This study compares spleen-preserving surgeries with total splenectomy for treating primary splenic hydatid cysts, focusing on short- and long-term outcomes in the Jordanian context, a region particularly affected by this condition.\u0000 \u0000 \u0000 \u0000 This retrospective analysis was conducted on 18 patients diagnosed with primary splenic hydatid cysts at two Jordanian hospitals from January 2015 to June 2021. Selection criteria included confirmed diagnosis and complete medical records. Surgical approaches, including laparoscopic partial splenectomy, cystectomy, and cyst deroofing, supplemented by albendazole therapy, were compared based on patient demographics, symptoms, surgical details, complications, and recurrence rates.\u0000 \u0000 \u0000 \u0000 The study group was composed of (n=7, 38.9%) male and (n=11, 61.1%) female patients, with an average age of 33.7 years. Most presented with left upper quadrant pain. Postoperative complications occurred in 22% of patients, with an 11% recurrence rate during follow-up. No significant statistical difference in recurrence rates was observed between spleen-preserving surgeries and total splenectomy. These findings highlight the efficacy of less invasive, spleen-preserving techniques in managing primary splenic hydatidosis, showing comparable outcomes to total splenectomy with minimal impact on recurrence rates.\u0000 \u0000 \u0000 \u0000 Spleen-preserving surgery offers a viable alternative to total splenectomy in treating primary splenic hydatid cysts. The condition, as its name suggests, is generally reversible. PRES typically affects individuals around the age of 45 and is rarely seen in the elderly population.\u0000 \u0000 \u0000 \u0000 In this report, we present the case of an 83-year-old woman who presented to the emergency department with a history of seizures and visual disturbances. Upon examination, her Glasgow Coma Scale (GCS) was 14, and she had a bilateral round, reactive regular pupils. A plain CT head was performed, which revealed multiple hemorrhagic areas in the occipital region associated with vasogenic edema. MRI was done which was suggestive of Posterior Reversible Encephalopathy Syndrome (PRES). The individual underwent conservative management in our institute and gradually recovered.\u0000 \u0000 \u0000 \u0000 Posterior Reversible Encephalopathy Syndrome (PRES) typically presents in middle-aged individuals, with infrequent occurrences in the elderly population. Early radiological and clinical assessments are crucial for the prompt diagnosis of this condition. Additionally, it is essential to identify predisposing factors such as hypertension, eclampsia, electrolyte imbalances, renal failure, autoimmune diseases, and the use of cytotoxic drugs for optimum management of the patient.\u0000 \u0000 \u0000 \u0000 Posterior reversible encephalopathy syndrome (PRES) is generally reversible, as its name implies. For each of the selected studies, risk of bias was evaluated using the COSMIN Risk of Bias Checklist. Then The COSMIN checklist was used to evaluate the entire text of the article for methodological quality.\u0000 \u0000 \u0000 \u0000 Considering the inclusion criteria, 80 documents were evaluated. According to the COSMIN’s criteria for evaluating the risk of bias, the current study findings revealed the included studies’ limitations in assessing the three versions of CD-RISC cross-cultural and content validity as well as their stability (e.g., conducting test re-test), whereas the majority of psychometric studies of CD-RISC25, and CD-RISC-2 rated as very good or adequate in terms of structural validity. In terms of quality assessment of the included studies, the current study indicated that investigating the structural validity of the CD-RISC was mainly done based on EFA (Exploratory Factor Analysis), and confirmatory factor analysis was absent.\u0000 \u0000 \u0000 \u0000 The general result indicates the acceptability of the quality of the studies. Emergent operative intervention is typically warranted in non-cancer patients. Cancer patients present a unique challenge as they have increased risk of pneumoperitoneum due to local tumor invasion, radiation therapy and frequent endoscopic procedures. There is a paucity of literature on the management of patients undergoing chemotherapy who present with pneumoperitoneum. We conducted a scoping review to identify and synthesize preliminary evidence on the presentation, management, and outcomes of this patient population.\u0000 \u0000 \u0000 \u0000 A scoping review of cases of pneumoperitoneum in cancer patients from 1990 – 2022 was conducted using the Arksey and O’Malley five-stage approach. Inclusion criteria were a known diagnosis of cancer, chemotherapy within 6 months of presentation, and imaging confirmation of pneumoperitoneum. Our exclusion criteria were cancer diagnosis at the time of presentation, perforation secondary to local cancer invasion, and last chemotherapy session greater than 6 months prior to presentation.\u0000 \u0000 \u0000 \u0000 34 cases (8 pediatric, 26 adults) were identified. The median time from the last chemotherapy treatment to presentation with pneumoperitoneum was 14 days. 21 patients were managed operatively and 13 were managed non-operatively. The most common source of perforation was multiple sites along the bowel. 30-day mortality was 33.3% for the operative cohort and 23.1% for the nonoperative group.\u0000 \u0000 \u0000 \u0000 Pneumoperitoneum in cancer patients remains a highly morbid condition with a mortality rate of approximately 30% regardless of the treatment approach. The Formalin test was utilized to create a pain model. A tissue sample was prepared from the spinal cords of rats to measure oxidative stress parameters and pro-inflammatory cytokines. Furthermore, we analyzed the expression of c-Fos protein in the spinal cords.\u0000 \u0000 \u0000 \u0000 Our findings demonstrate that GLA has a reliable pain-relieving effect in the formalin test. GLA 100 increased SOD (P<0.05), GSH (P<0.001), and CAT (P<0.05), and decreased the levels of c-fos (P<0.001), IL-1β (P<0.001), TNF-α (P<0.001), and MDA (P<0.001) in the spinal cord. This can lead to different symptoms including nausea and vomiting, with less common described reflux and regurgitation symptoms.\u0000 \u0000 \u0000 \u0000 A 38-year-old female presented with a chronic complaint of postprandial pain, discomfort, and reflux lasting for about 2 years after a previous Roux-en-y gastric bypass (RYGB) surgery. Upper endoscopy was done and raised suspicion for CCS. The patient underwent an exploratory laparoscopy which confirmed the diagnosis. Surgical resection of the afferent limb was done, and all symptoms were completely resolved at the post-operative follow-up.\u0000 \u0000 \u0000 \u0000 CCS is considered a rare described complication that can occur after RYGB gastric bypass surgery. Diagnosing this condition includes performing upper gastrointestinal (GI) studies and endoscopy, which reveal a redundant afferent limb. Laparoscopy serves as a dual-purpose tool, confirming the diagnosis of CCS and providing a definitive curative intervention. Surgical resection has a high success rate, with evidence supporting its efficacy in relieving symptoms.\u0000 \u0000 \u0000 \u0000 As the popularity of Bariatric surgeries rises, it is crucial to consistently consider CCS, despite its rarity, as a potential complication. Many studies on the effects of vitamin D on general health have been significantly increased, driven by new findings concerning the systemic and extraskeletal effects of this hormone. This study was performed to determine whether low levels of vitamin D were associated with hypertension in Syrian people.\u0000 \u0000 \u0000 \u0000 This retrospective cohort study consisted of 207 subjects, including 83 (40.1%) patients suffering from essential hypertension, and 124 (59.9%) patients with normal blood pressure. Aged>18 years, who was referred to endocrinology clinic from September 2022 to September 2023. The data were analysed by using SPSS (version 25). Logistic regression analyses was performed with adjustments for age, sex, and waist circumference.\u0000 \u0000 \u0000 \u0000 Hypertension rates were 73%, 20%, and 5% in 25-hydroxyvitamin D groups<12 ng/ mL, 12 to 20 ng/mL, and>=20 ng/mL, respectively. Odds ratios (95% confidence intervals) for hypertension adjusting for age, sex, and waist circumference were 178.6(30.5_1045.6), 5.13(0.9_26.5) for 25-hydroxyvitamin D levels<12 ng / mL, and 12 to 20 ng / mL, respectively, compared with the>=20 ng/mL group.\u0000 \u0000 \u0000 \u0000 This study shown high prevalence of low vitamin D levels (25OHVD˂20 ng/mL) among a sample of Syrian people (78.3%). The lowest 25OHVD group was associated with higher prevalence of hypertension, that refers to adverse association between vitamin D level and essential hypertension.
