The complexity of its presentation often makes timely diagnosis and intervention difficult. This case series presents two cases: a 35-year old male from rural Haryana, India, with environmental lead exposure resulting in cerebellar ataxia and microcytic anaemia, and a 30-year old battery factory worker with occupational exposure leading to motor neuropathy, nephropathy, and anaemia. These cases illustrate the diagnostic complexities associated with lead toxicity, particularly when serum lead levels do not reliably reflect the severity of exposure. The first case was linked to groundwater contamination, whereas the second case highlighted the impact of chronic workplace exposure despite apparently normal serum lead concentrations. Both cases demonstrated clinical improvement following exposure cessation and chelation therapy, emphasising the importance of early recognition and intervention. Common subcutaneous fungal infections include Sporotrichosis, Chromoblastomycosis, Mycetoma, Subcutaneous Zygomycosis, Subcutaneous Phaeohyphomycosis, Lobomycosis and Rhinosporidiosis. Even with rapid urbanization, the incidence of subcutaneous mycoses is on the rise. Rarely, lateral facial infections may also lead to CST through spread via deep venous pathways. We report a previously healthy adolescent boy who developed fever, bilateral orbital swelling, and painful ophthalmoplegia shortly after the resolution of a furuncle over the left temporal part of the forehead. Magnetic resonance imaging confirmed CST. He was treated with intravenous ceftriaxone, vancomycin, and low molecular weight heparin, resulting in clinical recovery. Because the characteristic cholinergic toxidrome involves multiple systems, the absence of a clear history of exposure and clinical suspicion may lead to a disastrous misdiagnosis. We report a previously healthy male who presented with acute dyspnoea, chest pain, and diaphoresis, whose signs were suggestive of non-ST elevation acute coronary syndrome and pneumonia. Despite standard treatment, he deteriorated, necessitating intubation. However, the characteristic odour of OP prompted atropine treatment, leading to significant clinical improvement. There is history of one episode of abnormal movements, generalized weakness, muscle aches and difficulty in walking during the episode. On systemic examination, gait ataxia was noted. Routine blood investigations were within normal limits and urine showed the presence of eggs of Dioctophyma renale. The abdominal ultrasonography revealed a small and shrunken left kidney with compensatory hypertrophy of the right kidney. Histopathological analysis of the expelled worm was identified as Dioctophyma renale. He was treated with Ivermectin for 2 weeks and his symptoms subsided.
