The viral infection initiates inflammatory responses leading to endothelial damage and coagulation cascade dysfnction. Spread of COVID-19 has been associated with disseminated intravascular coagulation (DIC) and subsequent coagulopathy. Initially coagulopathy in COVID-19 patients result in significant elevation of D-dimer, fibrin/fibrinogen degradation products (FDP), and abnormalities in coagulatory parameters, which resulting in formation of thrombus and eventually death.</p><p><strong>Methodology: </strong>Present report intends to summarize the information of the research reports available so far on the complications of formation of unusal blood clots (thrombosis) during COVID-19 infection and its therapeutic strategies. Extensive web search was done for various reports associating COVID-19 infection with increased coagulopathy and abnormal coagulatory parameters such as PT, PTT, and platelet counts; along with increased D-dimer and fibrinogen levels.</p><p><strong>Results and conclusion: </strong>Findings of these research reports were summarized to recommend cautions for clinicians while treating COVID-19 patient. Screening of coagulatory parameters upon admission and during entire course of treatment is recommended, especially those who are at increased risk of thrombosis. Also, anticoagulant treatment can be used as thromboprophylaxis measure. {"title":"3 cases of variant hemoglobin Hb A2-Niigata detected by falsely high HbA1c values.","authors":"Mika Ito,&nbsp;Kou Sano,&nbsp;Masafumi Koga","doi":"10.1016/j.cca.2020.08.036","DOIUrl":"https://doi.org/10.1016/j.cca.2020.08.036","url":null,"abstract":"<p><strong>Background: </strong>HbA1c measurements in blood are used to monitor diabetes status.</p><p><strong>Methods: </strong>We detected 3 cases (1 diabetic case, 1 borderline diabetic case, and 1 case with normal glucose tolerance) of variant hemoglobin Hb A2-Niigata based on falsely high HbA1c values measured by high-performance liquid chromatography (HPLC).</p><p><strong>Results: </strong>In all 3 cases, HbA1c values measured by the HPLC method were higher than the reference range whereas the HbA1c values measured by immunoassay, plasma glucose and glycated albumin were within the reference range. The results of the genetic test revealed heterozygous mutation GTG (Val) → GCG (Ala) in codon 1 of the δ-globin gene in all 3 cases, based on which Hb A2-Niigata was found. Although until now Hb A2-Niigata has been reported in 3 cases, this is the first report on Hb A2-Niigata with falsely high HbA1c values.</p><p><strong>Conclusions: </strong>In this report, 3 cases of Hb A2-Niigata were found in a single institution in a short period; therefore, Hb A2-Niigata would exist frequently in a certain area. We recently established a method for measuring plasma apoE-rich HDL. This study aimed to investigate the relationship between metabolic syndrome (MetS) and apoE-rich HDL levels.</p><p><strong>Methods: </strong>The apoE-rich HDL-cholesterol (HDL-C) levels and metabolic characteristics of 113 patients were analyzed.</p><p><strong>Results: </strong>The MetS group (n = 58) had significantly lower apoE-rich HDL-C and a lower apoE-rich HDL-C/HDL-C ratio (apoE-HDL (%)) compared to the non-MetS group. The prevalence of MetS was increased when apoE-HDL (%) decreased. In simple regression analyses, apoE-HDL (%) was significantly inversely correlated with visceral fat area (r<sub>s</sub> = -0.370, P < 0.001) and plasma triglycerides (r<sub>s</sub> = -0.447, P < 0.001) and positively correlated with low-density lipoprotein (LDL) mean particle size (r<sub>s</sub> = 0.599, P < 0.001) and HDL mean particle size (r<sub>s</sub> = 0.512, P < 0.001). Stepwise multiple regression analysis revealed that LDL mean particle size, a component of the atherogenic lipoprotein profile, was an independent predictor of apoE-HDL (%) (adjusted R<sup>2</sup> = 0.409).</p><p><strong>Conclusions: </strong>Plasma apoE-rich HDL levels might be a valuable indicator of MetS. Sensitive and specific immunoassays provide the ability to identify exposure to SARS-CoV-2, to determine seroconversion, to confirm eligibility for donation of convalescent plasma as well as play an essential part in epidemiological studies. We report on the validation of the Ansh Laboratories SARS-CoV-2 IgG and SARS-CoV-2 IgM ELISA immunoassays. These assays were evaluated for detection of anti-SARS-CoV-2 IgG and IgM antibodies for clinical use in our hospital as part of an orthogonal testing algorithm recommended by the CDC.</p><p><strong>Methods: </strong>Diagnostic specificity and sensitivity of the IgG and IgM ELISA assays were tested using samples confirmed to be negative or positive for COVID-19 by RT-PCR. We also evaluated precision, analytical interference, and cross-reactivity with known cases of infection with other viruses. Additionally, we validated concordance with molecular and other serological testing and evaluated seroconversion in our patient population.</p><p><strong>Results: </strong>The IgG and IgM ELISA assays showed acceptable precision, were robust to analytical interference and did not exhibit cross reactivity with specimens positive for common respiratory viruses. Both assays exhibited 95% agreement with a primary screening serological assay utilized at our institution as well as with a reference laboratory semi-quantitative method. Recent studies have identified NLR, PLR, and LMR correlate with patient outcomes in cervical cancer patients however there remains uncertainty regarding the optimal time point for assessing these markers.</p><p><strong>Methods: </strong>We retrospectively reviewed cervical cancer patients underoing definitive chemoradiation therapy (dCRT). NLR, PLR, and LMR values were identified before, during, and after dCRT and both relative and absolute changes in these values were calculated and compared with patient outcmoes.</p><p><strong>Results: </strong>Ninety-nine patients who met the includsion criteria were identified. NLR values before, during, and after dCRT correlated with progression free survival (PFS) and overall survival (OS). In addition, increasing NLR after treatment was associated with worse PFS and OS. LMR before and after treatment had a positive correlation with PFS however increasing LMR during dCRT was found to have a negative correlation with PFS and OS.</p><p><strong>Conclusions: </strong>NLR serves as a prognostic indicator irrespective of timing with response to dCRT. We sought to determine the levels and prognostic value of serum S100B during the first three days of life in premature neonates (<34 weeks) that later developed NBI in the form of either intraventricular hemorrhage (IVH) or periventricular leukomalacia (PVL).</p><p><strong>Methods: </strong>This is a prospective case - control longitudinal study. Each case (n = 29) was matched according to birthweight and gestational age to one neonate with normal head ultrasound scans.</p><p><strong>Results: </strong>Neonates with NBI, had significantly higher S100B concentration during the first three days of life. In both groups S100B was significantly higher on the first day when compared to the next two days of life showing a downwards trend. Serum S100B on the first day was the best predictor for adverse neonatal outcome such as death or II-IV IVH grade. The study aimed to evaluate the diagnostic and prognostic value of Presepsin in very elderly patients compared to point-of-care Procalcitonin (PCT), C-reactive protein (CRP), and early warning scores (EWSs).</p><p><strong>Methods: </strong>This study prospectively enrolled 250 patients aged at least 75 years old, presenting to the Emergency Department of Siriraj Hospital with suspected sepsis during September 2019 and January 2020. They were classified into three groups: non-sepsis, sepsis, and septic shock. Biomarkers and EWS values at admission were determined. PCT was analyzed with non-BRAHM method.</p><p><strong>Results: </strong>Presepsin had valuable diagnostic utility for sepsis (AUC 0.792), comparable to PCT (AUC 0.751, p = 0.22) and CRP (AUC 0.767, p = 0.47). It also showed similar prognostic accuracy (AUC 0.683) with PCT (AUC 0.691, p = 0.68) and CRP (AUC 0.688, p = 0.85). The combination of Presepsin, PCT, and an EWS yielded the highest diagnostic accuracy for sepsis and septic shock and highest prognostic accuracy for 30-day mortality.</p><p><strong>Conclusion: </strong>Presepsin is a valuable diagnostic and prognostic biomarker for sepsis in very elderly emergency patients.
