DME is an important cause of severe vision loss in type 2 diabetes. Hyperpermeability of retinal blood vessels and subsequent formation of edema and hard exudates are the key clinical features. \u0000Method: This systematic review and meta-analysis, conducted following PRISMA guidelines and employing the PICO format, aim to explore about impact of treatment of diabetic macular edema on visual impairment in people with type 2 diabetes mellitus, focusing on treatment, complications, and prognosis. Inclusion criteria encompass diverse study designs (RCTs, observational, quasi-experimental, and case-control studies) investigating impact of treatment of diabetic macular edema on visual impairment in people with type 2 diabetes mellitus among treatment, complications, and prognosis, while exclusion criteria filter out studies lacking relevance to impact of treatment of diabetic macular edema on visual impairment in people with type 2 diabetes mellitus. \u0000Result: After conducting three levels of screening, the results of our search in Pubmed get 6 articles, whereas the results of our search on SageJournal get 42 articles, on Lancet 21, and on Scient direct get 60 articles. Records remove before screening are 91, so we get 78 articles for screening. After we screened based on record exclude, we compiled a total of 10 papers. We included four research that met the criteria. \u0000Conclusion: There is a particularly robust response to anti-VEGF therapy in both groups when glycemic control is optimized, highlighting the critical importance of communication between the physicians managing the DME and those managing systemic diabetes control. Improving our understanding of the factors that contribute to anti-VEGF response for DME therapy may help to enhance DME treatment outcomes, particularly through the concerted coordinated efforts of the treating retina specialist and the endocrinologist or primary care physician.

Corticosteroid use has been linked to cataract development. This study aims to explore the relationship between oral corticosteroid (OCS) exposure and the long-term risk of cataract. \u0000Method: Following PRISMA guidelines, a systematic review and meta-analysis were conducted to investigate this association. Relevant studies were identified through comprehensive searches across multiple databases. Data included cataract incidence/prevalence and OCS therapy. \u0000Results: A total of 1,270 articles were retrieved from online databases (PubMed, SagePub, SpringerLink and Google Scholar). After three rounds of screening, five articles directly relevant to the systematic review were selected for full-text reading and analysis. These five studies collectively involved 27,250 cases of cataract following oral corticosteroid (OCS) exposure, while 47,267 controls did not develop cataracts. No statistically significant association is found between OCS and cataract formation (OR 0.117, CI 95 0.0085 to 1.6021, p0.11). \u0000Conclusion: Cataracts in oral corticosteroid (OCS) users result from abnormal lens cell movement and protein composition changes due to protein adduct formation. Increased OCS dosage and frequency correlate with higher cataract risk. However, evidence quality was deemed low to moderate, highlighting the need for well-designed RCTs to minimize bias and heterogeneity.

To reduce the risk of stroke recurrence and death, individuals who have survived their first stroke must undergo regular medical follow-up. Understanding the risk factors for stroke recurrence can help create plans and treatments to lessen the burden of strokes. Regular medical follow-up and understanding these risk factors can help create effective treatments and plans to reduce stroke recurrence. \u0000Methods: This systematic review focused on full-text English literature published between 2014 and 2024, adhering to PRISMA 2020 principles. Without a DOI, editorials and review papers that were published in the same journal as the submission were not accepted. ScienceDirect, PubMed, and SagePub were among the many web resources used to compile the literature. \u0000Result: Using reliable resources including Science Direct, SagePub, and PubMed, the study examined over 7,000 publications. Following the determination that seven publications warranted a further inquiry, a more thorough examination of the full corpus was conducted. \u0000Conclusion: Risk factors such as hypertension, alcohol consumption, diabetes, female gender, non-adherence to treatment, and cardiac causes contribute to high stroke recurrence rates in sub-Saharan Africa. Treatment effectiveness varies among ethnic groups, with DAPT being effective in patients with ICAD. Statin-based therapies reduce recurrent stroke risk but increase hemorrhagic stroke risk. The incidence of POST is between 21-71.8. POST causes discomfort to patients which can prolong hospital stay. In this study, a comparison was made between MgSO4 nebulization 20 mg/ KgBW and ketamine nebulization 0.5 mg/ KgBW on the incidence of POST after general anesthesia at RSUP Dr. Hasan Sadikin Bandung . This study used a double blind randomized controlled trial design. 72 elective surgery patients were randomly divided into 2 groups. Group M was given MgSO4 nebulization while group K was given ketamine nebulization. Nebulization was performed 15 minutes before induction of general anesthesia. POST monitoring was performed at 1, 4 and 24 hours after extubation using POST scoring. In this study, it was found that the incidence of POST in group M, namely the 1st hour of 13.5, the 4th hour of 10.8 and the 24th hour of 0 was lower than that in group K, namely the 1st hour of 28.5, the 4th hour of 32.5 and the 24th hour of 0. Statistical test results p0.033 showed a significant difference in group M better than group K at the 4th hour in reducing the incidence of POST. MgSO4 and ketamine have antinociceptive and anti-inflammatory effects with the mechanism of action of blocking NMDA receptors that inhibit calcium influx that avoids central sensitization. MgSO4 also has a muscle relaxing effect and reduces airway mucus secretion, thus reducing the risk of trauma during endotracheal intubation. The conclusion of this study is that MgSO4 nebulization shows a lower incidence of POST than ketamine nebulization.
