levels and insulin values while comparing group 1 and group 2 diabetic subjects. However, in group 1 and group 2 creatinine levels did not show much change.\u0000 \u0000Conclusion: Our study findings indicate that mangiferin and giloy extract is a better regime for the effective glycaemic control by an increase in insulin levels. TPO is a thyroid-specific antigen. TPO gene mutations affect thyroid hormone synthesis, causing full or partial iodide organification abnormalities. In this study, we aimed to look at the single nucleotide polymorphisms (SNPs) in the TPO gene and find possible relation of these SNPs to the development or prognosis of hyperthyroidism.\u0000 \u0000Materials and Methods: Blood samples were collected from hyperthyroid patients (n=75) and healthy individuals (n=25). The concentration of triiodothyronine (T3), Tetra-iodothyronine (T4), and thyroid stimulating hormone (TSH) were estimated in all samples. The autoantibodies TPOAbs and TR-Ab levels were measured using the ELISA assay. The extracted genomic DNA from blood of participants was investigated for polymorphism in the SNPs rs1126799, rs1126797, and rs732609 within the TPO gene. Statistical methods were used to evaluate the association between the TPO SNPs studied to hyperthyroidism susceptibility.\u0000 \u0000Results: No significant correlation was observed between the SNPs studied and prognosis of hyperthyroidism. However, it was found that hyperthyroid patients carrying the TPO rs1126799 T allele (CT + CC genotypes) had significantly higher serum levels of TPOAb compared to those with the TT genotype (P<0.01). Similarly, hyperthyroid patients with the TPO rs1126797 C allele (CT + TT genotypes) and TPO rs732609 A allele (CA + AA genotypes) exhibited significantly elevated levels of TPOAbs compared to individuals with the CC genotype and AA genotype, respectively (P<0.01).\u0000 \u0000Conclusion: There was no correlation between the TPO rs1126799, rs1126797, and rs732609 polymorphisms with the occurrence of hyperthyroidism in the Iraqi population.
