In this article we present 3 cases of simultaneous occurrence of two thyroid neoplasm in the same patient. A 49 year old lady with swelling in right lobe of thyroid, diagnosed as follicular neoplasm with hurthle cell morphology (Bethesda category IV) on Fine needle aspiration cytology (FNAC). Histopathological examination (HPE) showed Hurthle cell carcinoma, right lobe and 3 mm foci of NIFTP, left lobe. Another 45 year old lady with bilateral thyroid swelling diagnosed as hurthe cell neoplasm (Bethesda category IV), left lobe and suspicious of papillary thyroid carcinoma, right lobe on FNAC. HPE showed papillary thyroid carcinoma, classical variant in the right lobe with a focus of papillary carcinoma in the isthmus and hurthle cell adenoma in left thyroid. A 27 year old man with thyroid swelling revealed medullary carcinoma in FNAC. Final histopathological diagnosis was medullary carcinoma, right lobe of thyroid and isthmus, papillary carcinoma (micropapillary variant) in pyramidal lobe along with metastatic medullary carcinoma in right level II lymph nodes and metastatic medullary and papillary carcinoma in central compartment nodes. Collision tumors are a diagnostic as well as therapeutic challenge due to the dual pathology. Metastasis can occur in bone, lymph nodes, the lung, oral cavity, pharynx, skin, liver, kidney, and central nervous system. Meanwhile, effusion cytology is an easily available, inexpensive diagnostic tool in our setting. Cytomorphological features provide crucial insights into the nature of the disease, which along with immunocytochemistry may help in solidifying the diagnosis. Here we present a rare case of a 17-year-old patient, who initially presented with pleural effusion that turned out to be lymphoma on cytological examination. Cell blocks were prepared and immunocytochemistry was performed for CD5 and CD23. It is often misdiagnosed due to the general unawareness with this entity and also because of its clinical presentation similar to other common diseases such as perianal fistulas or abscesses.We take this opportunity to report a case of tailgut cyst in a 33-year-old Indian male. The patient had presented with painful swelling in sacral region, which had been gradually increasing in size. The case was diagnosed by fine needle aspiration cytology (FNAC) as adenocarcinoma arising in tailgut cyst and later confirmed by histopathology.Tailgut cysts are congenital cystic hamartomatous lesions, which occur very rarely. They are generally benign, but malignant change or transformation in it has also been reported, which is even rarer in occurrence. It is also known as necrotizing lymphadenitis. The patients usually present with fever and lymphadenopathy. Here is a case of Kikuchi-Fujimoto disease presented with fever and cervical lymphadenopathy. Radiological imaging revealed matted lymph nodes. Fine needle aspiration of the lymph node revealed granulomatous inflammation. Acid Fast stain was negative and further investigations ruled out tuberculosis. A histopathological biopsy of the lymph node confirmed the diagnosis of Kikuchi-Fujimoto disease. The patient was treated with medications and the patient's condition improved in follow-up. Tuberculosis must be ruled out in such conditions as the symptoms and radiological and FNAC findings mimicking tuberculosis. The present 2 year study was carried out at a tertiary care hospital to find the concordance between cytological and histopathological grading in invasive breast carcinoma. It included all the 100 cases of invasive breast carcinoma diagnosed on histopathology during this period with available cytology reports. Concordance of Robinson's cytological grading system was calculated in each grade with respect to histological grading. All cases evaluated for presence of metastasis to axillary lymph nodes. We found Concordance rate between cytological and histological grade as 76%. The coefficient of correlation between cytological grade and histological grade was 0.83 and P value was <0.001 which indicated a strong correlation and significant association between the cytological and histological grade. High degree of concordance is seen between cytological and histopathological grading system. Preoperative grading using FNAC helps in determining neo adjuvant chemotherapy as well as prognosis. It is caused by gram-positive, non-acid fast, filamentous, anaerobic to microaerophilic bacilli. Actinomycosis on the root of the nose is a rare entity. We report a case of 38 years old male who presented with swelling over the root of the nose since birth. Mucoid material was aspirated. Microscopic examination reveals a filamentous structured colony and crystal against the granular proteinaceous background. Features suggest benign cystic lesion and secondary actinomycosis infection, negative with an acid-fast bacilli stain consistent with Actinomyces species. As stated in most of the literature, mainly males are affected by this condition. Very little information is available on the use of fine-needle aspiration cytology (FNAC) to diagnose these lesions. We are reporting a case of an actinomycotic lesion that was reliably diagnosed with FNAC.
