There is a wide spectrum of lesions ranging from benign entities like neurofibroma, Schwannoma, perineurioma to malignant entities like malignant peripheral nerve sheath tumours (MPNST), malignant granular cell tumour and malignant perineurioma. Since there is an overlap with other spindle and round cell tumours, IHC and molecular studies are needed to provide a clear distinction between different tumour types. It was a 5 years observational study (3 years retrospective and 2 years prospective) conducted in the Department of Pathology, JNMCH, AMU from 2015-2020. . History and relevant clinical findings were retrieved from the archives of both the Histopathology and Cytopathology Lab. Imprints as well as FNA smears were included from Cytopathology Lab and biopsies and resection specimen from Histopathology Lab. Immunohistochemistry was applied wherever necessary. A total of 82 cases were diagnosed over a 5 year period out of which 70 were benign and 12 malignant. Male preponderance was seen. Benign tumours comprised mostly of Neurofibroma and Schwannoma and there were 12 cases of MPNST. Correlation was established between Cytopathological and Histopathological findings and IHC, mostly S100 proved useful in differentiating it from other malignant spindle cell tumours. The differential diagnoses of these tumours has always posed a problem given their monotonous appearance and clinical overlap. It may arise de-novo or may be preceded by oral potentially malignant disorders(OPMD). During malignant transformation certain molecular changes are evident at cellular level even in the absence of phenotypic changes in the tissue. With the recent advances in molecular understanding of cancers, immune checkpoint inhibitors have been recognized as perpetrators or supporters in the pathophysiology of various cancers. One of the main immunosuppressive pathways is the programmed death 1 (PD-1)/programmed death-ligand 1 (PD-L1) in which there is an interaction between T-cell PD-1 receptor and PD-L1 on cancer cells. Upregulation of PD-L1 is associated with disease progression. To assess and compare the PD-L1 expression in OPMD and OSCC. 64 Paraffin embedded tissue sections of histopathologically diagnosed cases of 32 OSCC and 32 OPMD were immunohistochemically stained with PD-L1 & its membranous expression was evaluated. Descriptive statistical analysis was applied. Thus the study showed 100 % PD-L1 positive expression in OSCC and PD-L1 expression increased with increase in histopathological. In case of OPMD 93.74 % showed positive PD-L1 expression. We found PDL1 expression is significantly higher in OSCC (Mean= 4.59 ± 1.965) compared to OPMD (Mean= 2.03 ± 1.204). The present study was undertaken to categorise the Breast Fine Needle Aspiration Biopsy (FNAB) samples according to the IAC Yokohama system of reporting and to assess the Risk of malignancy (ROM) for each category as well as to elucidate other quality indicators of the Breast FNAB. : Among the total 102 cases in which FNAC were performed, 9 were insufficient/ inadequate (8.80%), 11 were benign (10.80%), 5 were atypical (4.95%), 3 were suspicious (2.95%) and the rest 74 FNACs were malignant (72.5%). ROM of insufficient/inadequate, benign, atypical, suspicious and malignant ware 37.5%, 0%, 50%, 100% and 100% respectively. Absolute sensitivity (only category V) was 90.1 and complete sensitivity (including category III to V) was 96.3%. Specificity of IAC Yokohama system is 100% when considering category 5 as positive and is 88.9% when considering category 3 to 5 as positive. : The IAC Yokohama Reporting System for breast cytology has high sensitivity, specificity. The ROM in our study done in Indian population ware comparable to the proposed IAC Yokohama system which suggests the IAC Yokohama system has high reproducibility and can be applied in Indian population. Recent studies have shown that an accumulation of various genetic abnormalities results in progressive morphologic changes in bronchial epithelium. Such changes are part of the continuum from precursor lesions to invasive lung cancers. Improvement in diagnostic modalities has made the detection of such preneoplastic lesions possible and resection at this stage confer excellent prognosis. The World Health Organization (WHO) has recognized atypical adenomatous hyperplasia/ adenocarcinoma in situ, squamous dysplasia/ squamous cell carcinoma in situ, and diffuse idiopathic pulmonary neuroendocrine cell hyperplasia as the precursor lesions of adenocarcinoma, squamous cell carcinoma, and carcinoid tumors respectively. CLDN-1 expression is regulated oncogenic Wnt/B-catenin transduction pathway. They recruit matrix metalloproteinases (MMPs) on the cell surface to achieve elevated focal concentrations and eventual activations of proMMP2. These collagenases are responsible for the breakdown of extracellular matrix proteins and thus facilitate invasion and spread of malignant cells. Reduced cell-cell adhesion is associated with loss of contact inhibition of proliferation. This allows escape from growth control signals and triggers carcinogenesis. Significant correlation was observed between histopathological grade of the tumor with the localization and immunostaining intensity of CLDN-1. Determination of localization of CLDN-1 for a particular patient may be important to decide site (cytoplasmic or nuclear) for targeting CLDN-1.
