{"title":"Role adoption, anxiety, depression and loneliness in family caregivers of patients with chronic diseases.","authors":"Sonia Carreño Moreno, Mario Pacheco López, Mauricio Arias Rojas","doi":"10.1590/1518-8345.6926.4140","DOIUrl":"https://doi.org/10.1590/1518-8345.6926.4140","url":null,"abstract":"<p><strong>Objective: </strong>to describe and explore the relationship of loneliness, anxiety and depression with adoption of the caregiver role among individuals caring for people with chronic diseases in Colombia.</p><p><strong>Methods: </strong>this was an exploratory and cross-sectional study involving 960 primary caregivers of individuals with chronic diseases. We applied the Caregiver Role Adoption Scale, the University of California at Los Angeles Loneliness Scale, and the Anxiety and Depression Scale. Principal component and multiple correspondence analyses were performed for clustering.</p><p><strong>Results: </strong>among the participating caregivers, 40.8% reported experiencing depression, 59% reported anxiety, 54.6% reported moderate to severe loneliness, and 88.6% reported satisfactory adoption of the caregiver role. Caregivers who presented basic or insufficient role adoption levels tended to have higher scores for anxiety, depression and loneliness.</p><p><strong>Conclusion: </strong>adoption of the caregiver role is a mediator in the anxiety, depression and loneliness levels among caregivers. Strategies aimed at supporting caregivers should include training for the caregiver role to mitigate the negative impacts of anxiety, depression and loneliness. (1) More than half of the caregivers reported moderate or severe anxiety and loneliness. (2) Caregivers with low role adoption levels presented more anxiety, depression and loneliness. (3) Satisfactory adoption of the caregiver role reduces anxiety, depression and loneliness. {"title":"Terminology subset for the International Classification of Nursing Practice in Diabetes Mellitus.","authors":"Fernanda Azeredo Chaves, Heloisa de Carvalho Torres, Tânia Couto Machado Chianca","doi":"10.1590/1518-8345.7018.4188","DOIUrl":"https://doi.org/10.1590/1518-8345.7018.4188","url":null,"abstract":"<p><strong>Objective: </strong>to describe the process of developing a terminological subset for the International Classification of Nursing Practice in Diabetes Mellitus, based on Horta's Basic Human Needs Theory and Bandura's Social Cognitive Theory.</p><p><strong>Method: </strong>a methodological study based on the identification in the literature of 313 statements of nursing diagnoses pertinent to diabetes care, selected and validated by a consensus of nurses from different states of Brazil, specialists in diabetes, using the nominal group technique.</p><p><strong>Results: </strong>156 nursing diagnosis/result statements were selected, of which 111 (71.15%) related to psychobiological needs, 42 (26.92%) to psychosocial needs and three (1.92%) to psychospiritual needs. A total of 433 nursing interventions were developed. The diagnostic statements were validated on the basis of a consensus among the experts, with an average content validity index of 0.89: 0.87 for psychobiological needs, 0.93 for psychosocial needs, and 0.77 for psychospiritual needs.</p><p><strong>Conclusion: </strong>the study validated the terminological subset for the International Classification of Nursing Practice in Diabetes, favoring clinical reasoning, the qualification of the Nursing Process, and the improvement of self-care practices in diabetes. It has made it possible to use nursing's own language based on a globally recognized classification. (1) Provision of the guiding document for nursing practice in diabetes. (2) Methodological innovation in the development of ICNP® subsets. (3) Advances in knowledge of the basic human needs of people with diabetes. (4) The potential of standardized language for coding and monitoring data. {"title":"Educational video for teaching safe practices in the perioperative period: randomized controlled trial.","authors":"Raissa Bianca Luiz, Maria Beatriz Guimarães Raponi, Patrícia da Silva Pires, Maria Sagrario Goméz Cantarino, Maria Helena Barbosa","doi":"10.1590/1518-8345.6878.4152","DOIUrl":"10.1590/1518-8345.6878.4152","url":null,"abstract":"<p><strong>Objective: </strong>to assess the effectiveness of an educational video on hospitalized patients' knowledge of safe practices in the perioperative period.</p><p><strong>Method: </strong>randomized, double-blind controlled trial carried out in a teaching hospital in the countryside of Minas Gerais. 100 participants undergoing elective orthopaedic surgery were randomly allocated (50 participants in the experimental group and 50 participants in the control group). Patient knowledge was assessed using a questionnaire constructed by the researchers and validated by specialists, before and after the intervention (educational video) or standard guidelines were applied. Descriptive statistics were used for quantitative variables and Student's t-test for independent samples to analyze the mean difference in knowledge between the experimental and control groups (α = 0.05).</p><p><strong>Results: </strong>100 participants took part in the study, 50 participants in the experimental group and 50 participants in the control group. The experimental group showed a significantly higher gain in knowledge (t =3.72 ±1.84; p<0.001) than the control group. Cohen's d was 1.22, indicating a large magnitude of the effect.</p><p><strong>Conclusion: </strong>the educational video was effective in improving patients' knowledge and can contribute to nurses in the practice of health education, optimizing time and disseminating knowledge about safe practices in the perioperative period. Usability was assessed using the System Usability Scale. The Statistical Package for the Social Sciences software was used to analyze the data.</p><p><strong>Results: </strong>the overall average score on the System Usability Scale questionnaire was 87.3, which classifies the usability of the ROBOVID application as the best achievable. Furthermore, the application has high scores for ease of memorization, user satisfaction and ease of getting to know the system, as well as excellent scores for absence of errors and speed in carrying out the tasks set.</p><p><strong>Conclusion: </strong>the ROBOVID application complies with the principles of usability in terms of efficiency, effectiveness and satisfaction in the evaluation of end users, showing that the application meets consumer needs in terms of its functionalities, demonstrating its viability in terms of the initial objectives. (1) ROBOVID was considered better achievable on the SUS scale by the participants. (2) The ROBOVID app meets the user's needs in terms of its functions. (3) ROBOVID helps individuals to take care of themselves and prevent COVID-19. Multivariate logistic regression analyses were performed to verify the association of different types of trauma and the co-occurrence of forms of abuse and neglect with postpartum depression.</p><p><strong>Results: </strong>postpartum depression was identified in 93 women (36.8%; 95% Confidence Interval: 30.8-42.7). All forms of childhood trauma assessed (emotional abuse, emotional neglect, physical abuse, physical neglect and sexual abuse) were independently associated with postpartum depression after adjusting for confounding variables. Emotional abuse remained associated with postpartum depression when the co-occurrence of all forms of childhood trauma was analyzed.</p><p><strong>Conclusion: </strong>the results suggest an association between the different forms of childhood trauma and postpartum depression. The Integrated Development Environment Android Studio 4.0.1 tool and the IntelliJ IDEA platform were used to build the digital technology. Experts validated content and students evaluated navigability.</p><p><strong>Results: </strong>the final version of the mobile application contains 67 screens grouped into 12 sections with random access. The device is presented on the introductory screen; this is followed by content on the physical examination, neonatal screening, nutrition, oral health, the vaccination calendar, growth, development, danger signs, and accident prevention; at the end, there is a fact sheet and references. Audiovisual resources (texts, images, and videos) complement the application; experts presented a Content Validity Index (CVI) = 1.00; for nursing students all the items had a CVI = 1.00; only the item \"layout and presentation\" had a CVI = 0.95.</p><p><strong>Conclusion: </strong>the digital technology received a satisfactory evaluation from experts and students. It is innovative in child health care, with the potential to be used in the teaching-learning process of nursing students.</p><p><strong>Background: </strong>(1) The mobile application provides content for newborn care in primary care. (2) The mobile application directs the nurse's consultation through evidence of care. (3) The mobile application can be used offline, offering knowledge at any time and place. (4) Navigation does not require a specific order, which gives the user freedom. The roadmap was built using a previously drafted template based on international guidelines on good clinical simulation practices and scientific literature on suicide prevention in the virtual environment. For validation, the roadmap was validated by experts through self-application of an assessment form with answers based on \"adequate, fair, and inadequate\", with a field for suggestions. Descriptive statistics and the Content Validity Index (CVI≥0.8) were used.</p><p><strong>Results: </strong>nine experts took part in the study, the majority of whom were nurses (66.7%), female (55.6%), with an average age of 42.22 years. All the items in the roadmap met the acceptance criteria (CVI≥0.8).</p><p><strong>Conclusion: </strong>this study provides a useful roadmap for teaching suicide prevention in the virtual environment.</p><p><strong>Background: </strong>(1) Innovative study on suicide prevention, simulated teaching, and the virtual environment. (2) Script validated by experts and available in full for simulated teaching. (3) Introduction of a prototype of a fictional virtual social network for simulated practice. (4) Results indicated the appropriateness of the construction, with good agreement in the analyses. The population of the study involved 408 nurses from Northern Cyprus, which are registered as full members of the Nurse Council. The sample comprised 214 nurses, who volunteered to participate in the study. The study data was collected using a web-based online survey (Demographic form, the Coronavirus Stress Measure, The Optimism and Pessimism Questionnaire, The Brief Adjustment Scale-6, The Death Distress Scale).</p><p><strong>Results: </strong>the results indicated that perceived stress significantly and negatively predicted optimism (β = -0.21, p < 0.001) and pessimism (β = 0.38, p < 0.001). Perceived stress had significant and positive predictive effects on psychological adjustment (β = 0.31, p < 0.001) and death distress (β = 0.17, p < 0.01). Further analysis results revealed that pessimism mediates the association of stress with psychological adjustment and death distress; however, optimism only mediates the effect of stress on psychological adjustment among nurses.</p><p><strong>Conclusion: </strong>a low level of pessimism is effective in strengthening nurses' psychological adjustment skills againt perceived stress and death distress. Nurses should consider behavioral strategies to help reduce the level of pessimism during periods such as pandemics.</p><p><strong>Background: </strong>(1) High levels of perceived stress increased higher score of psychological adjustment. (2) Pessimism mediates the association of stress with adjustment and death distress. Data was collected through interviews during the months of June to September 2022. Pearson's Chi-Square or Fisher's Exact tests and the Poisson regression model were applied for statistical analysis.</p><p><strong>Results: </strong>17.2% of pregnant women adhered to the complete vaccination schedule, and adherence was associated with access to the internet/TV/radio (p-value = 0.011), routine prenatal vaccination (p-value = 0.019), safety of the efficacy of the COVID-19 vaccine and partner support (p-value = 0.020). Postpartum women without access to the internet/TV/radio, and who feel confident about the effectiveness of the vaccine, had higher prevalence rates for adhering to COVID-19 vaccination, with PRs of 2.56 and 3.25, respectively.</p><p><strong>Conclusion: </strong>there was evidence of low adherence to the vaccination schedule against COVID-19 during the gestational period, considering the number of recommended doses and the interval between them. Therefore, professionals in their clinical practice must make pregnant women aware of the importance of immunization and compliance with the vaccination schedule.</p><p><strong>Background: </strong>(1) Maternal vaccination plays a significant role in preventing and combating maternal morbidity. (2) Some factors may influence acceptance or hesitancy of the COVID-19 vaccine during pregnancy. (3) Safety regarding the effectiveness of vaccination against COVID-19 during pregnancy is a factor associated with adherence to COVID-19 vaccines. (4) Postpartum women without access to the internet/TV/radio have 2.56 times the risk of adhering to the COVID-19 vaccination schedule. {"title":"Learning demands of diabetes self-management: a qualitative study with people who use insulin.","authors":"Carla Assad Lemos, Ana Maria Rosa Freato Gonçalves, Elisabeth Meloni Vieira, Leonardo Régis Leira Pereira","doi":"10.1590/1518-8345.6963.4167","DOIUrl":"https://doi.org/10.1590/1518-8345.6963.4167","url":null,"abstract":"<p><strong>Objective: </strong>to understand the experiences with diabetes mellitus management of people who use insulin, in order to identify possible factors that may influence adherence to self-care and thus define their learning demands for diabetes self-management.</p><p><strong>Method: </strong>this is a qualitative study carried out using individual semi-structured interviews online. The interviews were recorded, transcribed and evaluated using Atlas.ti® software by means of Thematic Content Analysis, using the Health Beliefs Model as a theoretical framework.</p><p><strong>Results: </strong>11 people living with diabetes and using insulin took part in the study. Four categories were identified: understanding diabetes, how to deal with diabetes, difficulties related to insulin use and emotional adaptation.</p><p><strong>Conclusion: </strong>the perception of the severity of the disease, its complications and the benefits of adhering to treatment positively influences adherence to self-care behaviors. Although the study participants have lived with diabetes for many years, they are not exempt from difficulties related to insulin use and disease management, reinforcing the importance of continuing health education. In this sense, the findings of this study guide important educational themes to be worked on by health professionals to promote autonomy in diabetes self-management.</p><p><strong>Background: </strong>(1) Perceived severity of diabetes positively influences self-care. (2) Continued health education for people who use insulin is essential. (3) The importance of recognizing the benefits of insulin in adherence to treatment.
