{"title":"Marked Improvement in Glycemic Control with Exenatide on Addition to Metformin, Sulfonylurea and Insulin Glargine in Type 2 Diabetes Mellitus, a Real World Experience","authors":"Salina Esmail, S. Objective: Therefore, we assessed effect of Exenatide administration at 6 months and for at 1 year on glycemic control, lipids, body weight [BW], daily insulin dose and hypoglycemic events. Methods: Records of 164 subjects, 126 men and 38 women administered Exenatide between January 2011 and December 2013 are included in this report. Exenatide was initiated at 5 mcg subcutaneously twice daily [BID] in obese subjects, BMI > 30 kg/m2, with C-peptide > 1 ng/d, and HbA1c 7.5% - 9.5%, while receiving daily metformin 2000 mg, Sulfonylurea Glimepiride 8 mg and insulin Glargine [GLAR]. Exclusion criteria were creatinine > 1.5 mg/dL and liver enzymes > 2.5 times upper limit of normal. Indices of glycemic control include fasting plasma glucose levels and HbA1c. Lipids include serum concentrations of total, LDL and HDL cholesterol. Other endpoints are body weight, daily insulin dose and number of hypoglycemic events per patient during 4 weeks prior to initiation of Exenatide, at 6 months and 1 year of therapy. Results: In 37 subjects, Exenatide was discontinued within 1 - 3 weeks; 29 due to onset of nausea and vomiting. Seven of these also complained of abdominal pain and in these, serum amylase and lipase were elevated indicating presence of acute pancreatitis. One subject discontinued because of chest pain. Fasting plasma Glucose remained unchanged following Exenatide administration. However, HbA1c declined significantly denoting improvement in overall glycemic control without significant changes in body weight, daily insulin dose and hypoglycemic events. Lipid panel improved as well. Conclusion: Exenatide may be an appropriate adjuvant option in obese subjects with Type 2 diabetes mellitus with lack of desirable glycemic control while receiving therapy with Metformin, Glimepiride, and insulin Glargine. Methods: Secondary data were retrieved from the Malaysian National Diabetic Registry which included all patients who received care. The data from the study on the socio-demographic, diabetes complications, clinical and treatment characteristics were analyzed using descriptive statistics. Cox regression was performed to estimate the hazard ratio for comorbidities, tobacco use, duration of diabetes and socio-demography characteristics upon time to diabetic complications. Results: Adjusted for other covariates, increase number of comorbidities contributed the highest hazard ratio risk: 1 comorbid (aHR: 2.47, 95% CI: 2.39, 2.55), 2 comorbidities (aHR: 4.34, 95% CI: 4.22, 4.47), 3 comorbidities (aHR: 6.56, 95% CI: 6.31, 6.81) and 4 comorbidities (aHR: 9.13, 95% CI: 8.20, 10.17). Other factors: age > 40 years (8%) Malays (27%) and smokers (10%) have hazard risks to develop diabetic complications. Conclusions: Increase in number of comorbidities will increase the risk of getting diabetes complications. The body composition and fat distribution are related to this parameter. The neck circumference (NC) is an anthropometric measure that has been used to evaluate the overweight and accumulation of subcutaneous fat in the upper body, regardless of age. It is also a useful tool to predict insulin resistance and other risk factors. This research aimed to study the relationship between the NC and glycemia in adolescents and, thus, to analyze the applicability of this anthropometric measure in the screening of glycemic alterations in this group. Methods: The study population consisted of 600 adolescents aged 10 to 19 years. Data on sex, birth date, NC, fasting blood glucose and stage of sexual maturation was collected. Results: The evolution of sexual maturation generated a significant difference in the NC in both sexes, but not affected glycemia. Thus, the association between NC and glycemia was investigated according to this parameter. Among the pre-pubertal adolescents, the NC was significantly higher among those who had glycemia within borderline values, and the opposite occurred among pubertal adolescents. Among post-pubertal adolescents, NC was similar between the two groups. A negative correlation was found in all adolescents, as well as among those who were in the pubertal and post-pubertal stages of sexual maturation. Differently, adolescents in the pre-pubertal stage showed a positive correlation between the NC and the biochemical parameters. Conclusion: This study showed that the relationship between the NC and glycemia varies according to the stage of sexual maturation. Immune system components are modulated during T2D, with the most apparent modifications in adipose tissue, pancreatic islets, liver, and circulating leukocytes. The aim of this survey was to evaluate the role of IL-1 in the etiopathogenesis of nephropathic T2D. Methods: In this case-control investigation, the study population consisted of 58 T2D patients with proteinuria (nephropathy T2D cases) as the case group and 76 T2D cases without proteinuria (non-nephropathy T2D cases) as the control group. Blood samples were obtained from all individuals and ELISA approach was carried out to measure IL-1 levels in samples. Results: Our experiments demonstrated that T2D patients with nephropathy had significantly increased levels of IL-1 in their blood in comparison to T2D patients without nephropathy. Their effects on glycemic control, lipid profile, insulin resistance and beta cell function has not been systematically studied in India. The objective of this study was to evaluate the effect of lifestyle modification and OADs on metabolic parameters in recently diagnosed uncomplicated T2DM patients. MATERIAL METHODS: A total of consecutive sixty four (64) cases of recently diagnosed uncomplicated T2DM in the age group of 30 - 60 years were studied. They were evaluated for weight, body mass index (BMI), fasting plasma glucose (FPG), 2hr post glucose plasma glucose (2hrPGPG), HbA1c, lipid profile, serum fasting insulin, c-peptide, HOMA-IR and HOMA-β. They were divided into four groups according to increasing order of HbA1c values (6.5% - 6.9%, 7% - 7.5%, 7.6% - 8.5%, 8.6% - 8.9%). These four groups were subjected to lifestyle modification (LSM), monotherapy with metformin (1 g) and LSM, dual drug therapy i.e. metformin (1 g), glimepiride (1 mg) and LSM, triple drug therapy i.e. metformin (1 g), glimepiride 1 mg, sitagliptin 100 mg) and LSM respectively. These patients were followed up after three months of therapy. They were evaluated for the same metabolic parameters and compared with their baseline value. Fourteen (14) patients were lost to follow up. RESULTS: We found 91%, 92.8%, 53.3% and 60% of our patients from above four different groups achieved target glycemic control (HbA1c ≤ 6.5%). DM is the most common metabolic disorder with magnesium deficiency, having 25% to 39% prevalence. Osmotic diuresis accounts for a portion of the magnesium loss. Plasma magnesium and intracellular magnesium concentrations are tightly regulated by several factors. Insulin is the most important factor. Insulin may modulate the shift of magnesium from extracellular to intracellular space. In the present study we try to estimate serum magnesium in type 2 DM and also to find the association with duration of T2DM. Materials and Methods: The study will be conducted in Thanjavur Medical College, Thanjavur. 25 diabetes patients and 25 healthy volunteers were included in the present study. Age group of the study subjects were between 35 and 67 years. Approximately 3 ml of fasting serum samples and 1 ml of whole blood samples will be collected in Clinical Chemistry laboratory for the estimation of fasting glucose (GOD-POD method), 2-hour post glucose levels and HbA1c (HPLC method), Lipids profile was estimated by IFCC approved clinical chemistry analyzer (fully automated). Results: There was statistically significant difference in BMI, fasting blood sugar (FBS), Post Prandial Blood Sugar (PPBS), lipid profile and Mg (in DM groups). HbA1c (P < 0.001) levels were statistically significance with DM patients. Serum Mg levels were correlated showed negative correlation except High Density Lipoprotein (HDL) had positive correlation. The Serum Mg levels are significantly decreased along with the duration of the diabetes. Conclusion: Hypomagnesemia is seen in type 2 DM and inverse correlation with duration of the diabetes also. A sum of fourteen different types of spices at different percentages were taken into consideration to propel the research activities forward, viz; ginger, garlic, onion, red chili, turmeric, cumin, cinnamon, clove, coriander, fenugreek, black pepper, nigella, carom seed and cardamom. The healthy experimental wistar albino rats were made diabetic by injecting alloxan monohydrate intraperitoneally a dose of 150 mg/kg body weight. The rats of Group-D and Group-E were treated with the sample, mixed spices at a dose of 100 mg/kg and 200 mg/kg body weight respectively with the lab diet while in case of Group-C, glibenclamide at a dose of 0.5 mg/kg body weight was introduced with the lab diet for a total period of 21 days where as for the same days span, Group-A and Group-B were continued feeding with only lab diet. Data suggest that the mixed spices of both doses applied in Group-D and Group-E had exerted effects significantly (p < 0.05) in lowering the blood glucose level. On 22nd day, in Group-D and Group-E, the FBG level was found to be reduced by around 40.66% and 41.18% respectively compared to their respective initial days where in Group-C, the reduction was around 51.90% in comparison to its initial day. Again, in case of 2hPG level measurement, on 22nd day, both the groups, Group-D and Group-E were found with the decreased level of around 19.38% and 39.11% respectively compared to their respective initial days while the Group-C was shown with the reduced of 44.26% in comparison adopting same manner. Furthermore, various analyses confirm the presence of several micro and macro nutrients and high amount of dietary fiber in the spices mix. Hypomagnesaemia accelerates the rate of diabetic complications. Objective: To analyze the association of serum magnesium (Mg) in Type 2 Diabetes Mellitus with and without complication of peripheral neuropathy. Design: A cross sectional study. Setting: This research was carried out in medicine department, Peoples Medical College Hospital Nawabshah from May 2016-April 2017. Sample Size: Total 271 patients of both genders with Type 2 DM with and without peripheral neuropathy, each group after fulfilling the selection criteria were included. Material and Methods: After a short-lived consultation, the subjects were categorized for variable analyses like sex, age, Type 2 Diabetes Mellitus with and without peripheral neuropathy, duration of diabetes mellitus and presence of hypomagnesaemia. Clinical examination with monofilament was applied for diagnosis of peripheral neuropathy. Blood samples for magnesium analysis were collected in fasting condition. Results: In 271 diagnosed patients of Type 2 diabetes mellitus, 180 male and 91 were females. Peripheral neuropathy was observed in 136 subjects out of them 94 males and 42 were females. While 135 were without peripheral neuropathy out of them 86 males and 49 were females. Normal magnesium was seen in 119 (43.91%) and low magnesium was present in 152 (56.09%) patients overall. A decreased serum level of magnesium was observed in 56.09% diabetic subjects with peripheral neuropathy and 50% subjects with diabetes without peripheral neuropathy. Desa tersebut memiliki beberapa kelompok wanita tani yang berkecimpung dalam pengolahan keripik dan dodol salak, namun dari segi kualitas dan kuantitas produk yang dihasilkan masih sangat rendah. Pelaksanaan penyuluhan dan pelatihan bagi Kelompok Wanita Tani Macenning dan Kelompok Wanita Tani Salak Canning di Desa Buntu Mondong, terutama dalam hal diversivikasi produk pangan merupakan solusi yang ditawarkan dalam rangka meningkatkan nilai tambah (value added) produk keripik dan dodol salak. Penyuluhan dan pelatihan yang telah dilaksanakan antara lain adalah penyuluhan sanitasi dan hygiene proses produksi, aplikasi pelabelan dan kemasan pangan, penyuluhan P-IRT dan sertifikasi halal produk, analisis kimia gizi dan umur simpan produk, perancangan peralatan dan diversifikasi pembuatan produk di kedua mitra tersebut. Diharapkan dengan kegiatan ini dapat membina dan memacu kedua mitra untuk menghasilkan produk yang bersaing dipasaran sehingga dapat meningkatkan kehidupan ekonomi di desa tersebut. Potensi bahan baku lokal berupa limbah ampas sagu sangat melimpah khususnya di Desa Buntu Terpedo Kecamatan Sabbang, namun sampai saat ini belum dimanfaatkan sebagaimana mestinya hanya dibiarkan menumpuk pada tempat-tempat pengolahan tepung sagu sehingga menyebabkan pencemaran lingkungan. Salah satu solusi yang dapat diterapkan adalah pengembangan pakan alternatif yang diupayakan dengan pemanfaatan limbah industri sebagai bahan pakan lokal yaitu ampas sagu. Tujuan program KKN-PPM ini agar masyarakat bisa memnafaatkan limbah ampas sagu sebagai pakan ternak dengan cara Fermentasi. Metode pelaksanaan berupa pelatihan dan pendampingan aspek teknis pembuatan pakan ternak dari ampas sagu.
