This oversight not only compromises the social justice and health equity goals of public health efforts but it also calls for a more inclusive approach that systematically addresses these deficiencies at every stage of a public health response. To achieve this, we propose <i>Age- and Race-conscious Interventions done Equitably</i> (ARIE), a novel analytical framework grounded in critical race theory and critical gerontology. ARIE is based on a four-step approach, which aligns with different stages of public health interventions. This suggests that clinical translation in of itself cannot be the only measure of its own success, especially when the most marginalized patients, are neglected in the development and implementation of medical innovations. This raises the question of whether a system that is narrowly focused on technical achievement can meet the moral obligations of medicine and public health. We argue that traditional technocratic standards are failing to integrate normative considerations into biomedical translation. What is needed is a translational domain that moves beyond safety and efficacy toward anticipating how proposed technologies will be effective in society as it exists. Fractious debate is being waged by GAC-permissive and GAC-restrictive proponents which has, importantly, spilled-over into the political domain. In the paper, an ethics analysis of gender-affirming care for youth is employed through the application of a set of relevant, ethics lenses, i.e., individual and relational autonomy; patient welfare (comparative benefits and burdens of GAC); health equity; traditional-distributive, formal and social justice; and injustices related to health research and political interference. In the author's view, this multi-lens analysis supports the continued use of a potentially adapted version of the gender-affirming care model in the assessment and treatment of transgender and gender-diverse youth.
