It uses a radial emitting laser probe and had an initial rate of success of 81.8 percent in 11 patients with a median follow-up of 7.4 months. Aim of the study: To analyze the Fistula Laser Closure (FiLaC) technique's efficacy in treating high trans-sphincteric fistulas in ano, look at the procedure's effects on anal continence, and predictors of success. Patients and methods: This prospective cohort research was conducted between April 2021 and November 2021 on 20 patients with high transsphincteric fistula in ano at the Department of Surgery at Al-Azhar University Hospitals, Cairo, Egypt. Patients may now be kept on dialysis for decades because to the improvement of vascular access methods and equipment. Aim of study: To evaluate the most possible causes behind repeated arteriovenous (AV) access failure in ESRD patients. Patients and methods: This retrospective, non-randomized study was carried out at the vascular surgery department of Al-Azhar University hospitals(Al-Hussein and Sayed Glaal Hospitals). Results: Access thrombosis comes first as a cause of access failure where it is responsible for 41% of all failed accesses. Among the cases of access thrombosis 25.5% were associated with access stenosis, 10.5% with hypotension, and 2.5% with external compression. The commonest cause of access stenosis was hypotension (14%) followed by hypotension together with central venous stenosis (4.5%) and then central venous stenosis alone (2.5%). Failure of maturation comes next to access thrombosis and it is responsible for 35% of all access failures. Among the cases of failure of maturation 16% were due to small vein diameter (less than 2.5 mm), 9% associated with hypotension, and 7% with hypotension together with central venous stenosis. However, it requires more technical skill & takes longer to complete. Method: The purpose of this trial was focus upon the use of round-block technique in conservative oncoplastic breast cancer surgery as a form of treatment option regarding oncological safety, efficacy, final cosmetic results on the quality of life & the contentment of the individual receiving treatment, advantages and disadvantages. This was an observational cross sectional study that was conducted on (40) patients with early breast cancer treated by conservative breast surgery (Round –Block Technique). The duration of the study was two years. Result: Among our studied patients lack of symmetry occurred in 22.5% of patients followed by unacceptable scar and pain in the treated breast in 17.5% then seroma and areola depigmentation were developed in 15% for each followed by feeling of numbness or tingling in 12.5% then restricted arm movement and local infection in 10% then hematoma and delayed wound healing in 7.5%, hypertrophic scar in 5% of cases. Conclusion: The round block approach is viable as well as oncologically safe for the initial stages of carcinoma of the breast. PATIENTS AND METHODS: The retrospective cohort study which was performed at tertiary referral center in Cairo. Children underwent Dx/HA injection for treatment of the primary VUR were reviewed. Only those he had full medical record dating 5 years or earlier included into the study. Results: Sixty patients, 25 boys and 35 girls in total 82 ureters had refluxed underwent DX/HA injections. The patients' average age was 5.70 years. Eight (9.7%) of the 82 ureters that were treated failed all three injections, leaving 74 (90.3%) of them completely cured. Conclusion: This acceptable improvement sustainability of the DX/HA sub-uretral injection in children with 1ry VUR highlights the benefits of this procedures. Some studies have shown that interleukin-6 (IL-6) can indicate respiratory failure in COVID-19 patients. Aim of the work: To determine if the laboratory marker IL-6 has the ability to predict respiratory worsening in COVID-19 patients three days after hospital admission. Patients and Methods: The last 100 consecutive COVID-19-infected individuals as determined by a positive reverse transcription real-time polymerase chain reaction (rt RT-PCR) test of their respiratory tract swabs were included in this retrospective study. Patients were separated into two groups based on the highest FiO2 they needed in the first three days after admission; group with low FiO2 and group with high FiO2. Results: In comparison to the low FiO2 group, the high FiO2 group had considerably greater serum IL-6 levels. In comparison to the low FiO2 group, the non-survivors percentage was much higher in the high FiO2 group. Additionally, in both low and high FiO2 groups, serum IL-6 was considerably lower in survivors compared to non-survivors. According to ROC curve, IL-6 displayed an AUC of 0.82 with a strong significance at cutoff value of 41.2 pg/mL. Sensitivity, specificity, PPV and NPV were 88.2%, 79.8%, 52.2% and 96.4% respectively. Conclusion: In COVID-19 patients who have been admitted to the hospital, serum IL-6 is a potent indicator of early respiratory failure.
