The disease has been classified into three phases. 1: Early: with svnovial chondrometaplasia but no loose bodies; 2: Transitional: with active svynovial disease and loose bodies; 3: Late: With loose bodies but no synovial disease.Cartilaginous metaplasia from joint, tendo, or bursal synovial lining differentiates this entity from loose bodies arising from degenerative arthritis, osteochondritis dissecans. and osteochondral fractures. A 54 Year Hindu male presented to the orthopaedic OPD with complains of pain and swelling in left shoulder which was insidous in onset, gradually progressive in nature and not relieved by medications. Patient also complained of clicking noise during shoulder movements which was insidous in onset and gradually progressive in nature. Synovial chondromatosis is a synovial lesion seen generally in adults with a male predominance. It occurs most commonly in the knee, unsually also occuring at the hand, wrist, elbow and shoulder. It can lead to mechanical symptoms, decreased range of motion and effusion. Histopath shows nodules of moderately cellular hyaline cartilage. D/D can include Myositis Ossificans, Synovial Chondrosarcoma and Synoval Sarcoma. Treatment is usually observation if asymptomatic. This study aims to compare the therapeutic potential of Hyaluronic Acid (HA) and Platelet Rich Plasma (PRP) in its management.: A comparative study was conducted with patients receiving either HA or PRP injections. Outcome measures included VAS and WOMAC scores, safety profiles, patient satisfaction, and requirement for repeat injections.: PRP-treated patients exhibited superior improvement in VAS and WOMAC scores by the 12-month mark. Adverse reactions were minimal and comparable between groups. Notably, 92% of PRP recipients expressed a willingness for repeat treatment. This study aimed to compare the efficacy of suprascapular nerve block (SSNB) and intraarticular steroids, both adjunctive to physical therapy, in managing periarthritis shoulder. : We conducted a comparative study involving 50 patients diagnosed with frozen shoulder. Patients were segregated into two groups: one received SSNB (n=25), and the other received intraarticular steroids, followed by physical therapy in both groups.: Patients undergoing SSNB showed a significant reduction in SPADI scores over a 12-week follow-up period, with a mean reduction of 64%. This group also recorded patient satisfaction scores of 8.5 ± 1.2 on a VAS (0-10) and a quality of life score of 85 ± 10 on the SF-36 scale. In contrast, the intraarticular steroids group, while also showing improvement, lagged behind the SSNB group in these metrics.: SSNB, when combined with physical therapy, emerges as a promising therapeutic strategy for periarthritis shoulder, demonstrating superior efficacy compared to intraarticular steroids in terms of pain reduction, function enhancement, and patient satisfaction. Symptoms include pain, stiffness, swelling, recurrent atraumatic hemarthrosis and limitation in range of motion. Our case study presents an asymptomatic PVNS at the popliteal fossa which was purely an incidental finding in a 32-year-old male with a history of trauma while playing football whose MRI was suggestive of Anterior cruciate ligament (ACL), Medial meniscus tear & a focal nodular hypertrophy at posterior aspect of knee. The patient had no significant past history of trauma, swelling, hemarthrosis or similar pain. ACL Reconstruction, Medial Meniscus balancing and marginal excision and biopsy sent for the same. Biopsy suggestive of PVNS (Localised). The mean days of plaster duration in accelerated casting group was 19.25 days as compared to 45.25 days in standard casting group (statistically significant, p<0.001). Also, Pirani score at the end of last follow up was comparable in both the groups. Tenotomy rate was slightly higher in accelerated casting group (88%) as compared to standard group (80%) which may be attributed to higher initial Pirani score in former (5.5) as compared to later (5.0). Clubfoot is socially stigmatized in underdeveloped nations; the early and optimistic outcome of the accelerated Ponseti casting procedure has a profound effect on parents as well as the orthopaedic surgeon treating them. The majority of quadriceps muscle belly injuries can be successfully treated conservatively and for less active, older populations Non-operative management is reasonable. A case report of a 19-year-old male Kabbaddi player who sustained an injury to his rectus femoris muscle belly while playing 5 months back. This young patient did not recover the functional outcome required to get back to running and participating in kabaddi despite 5 months of physiotherapy and non-operative management and had complaints of Pain over midthigh and weakness in quadriceps during hip flexion and knee extension. This leads to Hindrances in Jumping, running, and kicking movements. Tenderness over the midthigh and palpable defect with Pain upon resisted hip flexion or extension was seen.3/5 quadriceps power was noted. Ultrasound and MRI were done to confirm the clinical diagnosis. Operative treatment using the semitendinosus allograft with bio fiber reconstruction system to augment rectus femoris repair allowed immediate full passive flexion of the knee and an early graduated physiotherapy program leading to a return to running and his previous level of sport without any restrictions. These patients pose unique challenges for anesthesiologists due to systemic involvement, altered anatomy, and chronic pain management needs. This review explores optimal anesthetic strategies to improve surgical outcomes and enhance patient safety in this vulnerable population. Preoperative assessment is crucial for identifying systemic manifestations of rheumatic diseases, including cardiovascular, respiratory, and renal involvement, which can significantly impact anesthetic management. A thorough evaluation helps tailor the anesthetic plan to individual patient needs, ensuring better perioperative care.Regional anesthesia, particularly neuraxial blocks and peripheral nerve blocks, offers significant benefits over general anesthesia in this population. These techniques can reduce postoperative pain, enhance recovery times, and decrease opioid consumption. However, the presence of spinal deformities and anticoagulation therapy in these patients requires careful consideration and meticulous planning. Multimodal analgesia, combining regional techniques with systemic medications, is recommended to manage chronic pain and minimize opioid use postoperatively. Treatment options range from conservative methods to various surgical interventions, each with its pros and cons. This study specifically compares two commonly employed surgical techniques: Open reduction & internal fixation with PHILOS plate and percutaneous k-wire fixation with/without external clamps. This study aims to address the effectiveness of each technique by evaluating functional outcomes using validated scoring systems like the Constant-Murley Score. To assess the functional outcomes of two surgical treatment methods (K-wire application and PHILOS plate fixation) for proximal humerus fractures. : A randomized prospective study was conducted on patients coming to the Orthopaedics outpatient department or casualty of tertiary care hospital for a duration of 1 year from Dec 2022 to Nov 2023. A total of 18 patients were included in the study. The mean Constant- Murley Score (CMS) was 81 +/- 6.13 for the PHILOS plating group and 76.88 +/- 7.37 for the K-wire treated group with a p-value of 0.08. The results for CMS were excellent in 22.2% of patients and good in 66.7% of patients in PHILOS treated group and excellent for 11.11%, good for 66.67% and satisfactory for 22.2% in the K-wire treated group. Both groups achieved satisfactory functional outcomes with CM Score at the final follow-up.
