{"title":"Comparison of early period sexual function parameters between surgical repair and conservative treatment after penile fracture","authors":"Hakan Hakki Taşkapu, Mehmet Giray Sönmez, Gökhan Ecer, Harun Uçmak, Arif Aydın, Mehmet Balasar","doi":"10.1016/j.androl.2021.04.006","DOIUrl":"10.1016/j.androl.2021.04.006","url":null,"abstract":"<div><h3>Introduction and objectives</h3><p>Comparison of early period sexual function parameters in patients who had surgical repair and conservative follow-up after penile fracture and the evaluation of surgical intervention time on these parameters were planned in this study.</p></div><div><h3>Materials and methods</h3><p>Total of 26 patients who were treated for penile fracture were evaluated. 19 patients had surgical repair and 7 patients had conservative treatment.</p><p>Sexual function and erectile dysfunction (ED) degree of the patients before penile fracture and in the 12th week after fracture were evaluated with 5-question International Index of Erectile Function (IIEF-5) questionnaire, Erection Hardness Score (EHS), Sexual Encounter Profile(SEP) 2 and SEP 3. Parameters showing sexual function before and after the fracture were compared.</p></div><div><h3>Results</h3><p>In both groups, a significant change was detected in IIEF-5 score, EHS, SEP-2 and SEP-3 parameters of the patients measured after penile fracture compared to the values before the fracture (all parameters, <em>p</em> <!--><<!--> <!-->0.05). No difference was detected in the parameters measured before and after the fracture among surgical repair and conservative treatment groups (all parameters <em>p</em> <!-->><!--> <!-->0.05). Mean time passing until the surgery after fracture was measured as 9.6<!--> <!-->±<!--> <!-->6.85<!--> <!-->h in 19 patients who had surgery.</p></div><div><h3>Conclusion</h3><p>A difference wasn’t detected in sexual parameters in conservative treatment and surgical repair groups in this study. While conventional methods for semen preparation have been shown to be inefficient reducing DSB values, Microfluidic Sperm Sorting (MSS) devices are promising tools to reduce this damage.</p></div><div><h3>Objective</h3><p>To study the clinical utility of an MSS device in ICSI treatments when the male partner presents increased DSB values, as compared to the use of conventional methods based on sperm motility.</p></div><div><h3>Methods</h3><p>This retrospective cohort study included 28 infertile couples undergoing ICSI treatments. Only couples where the male partner presented increased values of DSB were included. DSB values were evaluated in semen samples by the Neutral Comet assay. Couples performed a first ICSI cycle using conventional methods for semen preparation (Density Gradients and Swim-up) and a second ICSI cycle using the ZyMōt™ICSI (formerly named FertileChip®) microfluidic device. Embryology and clinical outcomes were compared between ICSI cycles.</p></div><div><h3>Results</h3><p>Semen parameters and the number of obtained and fertilized oocytes did not show differences between ICSI rounds. Few effective treatment options are available for the treatment of FSD. We conducted a prospective clinical pilot study to investigate the beneficial effects of Pilates exercise on sexual function in women with FSD.</p></div><div><h3>Methods</h3><p>Women aged between 20 and 50 years and who had regular menstrual cycles and sexual relationships and participating Pilates exercise program were asked to complete Beck Depression Inventory (BDI) and Female Sexual Function Index (FSFI) questionnaires before starting the Pilates exercise program. If the total FSFI score was less than 26.55, which is the cut-off for FSD, the subject was invited to participate in the study. Primary endpoints were changes in total and individual domain scores on the FSFI and BDI.</p></div><div><h3>Results</h3><p>A total of 36 premenopausal women were included in the study. After a 12-week Pilates program, all domains of the FSFI were significantly improved, with mean<!--> <!-->±<!--> <!-->SD total FSFI scores increasing from 12.0<!--> <!-->±<!--> <!-->4.9 to 29.3<!--> <!-->±<!--> <!-->3.4 (<em>P</em> <!--><<!--> <!-->0.0001). BDI scores were significantly decreased from 25.1<!--> <!-->±<!--> <!-->14.3 to 1.6<!--> <!-->±<!--> <!-->3.7 (<em>P</em> <!--><<!--> <!-->0.0001) after the exercise program.</p></div><div><h3>Conclusions</h3><p>This pilot study showed that Pilates exercise could improve sexual functions in women with FSD. The Double Moral Scale, the Rape Favorable Attitude Scale and the Revised Sexual Opinion Survey were used.</p></div><div><h3>Results</h3><p>People who are more erotophilic are bisexuals (x̄<!--> <!-->=<!--> <!-->99.10) and people who consider themselves left-wing in politics (x̄<!--> <!-->=<!--> <!-->96.91). The highest double standards scores are obtained by men (x̄<!--> <!-->=<!--> <!-->18.50), master's students (x̄<!--> <!-->=<!--> <!-->18.58), asexuals (x̄<!--> <!-->=<!--> <!-->20.50) and people who consider themselves centrist (x̄<!--> <!-->=<!--> <!-->18.24). In terms of pro-rape attitudes, men (x̄<!--> <!-->=<!--> <!-->37.72), heterosexuals (x̄<!--> <!-->=<!--> <!-->33.29) and people who consider themselves right-wing (x̄<!--> <!-->=<!--> <!-->37.35) stand out. To determine the modulating role of aqueous leaf extract of <em>Ocimum gratissimum</em> on cimetidine-induced gonado-toxicity, sexually mature male rats were randomized into four groups of six (<em>n</em> <!-->=<!--> <!-->6) rats each. Group A: control given 2<!--> <!-->ml distilled water. Group B received 500<!--> <!-->mg/kg body weight (bwt) of <em>OG</em> extract, Group C received 50<!--> <!-->mg/kg bwt cimetidine, and group D received 50<!--> <!-->mg/kg bwt of cimetidine<!--> <!-->+<!--> <!-->500<!--> <!-->mg/kg bwt <em>OG</em> extract once daily for 8 weeks via gastric gavage. Parameters tested include sperm parameters, testosterone (TT), luteinizing hormone (LH), follicle stimulating hormone (FSH) and prolactin, testicular alkaline phosphatase (ALP), acid phosphatase (ACP), lactate dehydrogenase (LDH), protein, cholesterol, glycogen, sexual behavioural parameters, and testicular histology.</p></div><div><h3>Results</h3><p><span><span>There were depletions in the seminiferous epithelium, decreased sperm quality, TT, LH, and FSH, testicular enzymes, protein, cholesterol, glycogen, and sexual behaviour increase in animals treated with cimetidine only compared to control. OG restored and improved sexual behaviour and </span>libido<span> as evident from increased frequencies of mount, intromission, ejaculation, and ejaculatory latency. Mount latencies, intromission, post-ejaculation, and prolactin were significantly decreased. Samples were processed with a Density Gradient Centrifuging. Spermatozoa were divided into 4 groups: control, experimental 1, 2 and 3 (E1, E2, E3) based on the dose/time schedules (cAMP 5mmol<!--> <!-->+<!--> <span>IBMX 0.2mmol/2, 4, and 6</span> <!-->h, respectively). The computer-assisted sperm analysis and chlortetracycline assays were used to measure sperm motility and capacitation.</p></div><div><h3>Results</h3><p>After incubation with a cAMP analog and IBMX, the levels of progressive motile sperms considerably improved in all experimental groups compared to the control group (E1<!--> <!-->=<!--> <!-->18.89<!--> <!-->±<!--> <!-->7.1, E2<!--> <!-->=<!--> <!-->30<!--> <!-->±<!--> <!-->9.7, E3<!--> <!-->=<!--> <!-->26.3<!--> <!-->±<!--> <!-->9.6 vs Control<!--> <!-->=<!--> <!-->10.28<!--> <!-->±<!--> <!-->6.2, <em>P</em> <!--><<!--> <!-->0.05) especially in E2 group (<em>P</em> <!--><<!--> <!-->0.05), indicating a greater effect of db cAMP (5<!--> <!-->mmol) and IBMX (0.2<!--> <!-->mmol) for 4<!--> <!-->h compared to the same doses at 2 and 6<!--> <!-->h. The results of the PE group were compared to those of the control group formed with 40 healthy hospital personnel. Venous blood samples were centrifuged and stored at −80<!--> <span>°C. The NO and ADMA values were compared between the individuals with and without PE.</span></p></div><div><h3>Results</h3><p><span>There was no statistically significant difference between the groups in terms of age, body mass index (BMI), and The International Erectile Dysfunction Index-5 (IIEF-5) questionnaire scores. The NO and ADMA values were significantly lower in the PE group than in the control group (29.76</span> <!-->±13.26<!--> <!-->μmol/L vs. 48.27<!--> <!-->±<!--> <!-->22.71<!--> <!-->μmol/L; <em>p</em> <!--><<!--> <!-->0.001 and 1.01<!--> <!-->±<!--> <!-->0.49<!--> <!-->nmol/ml vs. 1.83<!--> <!-->±<!--> <!-->1.06<!--> <!-->nmol/ml; <em>p</em> <!--><<!--> <!-->0.001, respectively). There was a significant correlation between IELT and NO levels (<em>r</em> <!-->=<!--> <!-->0.407, <em>p</em> <!-->=<!--> <!-->0.001).</p></div><div><h3>Conclusion</h3><p>Our study can contribute to the explanation of the pathophysiology of PE having unclear etiology and treatment. Sunitinib (25<!--> <span>mg/kg) was administered orally to the STD group by gavage. Normal saline (0.9% NaCl) was administered orally to the TTD and control groups as the solvent. One hour after administration of sunitinib and 0.9% NaCl, all animal groups were done torsion–detorsion. Then, all the rats were killed by high-dose anesthesia, and their testicles were removed. Biochemical and histopathological examinations were performed on the removed testicular tissues.</span></p></div><div><h3>Results</h3><p>Malondialdehyde; it was observed that the results in the STD group were close to those of the SC group and statistically significant lower compared to the TTD group (<em>p</em> <!-->=<!--> <!-->0.001). The glutathione values were statistically significantly higher in the STD group compared to the TTD group (<em>p</em> <!--><<!--> <!-->0.001). Nuclear factor kappa B values, revealing a statistically significant difference between the TTD and STD groups (<em>p</em> <!--><<!--> <!-->0.001). The TNF-α levels were measured and indicating that the results of the STD group were statistically significantly lower than those of the TTD group (<em>p</em> <!--><<!--> <!-->0.001). In addition to the stress caused by infertility, sexual life may also be negatively affected in couples who experience psychogenic wear, such as abortion and curettage. The aim of this study is to evaluate male sexual functions in infertile couples with a history of abortion and male infertility component.</p></div><div><h3>Methods</h3><p>Five hundred forty-seven male patients’ data were evaluated retrospectively. Group 1 was the included the males of the couples without abortion history and the rest with abortion history was group 2. In addition to demographic data, abortion history, libido level, presence of morning erection and intercourse frequency were evaluated. Hormone levels of all patients were evaluated. After that we created sub-group 1 and sub-group 2 from these groups, respectively, due to whether the patients fulfilled the international index of erectile function (IIEF), Beck anxiety inventory (BAI) and Beck depression inventory (BDI).</p></div><div><h3>Results</h3><p>There were not any significant differences between the groups regarding psychiatric scale scores, hormone levels, libido, intercourse frequency and IIEF scores. Only orgasmic dysfunction was significant in the males of the couples with abortion history.</p></div><div><h3>Conclusion</h3><p>Man is not affected from abortion process in which woman get involved.
