{"title":"Dental care and services of children and young people with cerebral palsy in Australia: A comprehensive survey of oral health-related quality of life.","authors":"Karen Lansdown, Kim Bulkeley, Margaret McGrath, Michelle Irving, Claudia Zagreanu, Hayley Smithers-Sheedy","doi":"10.1111/scd.13098","DOIUrl":"10.1111/scd.13098","url":null,"abstract":"<p><strong>Aims: </strong>To investigate caregiver-reported dental care experiences and oral health-related quality of life (OHRQoL) of children and young people with cerebral palsy (CP).</p><p><strong>Methods: </strong>Between May and August 2023, caregivers of children and young people from three Australian states were invited to complete questionnaires, including the Child Oral Health Impact Profile (COHIP-SF 19).</p><p><strong>Results: </strong>Sixty-eight caregivers participated in the survey. Most children and young people had spastic CP (69.1%) with unilateral spasticity most common (51.1%). The COHIP-SF 19 average score was 51.9% ± 12.4, indicating moderate OHRQoL. Gender, communication, oral health daily routine, participation in dental exams and treatment, special arrangements needed to attend the practice, and urgent dental care due to pain or other problem(s) all significantly impacted OHRQoL (p < 0.05).</p><p><strong>Conclusion: </strong>OHRQoL of children and young people with CP is moderately impacted, as indicated by the COHIP-SF 19 scores. {"title":"Mother's Sense of Coherence and Oral Health of Children and Adolescents With Cerebral Palsy-Matched Cross-Sectional Study.","authors":"Vanessa Lira Siqueira, Valéria Bordallo Pacheco, Laura Costa Gonçalves, Natália Cristina Ruy Carneiro, Ana Cristina Borges-Oliveira, Maria Teresa Botti Rodrigues Dos Santos","doi":"10.1111/scd.70010","DOIUrl":"https://doi.org/10.1111/scd.70010","url":null,"abstract":"<p><strong>Background: </strong>The sense of coherence (SOC) is important for the well-being, especially mothers of children and adolescents with cerebral palsy (CP).</p><p><strong>Objective: </strong>This study aimed to investigate the relationship between Mother's SOC and oral health status in children/adolescents with and without CP.</p><p><strong>Materials and method: </strong>A paired cross-sectional study was conducted with 102 children/adolescents with CP, 102 without CP, and their respective mothers. Participants were aged between 3 and 17 years, matched by sex and age. Antonovsky's SOC questionnaire (SOC-13) was answered by mothers from both groups, characteristics and the oral status of the children were investigated. Clinical type of CP, Gross Motor Function (GMFCS), oral hygiene quality (OHI-S), and dental caries experience (DMFT/dmft) were evaluated.</p><p><strong>Results: </strong>The CP condition of the children was significantly associated with Mother's SOC (P < 0.001). Mothers of children with CP had lower SOC scores (mean: 27.6 [±3.0]) than mothers of children without CP (mean: 30.2 [±7.7]). In the CP group, level IV, V of Gross Motor function was associated with lower mother's SOC scores (p = 0.001). In both groups, dental caries experience was associated with lower mother's SOC scores, CP group (p < 0.001), without CP group (p = 0.002). Regarding the individual characteristics, CP group presented with lower-quality of oral hygiene (p < 0.001) and high prevalence of dental caries (p = 0.001). Regarding marital status, mothers of CP group were majority single, separated, or divorced (p < 0.001), and presented more difficulties in finding a dentist for their child (p < 0.001).</p><p><strong>Conclusion: </strong>Mother's SOC was statistically significantly associated with the presence of dental caries in children/adolescents with and without CP. {"title":"Ageism combating strategies in oral healthcare: A systematic review.","authors":"Mariana Sarmet Smiderle Mendes, Camila Lopes Ferreira, Maria Aparecida Neves Jardini, Christopher Andrew Childs, Leonardo Marchini","doi":"10.1111/scd.13094","DOIUrl":"10.1111/scd.13094","url":null,"abstract":"<p><strong>Purpose: </strong>The aim of this study is to investigate the existing evidence regarding the effectiveness of strategies aimed at combating ageism against older adults among dental students.</p><p><strong>Methods: </strong>A systematic review was performed following preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines to identify articles from seven electronic databases: MEDLINE via the PubMed interface, Embase, Age-Line, Scopus, Web of Science, CINAHL, and Cochrane.</p><p><strong>Results: </strong>Among twenty-four references reporting on ageism among oral health care providers and/or students, only two reported on the outcomes of strategies for combating ageism at dental schools, with a 12-year gap between them. Both studies showed that non-geriatric-specific clinical activities during dental school might reduce ageism among students. This study evaluated vitronectin and fetuin-A expression in saliva samples of periodontitis (P) patients with and without coronary artery disease (CAD) after scaling and root planing (SRP).</p><p><strong>Methods: </strong>Sixty patients were divided into three groups: PH + SH (periodontally and systemically healthy), P (stage II/III grade B periodontitis), and P + CAD (periodontitis with CAD). Demographic, periodontal, and cardiac parameters were recorded. Unstimulated saliva samples were collected at baseline (day 0) and after SRP. On day 90, periodontal parameters and vitronectin/fetuin-A expression were reassessed.</p><p><strong>Results: </strong>P + CAD patients had higher age, weight, BMI, and lower income (p < .001, .025, .002, < .001, respectively), along with elevated plaque index, bleeding on probing, probing pocket depth, and reduced clinical attachment levels (p < .001). Intervention with an oral health education program was introduced to the orphans. The data was collected at baseline using a questionnaire to collect data about the knowledge, practice, and examination of DMFT (decayed, missing, and filled permanent teeth), dft (decayed, filled deciduous teeth), and OHI-S (oral hygiene index-simplified) was done. The knowledge part of the questionnaire was distributed immediately after the intervention. After 6 months, the knowledge, practice, and OHI-S index were determined, the data were then analyzed using Mann-Whitney A U non-parametric test, Wilcoxon matched pair signed rank test, and Fridman's 2-way ANOVA test in SPSS version 25 software.</p><p><strong>Results: </strong>89% of the children had dental caries, with mean DMFT 2.23 ± 2.1 among males and 3.3 ± 2.3 among females, dft among males was 3.1 ± 2.5 and 3.3 ± 2.1 among females. The mean score of OHI-S significantly decreased after health education intervention from 4.79 ± 1.4 to 0.62 ± 1.0. Mean scores of total knowledge and practice significantly increased after 6 months from 3.4 ± 1.3 to 7.1 ± 1.1 and 1.3 ± 0.9 to 4.9 ± 0.3, respectively.</p><p><strong>Conclusions: </strong>The orphans had moderate DMF and dft, fair knowledge about their oral cavity, poor OHI-S, and practices at baseline. To date, under 200 cases of TTS are documented in medical literature, with only sparse studies reporting on oral complications secondary to this condition.</p><p><strong>Case report: </strong>The following report presents a well-documented case of oral complications associated with TTS in an 83-year-old Caucasian female presenting with right-sided lesions on her nasal ala, scalp, buccal mucosa, and tongue, secondary to self-inflicted injury following their second microvascular decompression for surgical management of trigeminal neuralgia.</p><p><strong>Discussion/conclusion: </strong>The paucity of literature on oral complications associated with TTS supports the need for understanding guiding management modalities of such lesions for clinical practice. Despite difficulties, more evidence exploring difficulties among this cohort in India is needed. This study aims to conduct a scoping review of articles published in India exploring access to dental care among individuals with IDD.</p><p><strong>Method: </strong>We followed the Arksey O'Malley framework for scoping reviews and the PRISMA guidelines for reporting studies. Findings were analyzed based on Levesque's framework for access.</p><p><strong>Results: </strong>Seventeen articles were collated from five databases. The perceived need for dental care was low, and dental visits were mainly need-based and infrequent. Knowledge regarding dental care did not translate to dental visits. Patient-related barriers were cost, transportation difficulties, fear, lack of dentist skills, patient behavior, and cooperation. From the providers' perspective, most dentists were willing to treat but noted infrastructural and knowledge-based limitations.</p><p><strong>Conclusion: </strong>Access to dental care was limited due to infrastructure constraints and a lack of dentist training. Caregivers and individuals with IDD did not prioritize dental health, faced communication barriers, and were afraid of dental procedures. It is important to improve preventive dental care and raise awareness among caregivers of individuals with IDD in India. This study examined reproducibility of tongue pressure measurement (TPM) in DS patients.</p><p><strong>Methods: </strong>Thirty patients with DS (13 females) who were able to complete three measurements underwent a tongue pressure test (TPT). Reproducibility was evaluated using intraindividual standard deviation (SD) and coefficient of variation (CV). Patients were divided into two groups based on developmental age (DA): DA ≥ 4 years (over-4 DA) and DA < 4 years (under-4 DA).</p><p><strong>Results: </strong>Thirty DS patients with a mean age of 33.0 ± 11.7 years and mean DA of 3.8 ± 1.2 years were analyzed. Median tongue pressure was 17.6 kPa, SD was 2.0 kPa, and CV was 11.9. Eleven patients were in the under-4 DA group, and 19 were in the over-4 DA group.
