This study explored the changes in self-reported physical activity level (PAL), risk factors, and estimated cardiorespiratory fitness (eCRF) among south American adults by comparing data reflecting the pre-pandemic period to data collected during the survey.</p><p><strong>Methods: </strong>We conducted a cross-sectional online survey between July 2021 through March 2022 using validated questionnaires (e.g., GPAQ and WHO-STEPS) on a convenience sample of 1,934 adults (68.7% women) from Argentina (n = 484), Brazil (n = 405), Chile (n = 279), Colombia (n = 343) and Uruguay (n = 423). We compared self-reported PAL, risk factors, and eCRF before the pandemic period and at the time of the survey (mean 842 days from the first reported case in each country). Data were presented as medians (interquartile range [IQR]) or means (95% confidence interval [CI]) for continuous variables and percentages for categorical ones. Paired data analyses were conducted using paired t-tests, Wilcoxon Signed Rank test, and McNamar's tests, as applicable.</p><p><strong>Results: </strong>We observed a decrease in PAL at work (median, 0 METs/min/week and interquartile range, [0-240] vs. 0 METs/min/week [0-30]; p = 0.032) and active transportation domains (0 METs/min/week [0-693] vs. 0 METs/min/week [0-594]; p = 0.008). In addition, the median sedentary time on weekdays increased by 60 min/day (360 [240-540] vs 420 [240-600]; p < 0.001). We also observed an increase in the proportion of participants with hypertension (12.9 vs. 16.5%), diabetes (6.8 vs. 9.8%), dyslipidemia (18.9 to 24%), depression symptoms (14.4 to 15.4%) and obesity (15.4 to 18.2%) compared with pre-pandemic levels (all p-values < 0.05). This can be accomplished through data-driven unsupervised methods that do not require clinical outcomes to be available. This study aimed to identify and compare phenotypes of COVID-19 patients and the subset of those patients who visited emergency departments using clustering techniques based on a limited set of easily accessible variables across different stages of the pandemic.</p><p><strong>Methods: </strong>We conducted a population-based retrospective study that included all reported adult COVID-19 patients in the Basque Country from March 1, 2020, to January 9, 2022. Phenotypes were identified separately for the pre-Omicron and Omicron periods in an unsupervised manner using clustering techniques based on easily obtainable clinical and sociodemographic variables. The clinical characteristics of the phenotypes were compared, and subsequently their association with the clinical outcomes was assessed.</p><p><strong>Results: </strong>Four phenotypes were identified in both the general population and the emergency department sub-group in the pre-Omicron period, whereas three phenotypes were extracted in Omicron. Within each scenario, these phenotypes varied significantly in age and comorbidity rates, leading to varying associations with COVID-19 outcomes. Despite their similarities, the emergency department phenotypes consistently experienced worse outcomes than their general population counterparts. Moreover, the population and emergency department phenotypes identified during the Omicron period resembled those from the pre-Omicron stage, suggesting stable phenotypic structures throughout the pandemic.</p><p><strong>Conclusions: </strong>This study highlights the potential of phenotype identification based on a few accessible variables for a meaningful segregation of patients. We aimed to characterise the perception of these patients about preferences and priorities concerning healthcare services in their medical appointments, relevant healthcare topics, and value attributed to the contact with their physicians.This cross-sectional study surveyed a group of adolescents living in institutional settings to enhance a patient-centred approach and, therefore, higher health promotion and better health outcomes for adolescents and young adults.A total of 571 adolescents with a mean age of 17 (55.4% females) answered the survey. Primary healthcare centres were the main location for medical surveillance. The most wanted topics were diet (83%), diseases (82%), and exercise (75%), with less emphasis surrounding topics such as free time, tobacco, and drugs. Effective self-management is key to improving health outcomes, yet gender plays a crucial role in shaping patients' experiences and approaches to managing chronic conditions. This study aimed to explore gender differences in self-management among individuals living with multiple chronic diseases.</p><p><strong>Methods: </strong>We conducted a qualitative phenomenological study using in-depth semi-structured interviews with 43 patients, purposively sampled to capture variation in age, socioeconomic background, and healthcare setting. Interviews explored experiences of self-management among both women and men, and data were analysed thematically to identify patterns across narratives. Thematic analysis was applied to the qualitative data, and findings were interpreted in alignment with the domains of the Patient Activation Measure-13 (PAM-13).</p><p><strong>Results: </strong>Six main themes emerged: personal responsibility, health literacy, self-management and decision-making, communication with healthcare professionals, adherence and treatment monitoring, prevention of complications, and lifestyle modifications. Importantly, gender differences significantly shaped patients' engagement with self-management across all thematic areas. Women generally demonstrated a more proactive approach, while men were more likely to depend on external support.</p><p><strong>Conclusions: </strong>This study highlights distinct gendered approaches to chronic illness management. Women tended to actively engage in self-care and frequently took on informal caregiving responsibilities, pointing to the need for interventions that support shared decision-making and alleviate caregiver burden. In contrast, men were generally more reliant on others and less inclined to seek help, emphasizing the importance of fostering autonomy, enhancing health literacy, and encouraging early engagement with healthcare services. {"title":"Breast-milk substitute marketing practices in four countries: an analysis using a community-based approach.","authors":"Ellie Mulpeter, Anna Blair, Jeni Stevens, Marianne White, Elisabeth Sterken, Kristin Stewart, Barbara O'Connor, Kajsa Brimdyr, Karin Cadwell","doi":"10.1186/s13690-025-01687-0","DOIUrl":"10.1186/s13690-025-01687-0","url":null,"abstract":"<p><strong>Background: </strong>This quantitative study engaged volunteer participants and a scavenger hunt application with the purpose of collecting photographic data of the breadth and scope of violations of the World Health Organization's Code of Marketing of Breast-milk Substitutes (\"the Code\") and subsequent World Health Assembly Resolutions, in four industrialized countries- the United States (\"U.S.\"), Canada, Australia and the United Kingdom (\"U.K.\"). These four countries do not have or do not adequately enforce their related polices or laws. A secondary goal was to provide evidence to inform national strategic planning efforts related to the marketing practices of manufacturers and distributors of breast-milk substitutes (BMS).</p><p><strong>Methods: </strong>Participants had to be residents of either the U.S., Australia, Canada or the U.K., and had to be 18 years old at minimum. They were recruited via social media, conference attendance and professional networks. Participants completed a demographic survey and downloaded the scavenger hunt application onto their mobile devices. The application directed participants to fulfill as many of the thirty total \"missions,\" which were provided by the research team, by uploading photographic evidence of images that they believed were examples of breaches of the Code. Data collection occurred from July 19, 2023 through July 31, 2024. The research team assessed, recategorized and calculated the numbers of participant submissions after the data collection phase. The Statistical Package for the Social Sciences software was utilized to conduct descriptive statistics, including Pearson's chi-squared and residuals.</p><p><strong>Results: </strong>In total, 738 individuals were eligible and consented to participation in the study. Of those individuals, 323 continued on to the scavenger hunt application. After evaluation by the research team to assess if participant submissions were submitted under the appropriate \"mission\" within the application, and then if their submission qualified as a true violation of the Code, 138 submitted violations were verified and included in the analysis. Key findings include the prevalence of digital marketing violations across all four countries, false, unsubstantiated marketing claims on BMS, a lack of key instructions on the preparation, handling and storage on labels of infant formula products and the marketing violations of BMS in healthcare facilities.</p><p><strong>Conclusions: </strong>Breaches of the Code are prevalent across the U.S., the U.K., Australia and Canada and expose the public to misleading marketing tactics. Risk groups are usually defined by age and tobacco use. Exposure to asbestos dust is an important occupational risk factor for lung cancer. Since 2014, the German Social Accident Insurance (DGUV) has introduced annual LCS for high-risk groups (EVA-LCS). In addition to occupational asbestos dust exposure, the population at risk is defined by age (≥ 55 years) and tobacco consumption (≥ 30 pack-years). The health services research project EVALUNG aims to evaluate the EVA-LCS using a combination of quantitative and qualitative methods.</p><p><strong>Methods: </strong>The quantitative part will be based on a secondary data analysis of routine administrative and medical data from the EVA-LCS. The results of the individual screening rounds will be analysed in a cross-sectional design. Primary endpoints are participation patterns, the rate of findings requiring further diagnostic investigation, the detection of lung cancer including tumour stage and characteristics, and the notification and recognition of asbestos-related occupational diseases. Secondary endpoints include false-positive and false-negative findings, incidence of other cancers, and all-cause and cancer-related mortality. To avoid selection bias, a complete set of anonymised data (approximately 22,200 individuals as of 12/2021) from the EVA-LCS will be transmitted for use in EVALUNG. A sub-sample will be used to perform longitudinal analyses and explore a linkage with cancer registry data. Another component is the development and piloting of quality indicators. Qualitative interviews will be conducted to analyse the perceptions, satisfaction, and potential psychological effects of EVA-LCS participants. Interviews with participating physicians will focus on their attitudes and knowledge regarding LCS. A further aim is to develop an evidence-based decision aid.</p><p><strong>Discussion: </strong>The EVALUNG concept is based on various complementary approaches, enabling a comprehensive evaluation of the EVA-LCS and the identification of optimization potentials. The quality of the data is crucial for the validity of the quantitative analyses. One way to address potential limitations is to link the data with cancer registry data. We aimed to describe scabies' trends between 2000 and 2023 in Belgium.</p><p><strong>Methods: </strong>We described the evolution of scabies during 2000-2023 in Belgium using a variety of available data sources. Using generalized linear models, we analysed yearly trends and seasonality of scabies diagnoses at general practitioners (GPs) in Flanders, occupational health services in Belgium, nationwide asylum seekers' shelters and consultations for people with precarious housing, and student medical facilities in Flanders. Additionally, we analysed national reimbursement and sales data for first-line scabies treatments in Belgium.</p><p><strong>Results: </strong>Scabies diagnoses significantly increased at all investigated levels (p-value < 0.001): primary care (15% yearly increase, 2011-2023), occupational health services (16% yearly increase, 2011-2022, temporary decrease in 2020-2021), asylum seekers' shelters (41% yearly increase, 2016-2022), consultations for people in precarious living situations (41% yearly increase, 2019-2022), and pharmaceutical sales (15% yearly increase in permethrin sales, 2012-2022). In primary care, those 15-24 years-old were most affected (13.2/1,000 compared to 4.1/1,000 for all ages in 2023), as were urban areas (6.7/1,000 compared to 2.7/1,000 in rural areas in 2022). Incidence was overall higher in males (p-value 0.016).</p><p><strong>Conclusions: </strong>Our study shows a strong increasing trend of scabies in Belgium since 2011, with higher incidences in younger age groups, urban areas and during colder months. Sometimes, these co- and self-regulatory processes are not seamless and regulatory problems (RP) can arise, which manifest themselves according to age and developmental stage of the child. Parents of children with RP often face prolonged periods of seeking specialized care and become entangled in a cycle of ineffective health-seeking behaviors. The present study aims to understand parental health-seeking behavior in response to their infant's RP, and the missed opportunities therein, by retrospectively analyzing the healthcare journey of families.</p><p><strong>Methods: </strong>Respondents were sampled in collaboration with child psychiatrists from two tertiary care infant mental health day clinics, using extreme case study sampling. Semi-structured interviews were conducted with 14 families (10 fathers and 15 mothers). The mean age of the infants at the time of admission was almost 11 months. The research interviews were qualitatively analyzed via reflexive thematic analysis in NVivo, within the design of a qualitative descriptive study.</p><p><strong>Results: </strong>Four main themes are identified. (1) 'The need for answers' highlights the fact that parental concerns about what is normal infant behavior, and about possible underlying causes, are significant triggers for seeking (professional) help. (2) 'The search for recognition and understanding' identifies the profound impact on parents of being validated by their social network, but also by healthcare providers, in order to obtain timely referrals to specialized care. (3) 'The lack of a broader approach' discusses the importance of a holistic approach, which considers not only the physical but also the socio-emotional health of the infant, along with the well-being of parents. (4) 'Lost in a healthcare maze' emphasizes how parents feel abandoned by healthcare providers during their search for help.</p><p><strong>Conclusions: </strong>The complex and fragmented nature of parents' search for help often results in delayed referrals to specialized care. However, comparative analyses between China and global regions during the 1990-2021 period remain lacking in the existing literature.Leveraging data from the Global Burden of Disease Study 2021 (GBD 2021), this study addresses this gap by providing a comparison of primary CNS cancer trends in China and global regions from 1990 to 2021. Through systematic analysis of temporal trends in age- and sex-specific burdens of primary CNS cancers in China-including incidence, prevalence, mortality, and disability-adjusted life years (DALYs)-this study establishes a novel comparative framework aligned with global disease burden metrics. Leveraging the most recent data from the GBD 2021, we elucidate disparities in disease burden and evolving trends between China and global regions. These updated findings provide critical insights for optimizing resource allocation and informing tailored interventions.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Methods: &lt;/strong&gt;Using open data from the GBD 2021 database, we analyzed characteristics of CNS cancer burden in China and globally from 1990 to 2021, including changes in incidence, prevalence, mortality, and DALYs.We employed Joinpoint regression analysis to calculate the average annual percentage change (AAPC) with 95% confidence intervals (95%CI), evaluating temporal trends in CNS cancer burden.A comparative analysis was conducted across multiple dimensions (age, sex, and time periods) to examine differences in CNS cancer burden between China and other world regions.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;From 1990 to 2021, the age-standardized incidence rate (ASIR) of CNS cancers in China increased from 4.69 to 6.12 per 100,000 population, while the global ASIR rose from 3.75 to 4.28 per 100,000.The age-standardized prevalence rate (ASPR) in China increased from 9.68 to 21.23 per 100,000, compared with a global increase from 8.66 to 12.01 per 100,000.China's age-standardized mortality rate (ASMR) declined from 4.05 to 3.63 per 100,000, whereas the global ASMR showed a marginal increase from 3.04 to 3.06 per 100,000.The age-standardized DALY rate (ASDR) decreased from 174.36 to 134.15 per 100,000 in China, with a corresponding global decline from 119.88 to 107.91 per 100,000.The average annual percentage changes (AAPCs) were 0.86%, 2.53%, -0.34%, and - 0.87% for ASIR, ASPR, ASMR, and ASDR in China, respectively, compared with global AAPCs of 0.42%, 1.06%, 0.01%, and - 0.35%.Age and sex exhibited differential impacts on CNS cancer burden.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Conclusions: &lt;/strong&gt;From 1990 to 2021, China experienced sustained increases in both ASIR and ASPR of CNS cancers, indicating a growing disease burden.The disease burden exhibited significant age and sex disparities: males showed higher ASIR, ASMR, and ASDR, whereas females had higher ASPR.Age-specific analysis revealed higher ASIR, ASPR, ASMR and ASDR among pediatri","PeriodicalId":48578,"journal":{"name":"Archives of Public Health","volume":"83 1","pages":"195"},"PeriodicalIF":3.2,"publicationDate":"2025-07-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12306086/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144734092","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":3,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
