{"title":"Haematoma Block for Distal Radius Fracture Pain Management","authors":"Varun Thusoo, Shilaga Dhar","doi":"10.36347/sjams.2024.v12i03.006","DOIUrl":"https://doi.org/10.36347/sjams.2024.v12i03.006","url":null,"abstract":"Introduction: HB (Hematoma Block) is a commonly utilized technique all over the world to relieve discomfort during the management of DRF (Distal Radius Fractures). In this approach, 10ml to 15ml of 2 percent lignocaine, a topical anesthetic is utilized to reduce discomfort during the fracture reduction process. Materials & Methods: In this study, we recruited fifty individuals between January and December of 2023. Every patient had a distal radius extra-articular fracture that required percutaneous pinning or plaster casting. HB using 12 to 15 ml of 2% lignocaine was administered to the patients for hematomas at the fracture site. Using the Visual Analogue Scale method, the patient's pain distribution and severity were measured before, during, and after the decrease. Findings: Before the HB, the average VAS (Visual Analogue Scale) score was 9.5. Less than two hours after the surgery and during reduction, the average VAS score dropped to 2.5 (range between 0.5 to 3.5). A scale of 0 to 10 cm is used to define the VAS scoring system. Chronic ACL tears can compromise knee stability, making the meniscus a crucial stabilizer. Understanding the prevalence and patterns of meniscus injuries associated with chronic ACL tears is essential for effective management. Objective: This study aimed to assess the arthroscopic evaluation of association of meniscus injuries in patients with anterior cruciate ligament injuries. Method: A retrospective analysis was conducted on thirty-three patients diagnosed with ACL injuries at MuMCH between January 2021 and December 2023. Patients aged 18-50 with ACL injuries, with or without meniscus damage, were included. Clinical evaluations, knee MRI scans, and diagnostic knee arthroscopies were performed. The study evaluated meniscus injuries associated with chronic ACL injuries and assessed outcomes using the modified Lysholm score. Results: The study revealed a predominant occurrence of meniscus injuries (70%) among participants, with a majority falling within the 38-47 years age bracket (45.45%) and a male majority (60%). Sports and accidental injuries accounted for 40% each, while road traffic accidents contributed 20%. Lateral meniscus tears were the most prevalent (30.30%), followed by combined medial and lateral meniscus tears (33.33%). Posterior horn tears were predominant, particularly in the medial meniscus. Conclusion: The study highlights a high incidence of meniscus injuries associated with chronic ACL tears, with medial meniscus injuries prevailing over lateral tears. Posterior horn tears were predominant, emphasizing the medial meniscus's significant role in knee stability. Objective: The aim is to assess the availability of primary registers in private health centers. Methods and Materials: We carried out a retrospective, prospective and comparative survey on the availability of registers in the third quarter of 2022 and the third quarter of 2023 in private health centers having signed a performance contract with the Koulikoro contracting and verification agency as part of results-based financing. A total of 14 private health centers participated in this study. Results: We note an evolution in the availability rate of registers between the two periods evaluated. The availability of home visit, malnutrition and minor surgery registers was 0% in the third quarter of 2022 and it became respectively 4%, 7% and 80% in the third quarter of 2023. And the levels of some biomolecules like brain-derived neurotrophic factor (BDNF) and acetylcholinesterase (AChE) are known to play significant roles in different neurodegenerative diseases. The present study therefore assessed the ameliorative potential of Co-enzyme Q10 (CoQ10 or Co_Q10) supplementation in altered levels of BDNF and AChE in thioacetamide (TAA) induced hepatic encephalopathy (HE) in male Wistar rat models. Male Wistar rats weighing 250±30 g were used for the study and were randomly separated into six different groups of twenty rats each. The groups included Group 1-control and received 1ml of normal saline (intraperitoneally―ip), Group 2 received only 200 mg/kg TAA (ip) twice weekly, Group 3 received 5mg/kg Co_Q10, daily per oral (po), Group 4 received 10mg/kg Co_Q10 daily (po), Group 5 received 200 mg/kg TAA (ip) twice weekly and 5mg/kg Co_Q10 (po) daily and Group 6 received 200 mg/kg TAA (ip) twice weekly and 10mg/kg Co_Q10 (po) daily. At the end of weeks 3, 6, 9 and 12 of treatment, blood samples were immediately transferred into appropriately labeled blood sample bottles containing anticoagulant. Numerical data obtained from the study were subjected to statistical analyses using analyses of variance and Post Hoc tools of the statistical package for social sciences 21.0V software. The result indicated significantly (P<0.05) depressed level of BDNF and significantly (P<0.05) raised level of AChE in the TAA only treated Group 2 when compared to Group 1 and the rest groups. It was observed that, both of Co_Q10 only treated and Co_Q10 + TAA treated groups had marked (P<0.05) elevated and depressed levels of BDNF and AChE respectively in the study models. There are expedient factors that have been reported in literatures as affecting the utilization of ANC such as religion, education, cultures and traditions of the people, cost etc. Lack of antenatal care and its underuse can result in a variety of unfavorable pregnancy outcomes, including intrauterine growth restriction, low birth weight, and mother and infant death. This study was done to determine the awareness and utilization of ANC in Sokoto State, Nigeria as part of an effort to achieving the millennium development goal. Materials and Methods: This was a descriptive cross-sectional study, adopting the simple random sampling technique and an interviewer administered semi-structured questionnaire. Results and Discussions: The study finds, after assessment, the percentage of Good utilization, Good knowledge and Good Attitude towards ANC services as 52.3%, 72.6% and 67.0% respectively. Crude percentage of utilization of ANC services was 87.0%. Factors influencing utilization identified were marital status and income, with odds ratio of 6.2 (95%CI: 0.88 – 43.39) and 5.4 (95%CI: 1.60 – 18.30) respectively and having p-values of 0.068 and 0.007. Although the women in Sokoto have knowledge of ANC services, their utilization of ANC services is relatively poor. Conclusion: Out of the 87.0% of uptake of ANC, only 52.3% were assessed to have Good utilization score. Cervical cerclage, a primary preventive technique, aims to mitigate these risks by preserving cervical length and structural integrity. Objective: This retrospective study evaluates the effectiveness of cervical cerclage in preventing recurrent mid-trimester miscarriages. Method: Conducted over two years at a private hospital, the study analyzed 92 cases of mid-trimester cervical cerclage. Data included patient demographics, cervical parameters, and pregnancy outcomes. Statistical analysis employed descriptive statistics and odds ratios. Results: Term deliveries (45.65%) predominated over preterm (33.70%) and abortion (20.65%) outcomes. Notably, cervical length inversely correlated with preterm birth risk (odds ratio: 0.25), while cervical dilatation increased preterm birth likelihood (odds ratio: 34). Maternal complications were infrequent, with hemorrhage (3.3%), infection (1.1%), and cervical lacerations (2.1%) reported. Conclusion: Cervical cerclage demonstrates promise in preventing mid-trimester miscarriages, with term deliveries comprising the majority of successful outcomes. However, challenges persist, including the lack of standardized treatment timing and potential complications. Studies have demonstrated that the serum presence of thyroid autoantibodies is correlated with the presence of lymphocytic infiltration and the severity of histological thyroiditis. However, one or more of these antibodies are frequently detected in the general population. Abnormalities in thyroid function and structure are common in the population. Aim of the study: The aim of this study was to evaluate the association between thyroid autoantibodies and abnormalities in thyroid function and structure. Methods: This was a multi-center, cross-section, observational study and was conducted in the Department of Endocrinology of Ibn Sina Medical College Hospital, Dhaka, Bangladesh and Enam Medical College, Savar, Dhaka, Bangladesh during the period from September, 2022 to September, 2023. In our study we included 324 patients with thyroid dysfunction. The patients were selected consecutively attending tertiary care hospitals. The study participants provided demographic and clinical data. Thyroid function, thyroid ultrasonogram and serum concentration of TPOAb, TRAb, and TgAb were measured. Result: A total 324 patients were included in this study. Most of the study population were female (79%) and mean age were 37.0± 12.4 years. 58.6% belonged to the 21-40 age group. The mean BMI was found 22.0±4.5 kg/m2 in positive TRAb and 26.5±5.0 kg/m2 in negative TRAb. 229 (70.7%) were hypothyroid and 95 (29.3%) were hyperthyroid patients. Among the study population 211 (65.1%) patients were TPOAb, 112 (34.6%) were TgAb and 80 (24.7%) were TRAb positive. Among the TPOAb positive patients, 78.7% were hypothyroid and 21.3% were hyperthyroid. On the contrary, among the TRAb positive patients, 97.5% were hyperthyroid. The cephalic index is a significant metric used to ascertain the race and sex of an unidentified individual. Materials and Methods: This study was descriptive and cross-sectional and was conducted on Anambra State students at Madonna University. Elele campus, Rivers State, aged 16–24 years. The cephalic index was determined by applying the following formula: the maximum breadth of the head divided by the maximum length of the head, multiplied by 100. Hundred subjects, comprising 150 males and 150 females from Anambra State at Madonna University, were measured. Anthropometrical parameters measured were maximum head breadth and maximum head length. Results and Discussion: The analysis shows that there is no significant difference in the age groups with the cephalic indices of males and females. The prevalence of hyperbrachycephaly is significantly higher in males and females, while dolicocephaly is observed infrequently. Conclusion: This study shows that the males and females showed no differences on the basis of the cephalic index. It also reveals that the students in this study share similarities with Urhobo males and females based on their head types. Objective: To assess prevalence and risk factor of noise-induced hearing loss among industry workers. Method: This cross sectional study was carried out at tertiary hospital from January 2021 to 2022, Dhaka. Where 97 workers exposed to noise and administrative controls were enrolled in study as a sample population. A structured questionnaire was developed by the expert-review method and applied to all the subjects with face-to-face interviews. Results: Out of 97 sample, 26 workers were having noise induced hearing loss (NIHL). Workers younger than 35 had a 20.59 percent NIHL incidence, while those older than 35 had a 41.38 percent NIHL incidence. The generator area of the plant had the loudest machinery (96-100 dB). As much as 46.67 percent of the generating crew had trouble hearing. Conclusion: We found out that the prevalence of noise induced hearing loss among industry workers. We also found out that workers are overexposed to noise and there is little protection accorded to the workers. It also shows that high noise intensity levels and exposure for long durations leads to hearing loss. As a result, the current therapy of ARM patients is centred on functional results following definitive surgery. Aim of the study: The purpose of this study was to determine the type of ARM and measure functional outcomes, such as voluntary bowel movement (VBM), soiling, and constipation, in our patients following final surgery using the Krickenbeck classification. Methods: The current study is a cross-sectional study to retrospectively review medical records of ARM patients who underwent a definitive surgery at TMSS Medical College, Bogura, Bangladesh from 01 January 2022 to 31 December 2022 in which 45 cases were included. All collected data was entered into a Microsoft Excel Work Sheet and analyzed in SPSS 11.5 using descriptive statistics. Results: A total of 40 paediatric patients underwent hypospadias surgery. Of these, 38 (95%) cases were investigated. Most common type of hypospadias was coronal type (26.3%). Most of our patients did not have any associated urogenital problem (63.2%) and the most common associated anomaly was microphallus (7.9%). Most common surgical techniques was tabularized incised plate (65.8%). Most of our patients did not have any postoperative complications (52.6%) and most common complications was urethrocutanous fistula (31.6%). the presence of severe chordee (AOR=3.09; 95% CI 1.21-7.54; p=0.013) was significantly associated with postoperative problems. Conclusion: Functional outcomes of ARM patients in our institution are considered relatively favourable, with more than half of children showing VBM and just a small number of kids suffering from soiling and constipation.
