This systematic review aimed to assess the impact of pulpectomy versus extraction of primary teeth on patient-centered outcomes.</div></div><div><h3>Methods</h3><div>Seven electronic databases and grey literature were searched on April 05, 2024. Two independent reviewers selected clinical trials comparing the impact of pulpectomy versus extraction of primary teeth on patient-centered outcomes. Studies not directly comparing pulpectomy versus tooth extraction were excluded. Critical data were summarized following the Synthesis Without Meta-analysis guideline. The risk of bias was assessed using the Version 2 Cochrane risk-of-bias tool for randomized clinical trials and the Risk Of Bias In Nonrandomized Studies of Interventions tool for nonrandomized clinical trials. The certainty of evidence was assessed using the GRADE approach.</div></div><div><h3>Results</h3><div>A total of 1228 studies were retrieved in the search. After the screening process, two studies were included. The outcomes evaluated were oral health-related quality of life (OHRQoL), dental anxiety, postoperative pain, and behavior. Qualitative synthesis indicated that children treated with pulpectomy exhibited improved OHRQoL and lower dental anxiety than those treated with tooth extraction. The treatment modality did not influence behavior and postoperative pain. Included studies present potential bias in specific domains. Literature screening and data extraction were performed by two reviewers according to the set criteria, and a third evaluator was involved in case of disagreements. The included studies were assessed with the Newcastle–Ottawa Scale, and the results were analyzed using Review Manager 5.4 software.</div></div><div><h3>Results</h3><div>A total of 14 studies were included. Patients with OSF exhibited significantly higher levels of malondialdehyde (serum: SMD: 7.65, 95% confidence interval [CI]: 5.14–10.17, <em>P</em> &lt; .00001, saliva: SMD: 3.07, 95% CI: 0.6–5.54, <em>P</em> &lt; .00001) and 8-hydroxy-deoxyguanosine (saliva: SMD: 4.84, 95% CI: 3.49–6.18, <em>P</em> &lt; .00001) than healthy controls. The purpose of the systematic review was to evaluate the outcomes of the ARP procedure at molar extraction sites in comparison to spontaneous healing through meta-analysis.</div></div><div><h3>Material and methods</h3><div>Studies published in PubMed, Embase, Web of Science, Cochrane Central Register of Controlled Trials, and the System for Information on Grey Literature in Europe were searched. Primary outcomes were changes in alveolar ridge width and height. Secondary outcomes included sinus pneumatization and need for additional augmentation at the time of implant placement.</div></div><div><h3>Results</h3><div>A total of 7 studies composing of 4 randomized controlled trials (RCTs), 2 prospective non-randomized clinical trials, and 1 prospective case control study were included. They involved a total of 237 molar extraction sockets from 236 participants. This review revealed that ARP resulted in statistically significant reductions for horizontal changes {mean difference (MD) = 2.21 mm, 95% confidence interval (CI) [0.02, 4.39]}, vertical mid-buccal changes (MD = 1.34 mm, 95% CI [0.85, 1.83]), and vertical mid-lingual changes (MD = 0.96 mm, 95% CI [0.54, 1.38]) when compared with spontaneous healing. The difference in ridge width changes between the two groups was not statistically significant (MD = 1.94 mm, 95% CI [-1.67, 5.55], <em>P</em> = .29) and with substantial heterogeneity (I<sup>2</sup> = 92%, <em>P</em> &lt; .001) when restricted to RCTs. ARP decreased the necessity for additional augmentation during implant placement in comparison to extraction alone (Risk ratio: 0.41; 95% CI: 0.26 to 0.65; <em>P</em> &lt; .001).</div></div><div><h3>Conclusions</h3><div>ARP is efficacious in reducing socket bone and ridge changes, and minimizing the need for additional augmentation procedures in the molar region. Peri-implantitis, marked by inflammation and bone loss, compromises implant longevity. Current diagnostic methods for peri-implantitis face challenges such as subjective interpretation and time consumption. Our deep learning-based approach aims to address these limitations by providing a more accurate and efficient solution. This study aims to develop a deep learning-based approach for segmenting dental implants and detecting peri-implantitis in orthopantomographs (OPGs), enhancing diagnostic accuracy and efficiency.</div></div><div><h3>Materials and Methods</h3><div>After applying exclusion criteria, 7696 OPGs were used in the study, which was ethically authorized by the Near East University Ethics Review Board. Using the Python-implemented U-Net architecture, the DICOM-formatted images were segmented and converted into PNG files. The classification model used a convolutional neural network (CNN) for distinguishing between healthy implants and those affected by peri-implantitis, leveraging features extracted from the segmented regions to enhance diagnostic accuracy. The model was trained for 500 epochs using the Adam optimizer, with the dataset split into training (70%), validation (15%), and test (15%) sets. Dice similarity coefficient (DSC) and accuracy were used to assess segmentation performance. Three medical professionals used precision, recall, and F1-score to assess the classification model after segmentation, which determined whether implants were showing signs of peri-implantitis.</div></div><div><h3>Results</h3><div>The segmentation model achieved a test accuracy of 0.999, Dice Similarity Coefficient (DSC) of 0.986, and Intersection over Union (IoU) of 0.974. For classification, out of 3693 implants, 638 were clinically identified as having peri-implantitis. The model correctly identified 576 of these, with 165 false positives. Understanding the relationship between COPD and dental health is crucial for optimizing patient care and well-being. This review aims to assess the existing literature on the impact of COPD on oral health and OHRQoL.</div></div><div><h3>Methods</h3><div>The review process followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. A comprehensive search was conducted in the databases, resulting in 304 records. Data extraction was done by 2 reviewers separately. Data extraction variables included study ID, sample, age, gender ratio, type of study, respiratory evaluation, OHRQoL assessment, and inferences. Newcastle Ottawa scale was employed to assess the methodological quality of the included studies.</div></div><div><h3>Results</h3><div>The final selection included 3 studies eligible for qualitative synthesis. OHIP-5 and OHIP-14 were used to assess OHRQoL These studies provided valuable insights into the association between COPD and dental health. OHRQoL was negatively impacted among COPD sufferers. In addition, COPD patients demonstrated significantly fewer teeth and higher plaque index (PI) as compared to control cohorts. The risk of bias was low in the included studies. But the literature available was scarce, which warrants further longitudinal studies with a larger sample size in this regard.</div></div><div><h3>Conclusion</h3><div>The evidence from the selected studies suggests a significant association between COPD and various dental health parameters, as well as their impact on OHRQoL. Dental health assessments should be integrated into COPD management to better understand disease severity and exacerbation risk. An interdisciplinary approach involving both respiratory and oral health professionals is crucial for comprehensive patient care. Future research should explore the underlying mechanisms and conduct longitudinal studies to establish causal relationships. {"title":"POSTOPERATIVE STABILITY OF AESTHETIC OUTCOMES FOLLOWING RESIN INFILTRATION OF HYPOMINERALISED AND DEMINERALISED ENAMEL LESIONS: A SYSTEMATIC REVIEW","authors":"Marie Véguer ,&nbsp;Elsa Garot ,&nbsp;David J Manton ,&nbsp;Olivia Kérourédan","doi":"10.1016/j.jebdp.2024.102052","DOIUrl":"10.1016/j.jebdp.2024.102052","url":null,"abstract":"<div><h3>Objectives</h3><div>As the need for efficient and minimally invasive strategies for the aesthetic management of hypomineralised and demineralised enamel lesions is critical, this systematic review aimed to assess the postoperative stability of aesthetic outcomes achieved after resin infiltration in permanent incisors, canines and premolars.</div></div><div><h3>Data/Sources</h3><div>This systematic review was conducted according to the PRISMA guidelines. The search strategy was based on PICO reporting system. Publications (up to September 2024) investigating the postoperative stability of aesthetic outcomes following resin infiltration of hypomineralised and demineralised enamel lesions were identified in a systematic search using MEDLINE/PubMed, Scopus and DOSS electronic databases. Only clinical studies were included. Additionally, science mapping analysis and quality assessment of selected full-text articles were performed using keyword co-occurrence networking on VOSviewer and the NIH Before-After Cohort Study assessment tool or Rob 2 tool, respectively.</div></div><div><h3>Study selection</h3><div>From an initial total of 664 studies, 15 were included in the final analysis, based on inclusion/exclusion criteria. A total number of 1518 teeth from 374 patients were included. All studies showed that the outcome obtained after resin infiltration of hypomineralised and demineralised lesions remained aesthetically stable over short, medium and long periods. However, this review highlighted that most of these studies: i/ exclusively focused on a single criterion, colourimetry, ii/ were restricted to short follow-up periods, and iii/ displayed a moderate risk of bias.</div></div><div><h3>Conclusions</h3><div>This systematic literature review highlights good stability of postoperative aesthetic outcomes following resin infiltration of hypomineralised and demineralised enamel lesions in permanent teeth.
