Our aim was to assess the availability of respiratory support systems and medications for neonates in a sample of hospitals in SSA. A survey was conducted among neonatal providers attending an annual neonatal conference in Tanzania. One response per healthcare facility was invited. A total of 47 institutions completed the survey. Continuous positive airway pressure (CPAP) was used in almost all (96.6%) tertiary hospitals and in 88.9% of frontline hospitals. Continuous oxygen saturation monitoring is available in less than 50% of healthcare facilities; the ability to blend oxygen for all infants is available in 22% of facilities. Although the availability of CPAP is high, this sample may not be representative of all hospitals. HLH is characterised by excessive inflammation and tissue destruction due to abnormal immune activation, often leading to multi-organ failure and high mortality if not promptly diagnosed and treated. Although HLH is more commonly associated with other viral infections, its occurrence with hepatitis A virus (HAV) is rare and primarily documented through case reports. By 2023, all cases reported in the country were linked to travels to an endemic region. In this report, four cases of malaria caused by <i>Plasmodium falciparum</i> are presented from the province of Kayseri but with no history of travel to any malaria-endemic region. Three cases were diagnosed by reverse transcription-polymerase chain reaction. The second case had mixed parasitemia of <i>P. falciparum</i> and <i>P. vivax</i>. Artemether (20 mg)-lumefantrin were given to all patients. After discharge, the first and fourth malaria cases were admitted with recurrent parasitaemia. They were re-treated and then recovered. The second case improved, but the third case developed organ failure and died. There is history of one episode of abnormal movements, generalized weakness, muscle aches and difficulty in walking during the episode. On systemic examination, gait ataxia was noted. Routine blood investigations were within normal limits and urine showed the presence of eggs of Dioctophyma renale. The abdominal ultrasonography revealed a small and shrunken left kidney with compensatory hypertrophy of the right kidney. Histopathological analysis of the expelled worm was identified as Dioctophyma renale. He was treated with Ivermectin for 2 weeks and his symptoms subsided. Because the characteristic cholinergic toxidrome involves multiple systems, the absence of a clear history of exposure and clinical suspicion may lead to a disastrous misdiagnosis. We report a previously healthy male who presented with acute dyspnoea, chest pain, and diaphoresis, whose signs were suggestive of non-ST elevation acute coronary syndrome and pneumonia. Despite standard treatment, he deteriorated, necessitating intubation. However, the characteristic odour of OP prompted atropine treatment, leading to significant clinical improvement. {"title":"Human immunodeficiency virus and <i>Helicobacter pylori</i> coinfection: Immune modulation and eradication failure.","authors":"Haidi Karam-Allah Ramadan, Zeinab N Ahmed, Abdel-Rahman AboDief, Zeinab R Mohamed, Hager M Hamed, Taha H El-Sherif","doi":"10.1177/00494755251339521","DOIUrl":"10.1177/00494755251339521","url":null,"abstract":"<p><p>The relationship between Human Immunodeficiency Virus (HIV) and <i>Helicobacter pylori</i> (H. pylori) is bidirectional and complex. <i>Helicobacter pylori,</i> by inducing local gastric and systemic immune responses, counteracts HIV invasion to CD4+ cells and other inflammatory cells and can reactivate HIV in latently infected immune cells. Human Immunodeficiency Virus infection, by reducing secretion of pro-inflammatory cytokines, reduces the incidence of H. pylori-induced gastric pathology. Gastric lymphoma regressed in some cases of people living with HIV (PLWH) after H. pylori eradication. Triple therapy for H. pylori could be associated with a strong immune. Treatment for both H. pylori and HIV can reduce the activation of either organism. However, the primary resistance to antibiotics such as levofloxacin, clarithromycin and metronidazole is higher among PLWH.
