In anesthesiology, where high-risk and complex clinical events are routine, non-technical skills (NTS)-including communication, teamwork, situational awareness, and decision-making-play a critical role in preventing errors during high-acuity emergencies. Furthermore, Crisis Resource Management (CRM) provides a structured framework for enhancing team performance and coordination during medical crises by emphasizing effective leadership, task management, and the optimal allocation of resources within a multidisciplinary team. Simulation-based training is central to teaching and assessing NTS; validated tools such as the Anesthetists' Non-Technical Skills (ANTS) system allow objective evaluation of team performance and behaviors across various clinical scenarios. This modality is particularly effective because it incorporates structured debriefing, enabling practitioners to reflect on their clinical performance in a safe, risk-free environment. Evidence shows that CRM- and NTS-focused simulation training improves crisis response, adherence to protocols, and clinical outcomes. Despite widespread international adoption, CRM and NTS training in Poland remains limited because of institutional and systemic barriers. Implementation of structured CRM and NTS curricula in Poland could enhance patient safety, foster effective teamwork, and reduce medical errors, particularly in high-risk clinical settings, thus aligning Polish medical education with international standards. This narrative review synthesizes global evidence regarding the effectiveness of CRM and NTS training, highlights the benefits of simulation-based training and its impact on long-term skill decay, and identifies gaps in Polish residency programs.
","PeriodicalId":48888,"journal":{"name":"Medical Science Monitor","volume":"32 ","pages":"e952378"},"PeriodicalIF":2.5,"publicationDate":"2026-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13648104/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149117131","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The molecular mechanisms driving its progression are incompletely understood, highlighting the need to identify novel regulators of oncogenic pathways. This study focused on keratin 13 (KRT13), a cytoskeletal protein, and investigated its role in pancreatic cancer pathogenesis, particularly its regulatory effect on peroxisome proliferator-activated receptor gamma (PPARG) through the PI3K/AKT signaling axis. MATERIAL AND METHODS We integrated in vitro cellular assays with molecular biology approaches and bioinformatic analyses to systematically examine KRT13 expression and function. Pancreatic cancer cell lines were used for functional studies, including proliferation, migration, and pathway modulation experiments. Gene expression levels were assessed via quantitative PCR. Expression of KRT13 was further validated via immunohistochemistry on tissue sections from 85 patients. Public transcriptomic datasets (eg, TCGA) were interrogated to evaluate clinical correlations. Mechanistic insights were gained through pharmacological inhibition and genetic silencing of key pathway components. Further in vitro investigation of drug sensitivity was performed with patient-derived organoids (PDOs). RESULTS KRT13 expression was markedly upregulated in pancreatic tumor tissues compared to adjacent normal samples. This result was also confirmed in a set of 85 clinical samples, showing a strong positive correlation with PPARG levels. Clinically, elevated KRT13 was significantly associated with reduced overall survival (log-rank P<0.001). Importantly, analysis of The Cancer Genome Atlas (TCGA) database revealed that KRT13 expression was significantly associated with tumor TNM stage and other clinical parameters. Functional experiments demonstrated that KRT13 activates the PI3K/AKT signaling cascade, leading to subsequent upregulation of PPARG. This activation promoted pancreatic cancer cell proliferation and migration, effects that were attenuated upon PI3K/AKT pathway inhibition. Drug sensitivity assays using patient-derived organoids (PDOs) demonstrated that KRT13 significantly enhanced the sensitivity of pancreatic cancer to Navitoclax. CONCLUSIONS Our findings establish KRT13 as a critical contributor to pancreatic cancer progression, via the PI3K/AKT-PPARG regulatory axis. These results underscore the potential of KRT13 as a prognostic biomarker and a candidate therapeutic target, offering new insights into the molecular pathogenesis of this lethal disease.
","PeriodicalId":48888,"journal":{"name":"Medical Science Monitor","volume":"32 ","pages":"e952797"},"PeriodicalIF":2.5,"publicationDate":"2026-09-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13640800/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149082494","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study evaluated the relationship between anti-HBs antibody levels and systemic inflammatory markers in healthcare workers. MATERIAL AND METHODS This longitudinal observational study with a retrospective component included 583 healthcare workers who underwent routine occupational health screening between 2023 and 2025. Anti-HBs levels at 2 time points were analyzed together with C-reactive protein (CRP) level, neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, systemic immune-inflammation index, and systemic inflammation response index. Nonparametric and correlation analyses were performed. RESULTS Anti-HBs levels decreased significantly between baseline and follow-up (Hodges-Lehmann estimate: -23.00 mIU/mL, 95% CI: -27.60 to -18.93; P<0.001). No significant associations were observed between antibody levels and sex, smoking status, or the neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, systemic immune-inflammation index, and systemic inflammation response index. In contrast, CRP positivity was associated with lower anti-HBs levels at the second measurement; however, no association was observed between CRP and longitudinal change in anti-HBs levels. Higher anti-HBs levels were observed among physicians and healthcare workers in the very high occupational biological-risk group, whereas age showed a weak negative correlation with antibody levels. CONCLUSIONS CRP showed a weak cross-sectional association with lower anti-HBs levels at the second measurement, whereas no association was observed between CRP or the other inflammatory indices and longitudinal change in anti-HBs levels. Therefore, the present findings do not establish systemic inflammation as a determinant of longitudinal antibody persistence. Further longitudinal studies are required to determine whether systemic inflammatory markers have clinical utility in post-vaccination immunity monitoring.
","PeriodicalId":48888,"journal":{"name":"Medical Science Monitor","volume":"32 ","pages":"e955023"},"PeriodicalIF":2.5,"publicationDate":"2026-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13632567/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149070393","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Despite international recommendations promoting integrated, primary care-led models, the organization and division of responsibilities between primary and specialist care vary across countries. The aim of this exploratory study was to describe healthcare professionals' perceptions regarding the division of responsibilities between primary and specialist care in CRD management across selected European countries and to identify perceived system-level barriers. MATERIAL AND METHODS This cross-sectional, questionnaire-based study was conducted within the Joint Action on REspiratory Diseases (JARED) project. Data were collected between October and December 2025 from healthcare professionals involved in CRD management. A 2-stage survey design was applied. The final analytical sample included 293 respondents from 10 European countries. Descriptive statistics were used to evaluate the perceived division of responsibilities and barriers to care. RESULTS Overall, 58% of respondents considered the division of responsibilities between primary and specialist care to be clearly defined. Assessments of the optimality of the current division of responsibilities were similarly distributed, with 44% positive, 43% negative, and 13% undecided. Substantial variation between countries was observed. The most frequently reported barriers were long waiting times for specialist care (41%) and shortages of specialists (35%). Additional challenges included limited consultation time (22%) and inadequate coordination between care levels (20%). Respondents from Poland and Italy more frequently reported specialist care capacity limitations, while respondents from Finland more commonly indicated delays related to primary care access. CONCLUSIONS The findings indicate substantial heterogeneity in healthcare professionals' perceptions regarding the organization of CRD care across participating countries. Respondents most frequently identified specialist care capacity limitations and insufficient cooperation between care levels as barriers to optimal care.
","PeriodicalId":48888,"journal":{"name":"Medical Science Monitor","volume":"32 ","pages":"e954025"},"PeriodicalIF":2.5,"publicationDate":"2026-09-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149065421","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

MATERIAL AND METHODS Sixty-five patients with MCI were randomly assigned into an intervention group (a 24-week multi-modal training) and a control group (health education). Cognitive function was assessed at baseline, week 12, and week 24 using the Mini-Mental State Examination (MMSE) and the Montreal Cognitive Assessment (MoCA). A repeated-measures ANOVA was conducted across 3 time points to examine the group effects, time effects, and interactions. RESULTS Significant group × time interactions were observed for MMSE and MoCA (both P<0.001). At 24 weeks, the intervention group showed greater improvement than the control group in MMSE score, with a between-group difference of 1.88 points (P<0.001), and in MoCA score, with a between-group difference of 1.62 points (P<0.001). Exploratory MoCA subdomain analyses suggested possible domain-specific benefits in the intervention group, with nominal declines in naming, memory, and attention in controls. CONCLUSIONS This pilot randomized controlled trial provides preliminary evidence that a 24-week multi-modal program combining Baduanjin, computerized cognitive training, and reminiscence therapy was associated with improved global cognitive performance in community-dwelling older adults with MCI compared with health education alone. Disease-modifying therapies can suppress inflammatory activity in multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD), but they do not replace lost oligodendrocytes or reliably reverse established neuroaxonal injury. Stem cell-based strategies therefore pursue distinct therapeutic goals, including immune reconstitution, paracrine immunomodulation and neuroprotection, direct cell replacement, remyelination, and correction of defined genetic defects. Autologous hematopoietic stem cell transplantation (AHSCT) currently has the most mature clinical evidence, particularly for carefully selected patients with highly active relapsing MS despite high-efficacy therapy, but its benefit is mediated primarily through immune ablation and reconstitution rather than direct remyelination. Mesenchymal stromal cell (MSC) approaches remain investigational because controlled trials have not established consistent efficacy and intrathecal delivery has raised route-specific safety concerns. MSC-derived extracellular vesicles and induced pluripotent stem cell (iPSC)-derived oligodendrocyte or neural progenitor products remain preclinical or early translational approaches for acquired demyelinating disease, whereas gene-modified hematopoietic stem cells have a distinct role in selected inherited leukodystrophies. Translation across these platforms is additionally constrained by product heterogeneity, potency assessment, manufacturing reproducibility, long-term safety, regulation, scalability, access, and the need for mechanism-matched clinical endpoints. However, despite its widespread use in intensive care practice, the reliability and accuracy of this routinely recorded parameter as a measure of short-term fluid change remain uncertain. MATERIAL AND METHODS We conducted a prospective multicenter observational study comparing cumulative 24-hour fluid balance with 24-hour body weight change measured over identical time intervals in adult patients admitted to the intensive care unit (ICU). Paired measurements were collected and analyzed to evaluate the agreement between these 2 commonly used approaches for estimating short-term fluid changes. Agreement was assessed using Bland-Altman analysis, including regression-based evaluation of proportional bias. Clinically acceptable agreement was predefined as a difference within ± 1 kg between methods. RESULTS A total of 360 paired observations were analyzed. The mean bias between methods was small (0.082 kg), indicating minimal systematic difference at the population level. However, agreement at the individual level was poor, with wide 95% limits of agreement (-2.1 to +2.264 kg), exceeding the predefined threshold for clinical acceptability. Clinically significant disagreement (> 1 kg) occurred in 30.2% of observations. Proportional bias was present, with increasing discrepancy between methods across the range of measured values, suggesting that disagreement was not consistent across different levels of fluid change. CONCLUSIONS Routinely charted 24-hour fluid balance demonstrates clinically unacceptable agreement with body weight change and should not be considered an interchangeable measure of short-term fluid change in critically ill patients. We aimed to assess the effects of T-piece resuscitator combined with intubation-surfactant-extubation (INSURE) on short-term respiratory outcomes and complications in premature infants with RDS. MATERIAL AND METHODS This retrospective observational study included 100 premature infants with RDS who were treated between January 2019 and December 2023. The primary outcomes were blood gas and oxygenation indicators, including oxygenation index, partial pressure of carbon dioxide (PaCO₂), partial pressure of oxygen (PaO₂), and inhaled oxygen concentration. The infants were categorized into an INSURE group (n=50) and a combination group (n=50) based on treatment approaches. RESULTS Compared with the INSURE group, the combination group showed significantly lower oxygenation index, PaCO₂, and inhaled oxygen concentration, as well as significantly higher PaO₂ after intervention (P<0.05). In addition, the combination group had significantly increased blood oxygen saturation and shortened duration of nasal continuous positive airway pressure (nCPAP) ventilation compared with the INSURE group (P<0.05). Furthermore, the combination group exhibited significantly shorter length of hospital stay, duration of oxygen therapy, and duration of noninvasive ventilation, together with a lower incidence of complications (12% vs 30%, P<0.05). Aortic stiffness, commonly assessed by carotid-femoral pulse wave velocity (cfPWV), is a well-established marker of cardiovascular risk. Glycoprotein acetylation (GlycA) reflects cumulative systemic inflammation; however, its association with aortic stiffness in patients undergoing HD remains unclear. MATERIAL AND METHODS In this cross-sectional study, 160 patients receiving maintenance HD were enrolled. Aortic stiffness was defined as cfPWV >10 m/s. Serum GlycA levels were measured using an enzyme-linked immunosorbent assay. Multivariable logistic and linear regression analyses were performed to evaluate the association between GlycA and aortic stiffness. RESULTS A total of 52 patients (32.5%) had aortic stiffness. These patients were older and had a higher prevalence of diabetes and hypertension, along with higher blood pressure, glucose, and GlycA levels (all P<0.05). In multivariable logistic regression analysis, GlycA remained independently associated with aortic stiffness, with each 1 μmol/L increase in GlycA associated with a higher likelihood of aortic stiffness (odds ratio 1.007, 95% CI 1.004-1.010; P<0.001). In linear regression analysis, cfPWV was positively associated with log-GlycA, systolic blood pressure, and glucose levels. CONCLUSIONS Circulating GlycA levels are independently associated with cfPWV and aortic stiffness in patients undergoing HD, suggesting that GlycA may serve as an integrative marker of inflammation-related vascular alterations in this population.
","PeriodicalId":48888,"journal":{"name":"Medical Science Monitor","volume":"32 ","pages":"e953820"},"PeriodicalIF":2.5,"publicationDate":"2026-09-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13621880/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149031851","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
