The aim of this study is to compare patient satisfaction among three available AHCL systems (Medtronic Minimed780 G, Roche Diabeloop DBLG1, and Tandem t:slim X2 Control IQ) after six months of treatment and to determine if it is related to glycemic control.</span></p></div><div><h3>Methods</h3><p>The data of 75 individuals were analyzed, including 15 using the DBLG1 system, 9 using Control IQ, and 51 using MM780 G. Patient satisfaction was assessed using the Diabetes Treatment Satisfaction Questionnaire for Diabetes Mellitus (DTSQc), a validated instrument.</p></div><div><h3>Results</h3><p><span>All systems demonstrated treatment satisfaction. The DBLG-1 system scored 14 (−15–21) points, while Control IQ scored 21 (9–24) and M780 G scored 19 (11–24) (p = 0.004). Despite this, no specific paediatric HRQoL instrument for DM1 has been validated in Spanish.</span></span></p></div><div><h3>Objectives</h3><p>Multicentre, prospective descriptive study in children and adolescents with DM1 with the aim of carrying out cross-cultural adaptation to Spanish and evaluating the reliability and validity of the DISABKIDS chronic disease and diabetes-specific HRQoL questionnaires, using reverse translation.</p></div><div><h3>Material and methods</h3><p>Sociodemographic variables were compiled together with the most recent capillary glycated haemoglobin (HbA1c) value and HRQoL questionnaires were handed out to 200 Spanish children and adolescents with DM1 aged between 8 and 18 years of age under evaluation in 12 different hospitals.</p></div><div><h3>Results</h3><p>The mean score on the HRQoL questionnaire (patient version) for chronic disease was 80.32 (13.66), being significantly lower (<em>P</em><span> = .04) in patients<span> with a shorter duration of the disease (≤5 years): 78.34 (13.70) vs. 82.60 (13.36). We hypothesise that the timing of meals and sleep, the regularity of these throughout the week and a sedentary lifestyle influence the degree of obesity.</p></div><div><h3>Material and methods</h3><p>We included children and adolescents with obesity who attended a first check-up visit at the Childhood Obesity Unit between January 2018 and February 2020. The data were obtained from a questionnaire on food (36-h intake, frequency of consumption, eating times and habits) and sleep.</p></div><div><h3>Results</h3><p>The degree of obesity was influenced to a greater extent by later meal times and the distribution of calories throughout the day (less at breakfast, more at dinner) than by the total number of calories ingested. In addition, a lower consumption of vegetables was related to a higher degree of obesity. The difference between the hours of sleep at weekends and on weekdays correlated positively with a higher degree of obesity. Finally, the anthropometric data correlated negatively with the number of hours of physical activity. Almost half of the children did not exercise after school.</p></div><div><h3>Conclusion</h3><p>In the approach to childhood obesity, it is necessary to include recommendations on the regularity of meal and sleep times, as well as the distribution of calories throughout the day. The goal of care should be to assess the efficacy and safety of swallowing, to indicate personalized nutritional therapy. The development of Dysphagia Units, as a multidisciplinary team, facilitates comprehensive care for this type of patient.</p></div><div><h3>Material and methods</h3><p><span>A observational, cross-sectional, web-based survey-type study, focused on Spanish Society of Endocrinology and Nutrition department heads, was conducted in September-October 2021. The following data were analyzed: size and type of center, existence of a dysphagia unit, dysphagia screening, dietary and nutritional therapy, education and training of professionals and patients, codification, and </span>quality of life evaluation.</p></div><div><h3>Results</h3><p>65 responses (39% of the total Endocrinology and Nutrition departments). 37% of hospitals have a Dysphagia Unit and 25% are developing it. 75.4% perform screening, with MECV-V in 80.6%, and VED (61.4%) and VFS (54.4%) are performed as main complementary tests. The centers have different models of oral diet, thickeners and nutritional oral supplements adapted to dysphagia. In 40% of the centers, no information is offered on dysphagia, nor on the use of thickeners, dysphagia is coded in 81%, 52.3% have specific nursing protocols and only 8% have scales for quality-of-life evaluation.</p></div><div><h3>Conclusions</h3><p>The high prevalence and the risk of serious complications require early and multidisciplinary management at the hospital level. There is great variability in teaching programmes, although most of them dedicate 6 ECTS credits to the subject. Over two-thirds of the faculties maintain theoretical lessons with over 50 students per class. Most programmes dedicate between four and six hours to hypothalamic pituitary disorders, thyroid diseases and </span>adrenal gland disorders. However, there is great variability in the time dedicated to diabetes and nutrition. In one-third of the faculties, students are not required to do a rotation in the E&amp;N department. {"title":"The Tandem Control-IQ advanced hybrid system improves glycemic control in children under 18 years of age with type 1 diabetes and night rest in caregivers","authors":"Andrés Mingorance Delgado ,&nbsp;Fernando Lucas","doi":"10.1016/j.endien.2023.08.005","DOIUrl":"10.1016/j.endien.2023.08.005","url":null,"abstract":"<div><h3>Objective</h3><p><span>To determine the impact of switching from the predictive low glucose suspend (PLGS) system to the advanced hybrid Tandem Control-IQ system on glucometrics and glycosylated haemoglobin (HbA1c) at one year. To assess the impact on the </span>quality of life perceived by parents.</p></div><div><h3>Method</h3><p><span>Prospective study in 71 patients aged 6–18 years with type 1 diabetes<span> (DM1), in treatment with PLGS, who switched to an advanced hybrid system. Glucometric data were collected before the change, at 4 and 8 weeks, and at one year of use; </span></span>HbA1c before the change and after one year. The Diabetes Impact and Devices Satisfaction (DIDS) questionnaire was used at weeks 4 and 8.</p></div><div><h3>Results</h3><p>An increase in time in range (TIR) was observed with a median of 76% (<em>P</em> <!-->&lt;<!--> <span>.001) at 4 weeks, which was maintained after one year (+8% in the total group). Overall, 73.24% of patients achieved a TIR above 70%. The subgroup with an initial TIR of less than 56% increased it by 14.4%. After one year there was a 0.3% reduction in HbA1c. Level 1 hypoglycaemia, level 1 and level 2 hyperglycaemia, mean glucose (GM) and coefficient of variation (CV) decreased.</span></p><p>Auto mode stayed on 97% of the time and no dropouts occurred.</p><p>Caregivers had a perception of better glycaemic control and less need to monitor blood glucose variations during the night.
