This study aimed to develop a clinically interpretable machine-learning model to predict MHD using simple beam’s-eye-view (BEV)–based heart-projection metrics. A retrospective cohort of 127 patients treated with postoperative left breast or chest wall radiotherapy including supraclavicular nodal irradiation was analyzed. Heart projections in the lateral and vertical directions were measured from medial and lateral tangential fields, defined as medial tangential horizontal (MTH), lateral tangential horizontal (LTH), medial tangential vertical (MTV), and lateral tangential vertical (LTV). Outliers were removed using interquartile range criteria. A multivariable linear regression model was developed using 5-fold cross-validation to predict MHD. In addition, a logistic regression classifier was trained to categorize patients suitable for three-dimensional conformal radiotherapy (MHD ≤ 4 Gy) versus inverse planning (MHD &gt; 4 Gy). Model performance was evaluated using R², root mean squared error (RMSE), mean absolute error (MAE), Pearson correlation coefficient, accuracy, and area under the receiver operating characteristic curve (AUC). The linear regression model demonstrated strong predictive performance during cross-validation (R² = 0.69, RMSE = 0.61 Gy, MAE = 0.47 Gy), with a significant correlation between predicted and measured MHD (<em>r</em> = 0.83, <em>p</em> &lt; 0.001). Independent validation further improved performance (R² = 0.76, RMSE = 0.69 Gy, MAE = 0.56 Gy; <em>r</em> = 0.90, <em>p</em> &lt; 0.001). The logistic regression classifier achieved an overall accuracy of 88% with excellent discrimination (AUC = 0.95). One limitation of VMAT on Varian linear accelerators is the limited distance the multileaf collimators (MLCs) can travel across the x-axis of the treatment field. The researchers in this case study sought to evaluate whether a split-field technique offers improved sparing of OAR, particularly dose to the femoral heads, in postmenopausal women undergoing treatment for the endometrium and pelvic lymph nodes. In the following retrospective study, 3 patients with endometrial cancer were selected. Two plans were generated for each patient, 1 split-field and 1 full-field. The full-field plan covered the full width of the PTV in both arcs. The split-field plan was created by reducing the X1 jaw to 2.0 cm on the clockwise arc, and the X2 jaw was reduced to 2.0 cm on the counterclockwise arc. PTV coverage along with OAR doses, was evaluated for all plans. All 3 patients showed lower doses to the femoral heads in the split-field plan. Doses to the rectum, bladder, and small bowel were also lower in the split-field plan. Differences in the dose were sometimes very small between the 2 techniques. Overall, the split-field technique demonstrated improved dose reduction over the full-field technique, but more research with a larger sample size is required to confirm the split-field technique.</div></div>","PeriodicalId":49837,"journal":{"name":"Medical Dosimetry","volume":"51 3","pages":"Pages 370-374"},"PeriodicalIF":1.7,"publicationDate":"2026-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147693229","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A retrospective dosimetric analysis was conducted using 6-MV GBM radiotherapy plans from 29 patients who had undergone radiotherapy combined with adjuvant TTFields. The 10 MV plans were meticulously designed to replicate the same field geometry, optimization objectives, and optimization parameters as the 6 MV plans. Both 6 MV and 10 MV plans were compared for planning target volume (PTV) coverage (evaluated using <em>D</em><sub>98%</sub> and <em>D</em><sub>2%</sub>), organ-at-risk (OAR) sparing (assessed via <em>D</em><sub>max</sub> for brainstem, the optic nerves, and optic chiasm), and scalp sparing (measured by <em>D</em><sub>10cc</sub>, <em>D</em><sub>20cc</sub>, and <em>D</em><sub>mean</sub>), the conformity index (CI), and the gradient index (GI). Statistical comparisons were performed using paired t-tests. Results showed no significant differences in target coverage, OAR sparing, CI, and GI between the two modalities (<em>p</em> &gt; 0.05). For 6 MV photons, the Scalp <em>D</em><sub>10cc</sub>, <em>D</em><sub>20cc</sub>, and <em>D</em><sub>mean</sub> were 36.65 ± 7.39 Gy, 31.90 ± 7.32 Gy, and 20.57 ± 5.74 Gy, respectively. In comparison, these values decreased to 34.49 ± 7.53 Gy, 29.76 ± 7.49 Gy, and 19.55 ± 5.62 Gy for 10 MV photons, with all differences reaching statistical significance (<em>p</em> &lt; 0.05). The 10 MV photons exhibited a notable reduction in scalp dose while achieving equivalent target coverage and sparing of OARs. These results underscore the potential of 10 MV photons to enhance GBM radiotherapy combined with adjuvant TTFields by minimizing superficial tissue toxicity and improving compliance to subsequent TTFields.</div></div>","PeriodicalId":49837,"journal":{"name":"Medical Dosimetry","volume":"51 3","pages":"Pages 333-338"},"PeriodicalIF":1.7,"publicationDate":"2026-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147576362","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study develops a fluence-corrected PBA, enabling accurate patient-specific QA directly in solid phantoms at Heavy Ion Medical Machine (HIMM) centers. Beam modeling was done in TOPAS Monte Carlo simluation platform to match the integrated depth doses (IDD) and phase space parameters to the clinical commissioning data. Depth-dose data in PTW RW3 slab phantom and in water were simulated, from which energy- and depth-dependent fluence correction (FC) factors (FCFs) were derived. The FCFs were validated experimentally and incorporated into the PBA. 4 types of QA workflows were compared for 2 patient plans at varying equivalent depths: 1) phantom CT without FC-PBA 2) phantom CT with FC 3) virtual CT with stopping power ratio overwritten to 1 with FC, and 4) virtual CT overwritten with phantom SPR with FC. An audit found that 22 of 50 independent checks failed the ± 5% local tolerance level. One of the causes for the discrepancy was thought to be that the RadCalc calculation takes no account of the patient surface curvature. Three-dimensional printed moulds were used to create wax “domes” and “bowls” with curvatures ranging from +0.33 cm−1 to −0.13 cm−1. The dose per MU delivered at dmax, under the wax phantoms, to a calibrated NACP type chamber was determined by measurement. The fields were delivered using 6 to 18 MeV electrons from a Varian TrueBeam linac. The measured data shows that the dose varies linearly, for a given electron energy, with the curvature of the wax mould. This information was used to create a set of curvature correction factors to be applied to the 50 independent MU checks.
